Appointment types40-MINUTE APPOINTMENTS New patient visit: address extensive health history, medications andmultiple health concerns, including chronic conditions Existing patient visit: includes annual physical exams, well woman exams(PAP, pelvic & breast) and time for extended discussions with provider20-MINUTE APPOINTMENTS Acute illness care Follow-up visits for ongoing health concerns, as well as review ofdiagnostic testing results Medication management; any new prescription must be verified by aprovider at the centerSAME-DAY APPOINTMENTS Please do not walk in to the clinic for same-day care without anappointment; this helps ensure there is little to no wait time for any patient For acute illness, please call the clinic to request a same-day appointment;we will do everything we can to fit you into the scheduleTo schedule an appointment, call (260) 373-7524. Please have yourphoto ID and insurance card ready at the time of registration. This willbe updated yearly.Available servicesPRIMARY AND SECONDARY DISEASE PREVENTION AND TREATMENT Diagnosis and treatment of most chronic health conditions Routine screenings (glucose monitoring, blood pressure checks,cholesterol levels, etc.) Routine injections (flu vaccinations) Evaluation and basic medication management for behavioral healthconditions in patients over 21 years of agePREVENTIVE CARE Comprehensive well woman exams Routine physicals (adult, college, sport and camp)ACUTE CARE Treatment of acute, non-life-threatening illnesses, injuries and infectionsLAB TESTING Diagnostic and screeningROUTINE MEDICATION MANAGEMENT AND REFILLS Refills will only be good for six months; you must be seen every sixmonths to continue getting your medication Patients with outside prescription refills will need to establish care atthe FWCS Health and Wellness Center and are subject to the dispensaryguidelines Please allow 24 business hours for medication refillsADDITIONAL SERVICES Castlight Program to assist you with finding the best coverage foradditional testing and specialty services; visit www.CastlightHealth.comor call (866) 970-2317Please note: FWCS Health and Wellness Center does not process FMLA orshort-term disability paperwork.

Click the orange Create Account box. Follow the prompts to fill in your information and confirm that you are “not a robot”. Answer five security questions generated by Precise ID to verify your identity.
