FAA MEDXPRESS:Get in the Express Lane to Medical CertificationThe Federal Aviation Administration’s MedXPress system allows anyonerequiring an FAA Medical Certificate or Student Pilot Medical Certificate toelectronically complete their medical application (FAA Form 8500-8).Information entered into MedXPress willbe available to your aviationmedical examiner (AME)to review prior to and atthe time of your medicalexamination, if you providea confi mation number.HOW TO CREATEAN ACCOUNTMedXPress is located at:https://medxpress.faa.gov/First-time users should click on requestaccount and complete the online form.You will be asked to supply your nameand e-mail address; your e-mail addresswill be used as your account namewhen logging into FAA MedXPress.Next, you’ll be prompted to answer threesimple security questions that will be usedto verify your identity if you forget yourpassword or need help with your account.If you have a hand-held device—i-Phone, Smartphone,BlackBerry, etc.— you can fillout the medical applicationform “on the fly” at yourconvenience.After submitting your form, your initial password and instructions for completingthe account request process will be sent to the e-mail address you provided.IMPORTANT: Never use someone else’s MedXPress account toenter your information. Each applicant must create his or her ownaccount. Only one applicant per e-mail address is allowed.OK-12-1676-JAH

HOW TO LOG INOnce you receive e-mail confi mation that your account has been created,you can return to the FAA MedXPress login page and access the securesite using your e-mail address and the temporary password supplied in theconfi mation e-mail. You’ll need to enter a permanent password.APPLICATION FORMUpon signing in to MedXPress, you’ll see the medical application form.Complete the form and when you have finished, enter your password where itindicates I’m done. Send my application to the FAA. Click submit.MedXPress will assign yourapplication a confi mationnumber when it has beensuccessfully submitted.IMPORTANT: Once yousubmit your application,you can no longermodify the application.TIP: If you need to find information about the date of previous exams, medicationsused, or other such information before completing the MedXPress application, you cansave what you have entered thus far and return later to complete any missing data. Clickthe save now button on the bottom of the page and log off. When you log back on, theentries you have made will still be there.OK-12-1676-JAH

CONFIRMATION NUMBERA confi mation number is provided for you upon successfulsubmission of your medical application; this number is alsoe-mailed to you to confi m your application. When you makean appointment with your aviation medical examiner,you may be asked for this number so your AMEcan review your medical information priorto your physical.Take your confi mation number withyou when you arrive for your exambecause your AME will NOTbe able to access your examwithout it.We strongly recommendthat you view and printa copy of the completedform before logging off.MAKE YOURMOVEAfter submitting the electronicapplication form, you have 60 daysin which to schedule and take a physicalexamination with your AME.After 60 days, the application is deleted.PERMANENT RECORDWhen your AME imports your application usingyour confi mation number and saves it in the FAA’smedical certification system, the application isconsidered a permanent record.OK-12-1676-JAH
