The RN programsare space limited, competitive admission programs. Students who wish to enter an RN program must apply tothe appropriate nursing program after they have met specific admission requirements. Pleasenote: All official transcripts must be received no later than the application deadline.Demonstrate a minimum 2.5 cumulative grade point average (GPA) in the required courses in the1

curriculum that are completed, regardless of where the courses were completed, AND a minimum 2.0cumulative Clark State Community College institutional GPA, OR if no College courses have beencompleted, a high school GPA of 2.5Meet the following minimum academic requirements for admissionObtained a Proficient Level or higher in each individual area tested for the TEAS test. Entryto the nursing program is competitive and based on academic achievements. Applications are reviewed andscored on the criteria as indicated in the following table. Applicants with the highest points will be offeredadmission to the program.If a number of applicants have the same scores, these applicants will be prioritized by their application submissiondate. (Example: 10 students with scores of 13 and only 8 available spots – the 8 students with the earliestapplication date/time will be accepted.) TEAS Testing InformationStarting February 9, 2020, all students applying for admission to the RN program will be expected to completeTEAS testing. The purpose of this requirement is to assist in determining student’s academic preparedness forthe RN program. Students must: Meet the minimum proficiency level in each of the four exams-math, reading, science and English.Failure to meet the minimum proficiency level in one of these examinations will disqualify theapplicant from applying to the RN program. Students may repeat the exam, but after a second unsuccessful attempt, are encouraged toschedule a meeting with an Academic Advisor to discuss alternative career pathways. Students may only take the TEAS exam twice during a calendar year. The second attempt must be30 days after the initial attempt. Students will need to create an ATI account on the www.atitesting.com website. Please select ClarkState Community College as your institution. This must be done prior to taking the exam. Free study materials for the TEAS test are available on the ATI website once you have created youraccount. If you have taken the TEAS test within the last two years for another college, you may have thatreport sent to Clark State Community College by way of the ATI online store. It is the student’sresponsibility, if they wish to have prior TEAS test results (within the last 2 years) used forapplication purposes, to have these results associated with Clark State Community College. There is a fee from ATI Testing associated with taking this exam. Students are expected to pay thatfee to ATI Testing by use of a valid credit or debit card at the time of testing. Students will need toenter this credit card information into the ATI system at the time of testing. The current fee is 65.00 per test. All testing needs to be completed in the testing center on the Leffel Lane Campus (937-328-6049) orat the Beavercreek campus (937-429-8921). Block testing times will be available for this exam.Please consult the Clark State Community College website for dates and times. You will need to signup for a space during these block testing periods. Specific periods for registration for these blocktesting periods will be posted on the Clark State Community College website. If you require testing accommodations because of a disability, your Accommodation Letter must besubmitted to taylorma@clarkstate.edu at least one week prior to the time you wish to test. If youneed to secure disability accommodations, please contact Office of Accessibility Services 937-3286019. Exam results are valid for two years from the time the test was taken. TEAS testing must becompleted prior to the last day applications will be accepted for the application period the studentis applying. For further information regarding TEAS testing as well as registering to take the TEAS test at ClarkState, please go to the Admissions tab located on the Clark State Community College website andclick on Placement Testing.Submission of ApplicationsThe following items must be submitted to the Health, Human, and Public Services Division office in Room 133 ofthe Applied Science Center of the Leffel Lane campus Springfield.Nursing Application form - available online two months prior to the submission deadline -for-space-limited-programs/3

Clark State course listing – log into “myclarkstate” via the Clark State main web page. Click on “Open SelfService,” then select “Student Planning” then “Transcript.” Select download and print. This is the onlyprint out that will be accepted.Transcripts from high school or previously attended regionally accredited colleges that demonstratecompletion of qualifying requirements, course pre-requisites, or completion of curriculum courses.These may be unofficial. NOTE: The corresponding official transcripts must be submitted directly fromthose high school/college(s) to Clark State’s Admissions Office.Incomplete applications and applications submitted after the deadline (see page one for deadlines) will not beconsidered.Notification of Selection into RN ProgramApplicants will be notified of selection, or denial via U.S. mail sent to the address listed on their programapplication form. Students must notify the School of Health, Human, and Public Services of a change in addressand/or phone number. Students who cannot be contacted because of outdated information will be removedfrom the admission list.Selected applicants are required to attend a mandatory Nursing Program Orientation session. The date of thisorientation will be included in the letter notifying the student of their acceptance to the program. Students whoare not able to attend this orientation must notify the School of Health, Human, and Public Services by calling937-328-6057 prior to the orientation date. Selected applicants who do not attend the orientation and do notnotify will have their acceptance rescinded.Students who are not selected for admission are encouraged to meet with their success coach to discuss theselective admissions rubric and their options, including ways to increase their score in preparation forreapplication the following year. ADDITIONAL IMPORTANT RN PROGRAM INFORMATIONOnline and Traditional Classroom FormatsThe non-nursing and non-clinical nursing courses in the RN program curriculum are available in an onlineclassroom format as well as in a traditional onsite format. Students are still required to come to campus forsome labs and testing.Courses with age limitsSome course must have been completed recently in order to meet program entry, progression and graduationrequirements. Other courses are acceptable regardless of when completed. The courses that must becompleted within a designated time period are: BIO 2121 and 2122, Anatomy and Physiology courses, must have been completed within the FIVE years priorto the date the student enters the program. If a course is older than FIVE years, the course will have to berepeated. MST 1181, Nurse Aide Training, must have been completed within the 24 months prior to entering theprogram. If the course is older than 24 months, the course will have to be repeated. Current STNAcertification will also meet the nurse aide requirement. NUR 1110, Dosage Calculation, is the only nursing courses that may be taken prior to acceptance into theregistered nursing program. It is recommended that this course not be taken any earlier than within theyear immediately prior to acceptance into the nursing program. If the course is completed more than 12months prior to starting the program, the course will have to be repeated. Students may elect to take aproficiency exam instead of retaking the course.All other “NUR” courses must be completed in the sequence listed in the curriculum plan. If the student stopsout after starting the sequence, he or she may apply for reinstatement if no more than one year has lapsed inaccordance with the reinstatement policy. Students may be required to demonstrate competency in previouslycompleted courses and some courses may need to be repeated. Reinstatement is on a space available basis andis not guaranteed. Technical StandardsSpecific attributes, characteristics, and abilities are essential to nursing. Professional competency is thesummation of many cognitive, affective, and psychomotor skills. Students who enter the nursing program mustbe able to perform (with or without reasonable accommodations) these technical standards, which are availablevia a link on the Registered Nursing program page on the College’s web site and in the application packet.Students who may require accommodations to perform the techni al standards should contact the College’sOffice of Accessibility to request reasonable accommodations. Students are asked to sign a form certifying thatthey have read, understand, and are able to meet, with or without accommodations, the Technical Standards ofthe Student Nurse as part of the program application process. Attendance at this session is required for allstudents who have been accepted to start the nursing program’s technical courses.Clinical RequirementsHealthcare provider cardiopulmonary resuscitation certification is required at the time of entry into theprogram. Acceptable certifications require a cognitive and psychomotor assessment (written exam andevaluation of performance of skills) and must be those designed specifically for healthcare providers. EMS 1171Basic Life Support, which is offered at Clark State meets this requirement. Certification can also be obtainedthrough the American Heart Association, American Red Cross, and American Safety and Health Institute.Students obtaining certification through these agencies are responsible for obtaining a certification that meetsthe above requirements. Students must maintain active certification throughout the program.Students must also meet health requirements, show proof of health insurance, and meet criminal backgroundcheck requirements before starting the program. Drug screens are also required. Second-year students mustupdate health requirements, criminal background check, and drug screen (if required by clinical agency) inaccordance with program and clinical agency policies.Additional information about these requirements is provided at the nursing program orientation sessions thatare required for all students who have been accepted to start the program. Students must complete theserequirements at their own expense. Students will be billed for liability insurance for each year of clinical coursesas part of a course fee. Please do not submit proof of these requirements with your application. Details onhow and when to submit these documents will be provided at the time of orientation.Background ChecksConviction or guilty pleas to some criminal offenses may prevent students from participating in clinicalexperiences. Reasonable efforts will be made to place admitted students in clinical experiences, but it is up tothe clinical facilities to determine whether a student will be accepted for clinical placement. Students whocannot be placed clinically cannot meet the course and program requirements and therefore would be unable tocomplete the program. Clinical facilities may consider expunged convictions in placement decisions. Standardsmay change during a student’s program of study.Progression RequirementsIn order to enroll in nursing courses and progress to subsequent clinical nursing courses, students must havesuccessfully completed all prerequisite courses with a grade of C or better.6

Graduation RequirementsTo qualify for an associate degree, Registered Nursing students must have a cumulative grade point average of2.0 and have a C or higher in all courses in the curriculum.LicensureUpon completion of the program, graduates are eligible to apply to take the NCLEX-RN examination. Licensure ismandatory for practice as a RN. Candidates for licensure to be a nurse in Ohio must obtain a criminalbackground check and disclose information related to prior misdemeanors or felonies, immoral behaviors,mental health disorders, and illegal use of chemical substances or controlled substances. The Ohio Board ofNursing (OBN) will determine whether a candidate may obtain licensure. Students should refer to the OBN website, tm for more details.
