These providers are listed as a resource for those individuals whose primarycare provider is unable or unwilling to prescribe PrEP to HIV negative patients at high risk for becoming infected. These providers are available forPrEP only at the clinic or institution listed, regardless of other affiliations. Only the providers listed have notified us of their willingness to seepatients for PrEP. PrEP prescriptions are at the discretion of the health care provider. Insured patients who have a pharmacy co‐pay can sign up andprint the activated Truvada co‐pay card from the following website: -card. Most patientsusing the co‐pay card will have no out of pocket cost for Truvada . Patients who do not have insurance coverage for medications may be able toobtain medication through Gilead’s Truvada Patient Assistance Program for PrEP by filling out the application via the following lead enrollment form.pdf?utm source truvada prep&utm medium referral. The application requiresa signature from the prescribing physician.Notice to Providers: By electing to be listed on this PrEP Provider Directory, you are indicating that you are willing to: (1) evaluate patients forPrEP; (2) prescribe PrEP to persons considered at high‐risk for HIV infection, when you determine PrEP to be appropriate; and (3) monitor patientson PrEP in accordance with CDC PrEP Guidelines. CDC PrEP guidelines and other recommended resources are available on the HRSB website an-directory/. The purpose of this voluntarydirectory is to enhance our community with available resources for PrEP access to those HIV‐negative individuals at high risk for becoming infected.Please know you may elect to have your name removed from the directory at any time. The Clinician Consultation Center is part of the US Health Resources and ServicesAdministration (HRSA) AIDS Education and Training Centers (AETC) Program, and was made possible through supplemental funding fromCDC.
