Local Donor Champions Unique Program toEngage and Retain Nurses and Social WorkersExpanded Learning and Collaborations Improves Patient Careand Promotes Community Connections“It was fabulous to workwith Program DirectorJennie Wright to have thisprogram bubble up fromthose in the iCMP Programand its practitioners.Newton-Wellesley is acommunity hospital witha welcoming culture. The doctors and nursesare very patient-centered.People smile at you inthe hallways. I decidedto continue to get mycare and to supportNWH because ofthe culture. I am sovery glad that I did.”– Dr. Elizabeth Gardner4Thanks to local resident and generous donorElizabeth Ballantine Gardner, PhD, there isa new program to engage and retain talentedhealthcare professionals. The Elizabeth BallantineGardner, PhD, Scholar Program is now a partof the Integrated Care Management Program(iCMP) at Newton-Wellesley Hospital. The iCMP,a Partners HealthCare Program, coordinatesthe care of chronically compromised patients bymatching these patients with a nurse or socialworker. The Program’s goal is to help medicallycomplex patients stay healthy and out of thehospital by providing the specialized healthcareand services that these high-risk patients need. From Participant To ChampionA two-time cancer survivor with multiplemedical conditions, Dr. Gardner has experiencedthe benefits of the Program first-hand as a Programparticipant. She is quick to credit the iCMPproviders for the pivotal role they play in theirpatients’ lives.Propelled by iCMP’s significant and positiveeffect on her own well-being, Dr. Gardnerdecided to do something meaningful for theiCMP providers at Newton-Wellesley by investingphilanthropically. Working with the CharitableFoundation’s staff to structure an approach, Bethmade a charitable gift to establish the ElizabethBallantine Gardner, PhD, Scholar Award. TheScholar Program will continue for a minimum offive years, benefitting five individual participantswho are selected annually as Scholars.A Unique Program BeginsA professor of biology at Pine Manor College,Dr. Gardner approached the idea of how bestto help the iCMP providers with a scientist’scuriosity and analytical skill. She combined herinterest in the iCMP with her passion for teachingand education, framed around a central question:How can I help? The annual awardto a member of the iCMP team gives protectedtime to identify and work on a healthcare issuethat will help our patients and our community.“The iCMP gives patients an advocate. Butthere are not enough advocates! And it is noteasy to find someone who will be a good fit as anadvocate,” points out Dr. Gardner. “The ScholarProgram gives the iCMP nurse an opportunity togo out and do something that she or he wantsto do. That, in turn, gives another provider anopportunity for someone else to come in andlearn what it is like to be an iCMP nurse. Patientsand the community will benefit, just as I have.”Continued on page 7SUMMER 2017

The Gardner Scholar could support a project to developrelationships and create formal communication channels withcommunity partners to keep and treat patients with heart failure athome, which is the goal.”www.nwh.org/giving/“Positive working relationships among groups help to break downbarriers and improve healthcare for patients,” says Wright. “It thenis easier to connect our patients – our most important priority – withresources for treatment.”To learn more about the Integrated Care Management Program, andhow you can help, please contact Amy Hurley at alhurley@partners.orgor at 617-243-6438.O u r C o mm u n i ty Ou r H o s p i ta l/SUMMER 20177
