DenverTulsa, Oklahoma 74103-3832Reference:Fee Proposal for Additional Roof Consulting ServicesTulsa County Annex Building500 South Denver AvenueTulsa, Oklahoma 74119Project No. 201307Dear Mrs. Dorell:As a follow up to our Roof Survey Report & Recommendations dated January 24,2014 we offer ourfirm's services for the Development of Bid Documents, Bidding Phase and Construction Phase Servicesfor reroofing of the Tulsa County Annex Building located at 633 West 3rd Street in Tulsa, Oklahoma.Our sevices would be priced as follows:Development of Bid Documents 10,000 1,500 not including printingBidding Phase Services& mailingPrinting & Mailing expenses are reimbursable 700usually costing approx.Construction Phase Services 9,000Bid Documents can be completed and a Check Set delivered to your office for review within 2 weeks ofacceptance of our fee proposal. A detailed Scope of Work follows for each Phase of our Services. Allwork will be supervised by R. Edward Owen, RRC,AIA,CCS,CCCA a registered architect in the State ofOklahoma and a Registered Roof Consultant with the Roof Consultants Institute. Roof Consultants willmaintain its standard insurance policies through the course of this project and will list Garfield CountyBoard of Commissioners as a Certificate Holder. Any requirement for additional insurance coveragebeyond the standard limits provided by Roof Consultants shall be provided at extra cost. Our billing willbe at the end of each Phase of services or monthly during the construction phase.Once a contract for roof replacement is awarded, we will make daily inspections during the constructionwith written reports and photos. Prepare exterior building elevations for recommended masonry water proofing work.2.No structural, mechanical or electrical engineering modifications are anticipated. If structural,mechanical or electrical engineering modifications are required, the work shall be estimated asAdditional Work on an as needed basis. An additional fee proposal shall be presented for approval priorto performance of additional work.3.Prepare Specifications for roof replacement including sealants, retrofit roof drains and metalflashings and sealants.4Add special Sections as needed for General Conditions, General Requirements, demolition,cutting & patching, flashings & roof accessories and sealants. No hazardous materials are anticipatedother than asbestos roofing felts (non-regulated substance). Hazardous materials if discovered will beidentified to the Owner for testing. Testing, analysis, recommendation for abatement and or disposal isthe responsibility of the building Owner. Special General Conditions & Bidding Requirements forBonds and Insurance will be used as requested by the Client.5.Deliver one copy of a Check Set of the Project Manual with Bidding Requirements, Bid Form,Drawings & Specifications to the Owner's Representative for Review.6.Prepare an estimate of the probable cost of construction for the Owner's review.7.Make final changes required by the Owner's Review.Additional services beyond the above limits such as (redesign, rebid, structural, mechanical, electrical orcode compliance problems beyond the scope of roofing replacement) will be charged at the rates listed onRoof Consultants Hourly Rate Schedule after prior authorization and determination of Additional Scopeof Work and fee estimate.

Printing & mailingexpense of Addenda is not included in Roof Consultants fee and will be billed as a reimbursable expense.5.Roof Consultants will review Bids received by the Owner and will Analyze Bids and will preparea report with recommendations for award of Contracts or rejection of bids.

ROOF CONSULTANTS5350 East 46th Street, Suite 116Tulsa, Oklahoma 74135(918) 660-6844 fax (918) 660-6881January 27,2014Reference:Scope of Work forTulsa County Annex BuildingRoof Replacement633 West 3rd StreetTulsa, OklahomaProject No.: 201307ON-SITE CONSTRUCTION PHASE INSPECTIONStime required (to be determined by work awarded and Contractor's Schedule)1.Upon award of a construction contract the following tasks will be perfonned by RoofConsultants.2.Roof Consultants shall prepare the AlA Agreement fonn between the Owner and the Contractoras acceptable to the Owner and shall review and forward on the Contractor for signature and adding ofthe Contractor's insurance certificates.3.Roof Consultants shall conduct a Pre-Construction Conference with the Contractor,Subcontractors, and the Owner's Representative and Staff as requested.4.Roof Consultants shall review the Contractor's product submittals and shop drawings and shallreturn comments and marked up required corrections to documents to the Contractor and the Owner'sRepresentative.5.Roof Consultants shall conduct daily inspections during roof demolition and critical portions ofthe Work and shall evaluate damage to the existing metal roof deck structure. If damaged deck is foundit shall be included in a report with the quality and quantity of Work in progress and the quantity of UnitPrice Items that constitute extra work for the Contractor. Roof Consultants shall document WorkProgress and decisions made in the field with the Contractor on behalf of the Owner. Reports and photosshall be e-mailed to the Owner and Contractor. Written copies of all Field Reports shall be mailed to allparties concerned.6.Roof Consultants shall prepare Field Orders and/or Change Orders for required constructionchanges and shall e-mail and fax to the Owner's Representative and Contractor for review. Changes inScope of Work shall be billed per our Hourly Rate Schedule as extra work.7.Roof Consultants shall prepare an AlA Certificate of Substantial Completion fonn for the workand issue to the Owner's Representative and the Contractor when the Work has progressed to a point thatthe building is watertight and assist in detennination of the beginning of the Roof Warranty period.8.Roof Consultants shall prepare a Pre-Final Punch List jointly with the Contractor of items ofwork remaining.9.Roof Consultants shall review the Contractor's or Roof Manufacturer's Warrantiy submittal asapplicable.10.Roof Consultants shall conduct a Final Inspection of the Work and shall issue a recommendationto the Owner's Representative to recommend Final Acceptance once work is complete.
