Overview of CAP (Waiver Service) Day and Supported Employment service directlyoperated by MHSADS Have been providing day services for over 20 years andemployment services for over 15 years Program has 16 individuals receiving services Currently, program provides both day and employmentservices to 4 individuals and employment only servicesto 1 individual

Overview Continued Licensed by DBHDS 12 full time direct service staff, 1 employment specialist, 1program manager and 4 relief staff Services are provided in the community at a ratio of 1:1for employment Services are person-centered

Criteria for Admission to CAPIndividual must have a Community Living waiverBe a resident of Loudoun CountyBe referred by a Support CoordinatorMust be able to access the community and fully engage inactivities, employment (if requested) and/orvolunteering opportunitiesAbility to be paired with 1 or more other individuals

Admission Criteria ContinuedMust participate in CAP a minimum of 2 days per week.Must undergo a 60 day assessment period to determine ifCAP services are appropriateIf it appears that CAP will not be appropriate for theindividual, other options will be explored

Job Link Services Situational Assessments Job Development Job Placement and Training Follow Along Supports Benefits Counseling Information/liaison services for transitioning LCPS students and their families Referrals to other service providers as needed

Basic Requirements for Job Link ServicesIndividual must want to work and requires the level of support Job Link providesIndividual must be able to be independent in the community when coming to JobLink. Job Link does not provide community training, only employment services.Individual must be willing and able to use public transportation, or othertransportation resources to get to work. Job Link does not provide transportation.Individual must be willing to take ownership of their job and be willing to work withthe Job Link staff.Must meet the current CSB criteria for long term services (MH, ID or DD diagnosis).

Basic Skills Needed for CompetitiveEmployment Know full name, address, phonenumbers Ability to be able to beindependent/appropriate invarious settings Be able to work independentlywithout a 1:1 after initial jobtraining is complete Ability to follow directions fromsupervisors/coworkers Can express their wants and needs Stamina to work for 3 to 4 hourswithout taking a break Have good hygiene practices Be able to take medicationsindependently, if applicable

Role of the Job Coach Listen to individual choices and preferences for job placement Assists in identifying strengths, abilities, and interests of individuals Negotiates customized jobs for individuals Addresses employer and business concerns regarding hiring individuals withdisabilities Identifies a wide variety of workplace supports Documents progress on the job, the effectiveness of workplace supports, anddetermines the fading process from the job site Provides proactive on-going follow along services for the length of employment

General InformationStudents must have a social security card and state issued ID for Job Link toget them a job. Employers are required to obtain this information.Should an individual have a legal guardian, Job Link staff will developemployment opportunities based on the desires and abilities of theindividual served.Individuals can be referred to Job Link through DARS or by contacting JobLink directly to determine eligibility after school eligibility is exhausted.

More Things to KnowJob Link services are paid for by DARS, if DARS refers them to Job Link.Once the DARS funding ends, the individual may continue to receiveservices at no cost at this time. However, the Department may assess amonthly fee in the future.Job Link will assist individuals in obtaining one job. We typically do notassist individuals in finding a second job.Job Link does not develop or provide supports for volunteer positions.Job Link may provide referrals for other services that could be of benefitthe individual.Job Link currently does not have a waiting list.

Discharge From Job LinkJob Link reserves the right to discharge individuals under the followingconditions:- Ignoring the recommendations of the job coach which results intermination- Termination from job due to individuals behavior (theft, failing to goto work, or other behaviors that result in termination by theemployer)- Incarceration- Moving out of Loudoun County- Failure to keep in contact with the Job Coach- No longer meet MHSADS criteria for servicesJob Link will assist individuals as long as they want the service. We haveseveral individuals that have been receiving services for over 25 years.

Job Link StatisticsFY 2016 ( July 1, 2015 thru June 30, 2015) Number of individuals served was115 Total number of individuals that Average number of days onwaiting list: 8 DARS revenues- 50,082.45were employed was 88 (88%). Job Link participants earned Individuals working 90 days was82 (93% of all employedindividuals)approximately 758,822.00 fortax year 2015

Things to Think About Adult services are not mandated. Individuals may have to wait toreceive services and not all services are always available. The current number of people on the wait list for CAP service is 14.At this time, we are unable to determine how the long the wait willbe for individuals wanting CAP services. Individuals on the wait listhave been waiting for over 24 months at this time. This wait list isdetermined by date of application to the program made by SupportCoordinators. Job Link does not have a waiting list and is taking referrals at thistime.
