Message from our MayorA healthy and well community is one thatgives everyone the opportunity to participate,connect and enjoy full and happy lives, with asense of belonging and the ability to cope withadversity.Community spirit is exceptionally high in BassCoast and is well above broader Victorianaverages. We connect with our neighbours,feel overwhelming welcome in ourneighbourhoods, feel safe to walk at night andare more likely to volunteer and be involvedwith a local sporting club. These are all keyelements of a healthy and well community. Werecognise that we can build upon this strongfoundation and the Healthy Communities Plan2021-25 outlines the actions that Council andits partners will take over the next four years,to improve health and wellbeing across theBass Coast community. This Plan reflects theaspirations of our community.We know there are many factors that caninfluence our health and wellbeing: wherewe live, the food we eat, our age, our genderand culture; whether we are employed, ouraccess to transport and healthcare facilities,our level of stress and whether we feel partof the community. This Plan seeks to addressthese broader influences and looks at waysto empower individuals and communities tomake positive decisions about their health.Following extensive consultation with healthservice providers and the community, fourpriorities were established: creating healthylifestyles, improving mental wellbeing,supporting affordable housing and improvingequity to maximise the health, happinessand wellbeing of the community. Theimplementation of this Plan will play a majorrole in ensuring our community can becomeand remain healthy, sustainable, connected,equitable and resilient.We wish to acknowledge the generous andcollaborative approach of South GippslandShire Council and the South Coast Primaryand Community Partnership in developingthis joint Plan. Executive SummaryThe Healthy Communities Plan 2021-25 (the Plan) has been developed in partnershipwith the community and local organisations. The Plan priorities have been developed withconsideration of local health and wellbeing data, the Victorian Public Health and WellbeingPlan 2019 – 2023 and extensive community consultation. The strategic directions of thisplan will lay the framework for the development of an annual action plan; listing partners,detailed activities and initiatives and timelines for delivery.Four priorities have been identified within the plan: creating healthy lifestyles, improvingmental wellbeing, supporting affordable housing, and improving equity.There are many factors influencing our health and wellbeing: the conditions in which we areborn and live in, the food we eat, our age, social status, gender or culture; whether we areemployed, have access to transport and healthcare facilities, our level of stress and whetherwe feel part of the community.A healthy and well community is one where a sense of belonging and ability to cope withadversity exists. It is where economic and social opportunities are available and there is ahealthy environment where we can be protected from the impacts of climate change.This plan addresses the broader influences on health and aims to reduce social inequalitiesincluding access to healthcare and to empower individuals and communities to make positivedecisions about their health.The Healthy Communities Plan has been developed in line with responsibilities under thePublic Health Act 2008, the Local Government Act 2020, the Victorian Public Health andWellbeing Plan 2019-2023 and the Climate Change Act 2017.This Plan aligns with the Council Plan 2021-25 pillar of “Healthy Community” and is supportedby other Council strategies and plans including the Social and Affordable Housing Strategy,the Climate Change Action Plan 2020-30 and the Disability Action Plan 2021-25.6Healthy Communities Plan 2021-25

Integrated PlanningBass Coast Shire Council, South Gippsland Shire Council, South Coast Primary andCommunity Partnership, Bass Coast Health, Gippsland Southern Health Service and SouthGippsland Hospital are working in partnership to develop the Healthy Communities Plan2021-25. Each of our organisations commits to working together to improve the health andwellbeing of the communities of South Gippsland and Bass Coast, also known as the SouthCoast. IntroductionThe Healthy Communities Plan 2021-25 provides the strategic direction for a range oforganisations and Community groups in South Gippsland and the Bass Coast Shire to supportthe health and wellbeing of our communities.Bass Coast Shire Council, South Gippsland Shire Council, South Coast Primary and CommunityPartnership, and the local health services (Bass Coast Health, Gippsland Southern HealthService and South Gippsland Health Service) take lead roles in developing the plan, howeverthe delivery of strategies and actions are in partnership with many organisations and thecommunity. Partnerships are vital to creating environments that support healthy communities.Successful partnerships can take many forms and may include community members, communityorganisations, government and industry organisations. The strategic directions of this plan will lay the foundations for the development of detailedimplementation plans and service plans for health and community services, businesses andthe community.Together we will take a collective impact approach to improving the health and wellbeing ofthe South Coast communities.Many local health, community and Council services have contributed their experience,knowledge and expertise to the development of this plan. Preparing the Healthy Communities Plan2021-25 – Our StoryOur Story – The Community Health and Wellbeing Profile for South Coast is a snapshot ofthe health and wellbeing of our community. Alongside community feedback, it has been usedto shape our combined priorities and actions.Our Story was prepared to assist in the consultation, development and monitoring of localhealth and wellbeing priorities and actions. Data for South Gippsland, Bass Coast and Victoriahas been included. Where local government area data is unavailable, combined or regionaldata has been used. The document has been designed to be live, with updates occuring asnew data becomes available.To accompany the document, we created a five-minute animation of the key health andwellbeing data. Our Story – The Community Health and Wellbeing Profile for South Coastand the five- minute animation are available on Council’s website.Relationship to Community Vision and Council PlanThe Healthy Communities Plan outlines the priority areas that support the implementation ofthe “Healthy Community” pillar of the Council Plan. The aspiration statement for this pillar is:“In Bass Coast, we have access to services and facilities that encourage wellbeing. Tackling Climate ChangeSupporting sustainable living and climate adaptationThe effects of climate change on health are being felt today and have a significantrisk to human health. Health IndicatorsHow do we know we have made a difference?Outcomes reflect the changes we wish to see as a result of working together on theidentified priorities for a healthy community. Outcomes reflect long-term goals for allpartners in the plan and do not relate only to Council or health service actions.Health indicators were developed by matching outcomes with the data sources available.Measuring outcomes relies on having appropriate benchmarks where health partners cantrack progress towards positive change to the health and wellbeing of the community.
