DWELLING FORMSummary of Significant Changes, December 31, 20001.Section III. Property Covered, A. Coverage A Building Property, 2.Additions and extensions to buildings that are connectedby a rigid exterior wall, a solid load-bearing interior wall, astairway, an elevated walkway, or a roof may be insuredas part of the dwelling. At the option of the insured, theseextensions and additions may be insured separately.Additions and extensions that are attached to and incontact with the building by means of a common interiorwall that is not a solid load-bearing wall are alwaysconsidered part of the dwelling and cannot be insuredseparately.2.Section III. Property Covered, A. Coverage A Building Property, 3.Coverage for detached carports has been eliminated.3.Section III. Property Covered, B. Coverage B Personal Property 4.Coverage has been extended to include coverage forcooking stoves, ranges, or refrigerators belonging to therenter, as well as 10 percent of contents coverage forimprovements made by the renter to the building.4.Section III. Property Covered, B. Coverage B Personal Property, 5.Coverage for condominium unit owners has beenextended to apply up to 10 percent of the contentscoverage for losses to interior walls, floors, and ceilingsnot covered by the condominium association's masterpolicy.5.Section III. Property Covered, B. Coverage B Personal Property, 6. Special LimitsCoverage for fine arts, collectibles, jewelry, and furs, etc.has been increased to 2500. Also, personal propertyused in a business has been added under this extensionof coverage.6.9.Section IV. Property Not Covered, 14.Coverage for swimming pools, hot tubs and spas (that arenot bathroom hot tubs or spas), and their equipment isnow excluded.10. Section V. Exclusions, B.1. & 2The explanation of when coverage begins as it relates to aloss in progress has been simplified.11. Section V. Exclusions, C.Coverage has been clarified to pay for losses from landsubsidence under certain circumstances. Subsidence ofland along a lake shore or similar body of water whichresults from the erosion or undermining of the shorelinecaused by waves or currents of water exceeding cyclicallevels that result in a flood continues to be covered. Allother land subsidence is now excluded.12. Section V. Exclusions, D.4.b.(3)Coverage is now excluded for water, moisture, mildew, ormold damage caused by the policyholder's failure toinspect and maintain the insured property after the floodwaters recede.13. Section V. Exclusions, D.6.Coverage is now added for damage from the pressure ofwater against the insured structure with the requirementthat there be a flood in the area and the flood is theproximate cause of damage from the pressure of wateragainst the insured structure.14. Section V. Exclusions, F.An exclusion for the cost of testing for or monitoring ofpollutants unless it is required by law or ordinance hasbeen added.Section III. Property Covered, C. Coverage C Other Coverages, 2.a. & b.15. Section VII. General Conditions, G. Reduction andReformation of Coverage, 2.a.(2)Coverage for the two loss avoidance measures(sandbagging and relocation of property to protect it fromflood or the imminent danger of flood) has been increasedto 1,000 for each.If it is discovered before a claim occurs that there isincomplete rating information, the policyholder has 60days to submit the missing rating information. Otherwise,the coverage is limited to the amount of coverage that canbe purchased for the premium originally received and canonly be increased by an endorsement that is subject to theappropriate waiting period (currently 30 days).7.Section IV. Property Not Covered, 5.a. & b.Coverage has been changed to pay for losses to selfpropelled vehicles used to service the described locationor designed to assist handicapped persons provided thatthe vehicles are in a building at the described location.8.Section IV. Property Not Covered, 7.Coverage is now specifically excluded for scrip and storedvalue cards.16. Section VII. General Conditions, V. LossSettlement, 3. Special Loss Settlement, b.(1)Coverage for a manufactured or mobile home or a traveltrailer eligible for replacement cost coverage is limited to1.5 times its actual cash value.

THE FLOOD INSURANCE PROVIDED IS SUBJECT TO LIMITATIONS,RESTRICTIONS, AND EXCLUSIONS.THIS POLICY COVERS ONLY:1.A NON-CONDOMINIUM RESIDENTIAL BUILDING DESIGNED FOR PRINCIPAL USE AS A DWELLING PLACE FORONE TO FOUR FAMILIES, OR2.A SINGLE-FAMILY DWELLING UNIT IN A CONDOMINIUM BUILDING.I. AGREEMENTThe Federal Emergency Management Agency (FEMA)provides flood insurance under the terms of the NationalFlood Insurance Act of 1968 and its amendments, andTitle 44 of the Code of Federal Regulations (CFR).2.Comply with all terms and conditions of this policy;and3.Have furnished accurate information and statements.We will pay you for direct physical loss by or from floodto your insured property if you:We have the right to review the information you give us atany time and to revise your policy based on our review.1.Have paid the correct premium;II. DEFINITIONSA.In this policy, "you" and "your" refer to the insured(s)shown on the Declarations Page of this policy andyour spouse, if a resident of the same household."Insured(s)" includes: Any mortgagee and loss payeenamed in the Application and Declarations Page,as well as any other mortgagee or loss payeedetermined to exist at the time of loss in the order ofprecedence. "We," "us," and "our" refer to the insurer.2.Collapse or subsidence of land along the shore of alake or similar body of water as a result of erosion orundermining caused by waves or currents of waterexceeding anticipated cyclical levels that result in aflood as defined in A.1.a. above.B.The following are the other key definitions that we usein this policy:Some definitions are complex because they are providedas they appear in the law or regulations, or result fromcourt cases. The precise definitions are intended to protectyou.1.Act. The National Flood Insurance Act of 1968 andany amendments to it.2.Actual Cash Value. The cost to replace an insureditem of property at the time of loss, less the value ofits physical depreciation.3.Application. The statement made and signed by youor your agent in applying for this policy. Theapplication gives information we use to determinethe eligibility of the risk, the kind of policy to beissued, and the correct premium payment. Theapplication is part of this flood insurance policy. Forus to issue you a policy, the correct premiumpayment must accompany the application.4.Base Flood. A flood having a one percent chance ofbeing equaled or exceeded in any given year.Flood, as used in this flood insurance policy, means:1.A general and temporary condition of partial orcomplete inundation of two or more acres of normallydry land area or of two or more properties (at leastone of which is your property) from:a.Overflow of inland or tidal waters;b.Unusual and rapid accumulation or runoff ofsurface waters from any source;c.Mudflow.Page 1 of 19

5.Basement. Any area of the building, including anysunken room or sunken portion of a room, having itsfloor below ground level (subgrade) on all sides.6.Building.a.b.c.A structure with two or more outside rigid wallsand a fully secured roof, that is affixed to apermanent site;A manufactured home (a "manufactured home,"also known as a mobile home, is a structure: builton a permanent chassis, transported to its site inone or more sections, and affixed to a permanentfoundation); orA travel trailer without wheels, built on a chassisand affixed to a permanent foundation, that isregulated under the community's floodplainmanagement and building ordinances or laws.Building does not mean a gas or liquid storage tankor a recreational vehicle, park trailer, or other similarvehicle, except as described in B.6.c. above.7.Cancellation. The ending of the insurance coverageprovided by this policy before the expiration date.8.Condominium. That form of ownership of realproperty in which each unit owner has an undividedinterest in common elements.9.Condominium Association. The entity made up ofthe unit owners responsible for the maintenance andoperation of:in a building under a condominium form ofownership.14. Elevated Building. A building that has no basementand that has its lowest elevated floor raised aboveground level by foundation walls, shear walls, posts,piers, pilings, or columns.15. Emergency Program. The initial phase of acommunity's participation in the National FloodInsurance Program. During this phase, only limitedamounts of insurance are available under the Act.16. Expense Constant. A flat charge you must pay oneach new or renewal policy to defray the expenses ofthe Federal Government related to flood insurance.17. Federal Policy Fee. A flat charge you must pay oneach new or renewal policy to defray certainadministrative expenses incurred in carrying out theNational Flood Insurance Program. This fee coversexpenses not covered by the expense constant.18. Improvements. Fixtures, alterations, installations, oradditions comprising a part of the insured dwelling orthe apartment in which you reside.19. Mudflow. A river of liquid and flowing mud on thesurfaces of normally dry land areas, as when earth iscarried by a current of water. Other earth movements,such as landslide, slope failure, or a saturated soilmass moving by liquidity down a slope, are notmudflows.a.Common elements owned in undivided shares byunit owners; and20. National Flood Insurance Program (NFIP). Theprogram of flood insurance coverage and floodplainmanagement administered under the Act andapplicable Federal regulations in Title 44 of the Codeof Federal Regulations, Subchapter B.b.Other real property in which the unit ownershave use rights;21. Policy. The entire written contract between you andus. It includes:where membership in the entity is a required conditionof unit ownership.10. Declarations Page. A computer-generated summaryof information you provided in the application forinsurance. The Declarations Page also describes theterm of the policy, limits of coverage, and displaysthe premium and our name. The Declarations Pageis a part of this flood insurance policy.11. Described Location. The location where the insuredbuilding(s) or personal property are found. Thedescribed location is shown on the DeclarationsPage.12. Direct Physical Loss By or From Flood. Loss ordamage to insured property, directly caused by aflood. There must be evidence of physical changesto the property.a.This printed form;b.The application and Declarations Page;c.Any endorsement(s) that may be issued; andd.Any renewal certificate indicating that coveragehas been instituted for a new policy and newpolicy term.Only one dwelling, which you specifically describedin the application, may be insured under this policy.22. Pollutants. Substances that include, but are notlimited to, any solid, liquid, gaseous, or thermal irritantor contaminant, including smoke, vapor, soot, fumes,acids, alkalis, chemicals, and waste. "Waste"includes, but is not limited to, materials to be recycled,reconditioned, or reclaimed.13. Dwelling. A building designed for use as a residencefor no more than four families or a single-family unitPage 2 of 19

23. Post-FIRM Building. A building for whichconstruction or substantial improvement occurredafter December 31, 1974, or on or after the effectivedate of an initial Flood Insurance Rate Map (FIRM),whichever is later.24. Probation Premium. A flat charge you must pay oneach new or renewal policy issued covering propertyin a community that the NFIP has placed on probationunder the provisions of 44 CFR 59.24.25. Regular Program. The final phase of a community'sparticipation in the National Flood InsuranceProgram. In this phase, a Flood Insurance Rate Mapis in effect and full limits of coverage are availableunder the Act.26. Special Flood Hazard Area. An area having specialflood, or mudflow, and/or flood-related erosionhazards, and shown on a Flood Hazard BoundaryMap or Flood Insurance Rate Map as Zone A, AO,A1-A30, AE, A99, AH, AR, AR/A, AR/AE, AR/AH,AR/AO, AR/A1-A30, V1-V30, VE, or V.27. Unit. A single-family unit you own in a condominiumbuilding.28. Valued Policy. A policy in which the insured and theinsurer agree on the value of the property insured,that value being payable in the event of a total loss.The Standard Flood Insurance Policy is not a valuedpolicy.III. PROPERTY COVEREDA.(1) Only while such work is in progress; orCOVERAGE A - BUILDING PROPERTY(2) If such work is halted, only for a period of upto 90 continuous days thereafter.We insure against direct physical loss by or from floodto:1.The dwelling at the described location, or for aperiod of 45 days at another location as set forth inIII.C.2.b., Property Removed to Safety.2.Additions and extensions attached to and in contactwith the dwelling by means of a rigid exterior wall, asolid load-bearing interior wall, a stairway, anelevated walkway, or a roof. At your option, additionsand extensions connected by any of these methodsmay be separately insured. Additions and extensionsattached to and in contact with the building by meansof a common interior wall that is not a solid loadbearing wall are always considered part of thedwelling and cannot be separately insured.3.4.5.A detached garage at the described location.Coverage is limited to no more than 10 percent of thelimit of liability on the dwelling. Use of this insuranceis at your option but reduces the building limit ofliability. We do not cover any detached garage usedor held for use for residential (i.e., dwelling),business, or farming purposes.Materials and supplies to be used for construction,alteration, or repair of the dwelling or a detachedgarage while the materials and supplies are stored ina fully enclosed building at the described locationor on an adjacent property.A building under construction, alteration, or repair atthe described location.a.If the structure is not yet walled or roofed asdescribed in the definition for building (see II.B.6.a.) then coverage applies:b.However, coverage does not apply until thebuilding is walled and roofed if the lowest floor,including the basement floor, of a non-elevatedbuilding or the lowest elevated floor of anelevated building is:(1) Below the base flood elevation in ZonesAH, AE, A1-A30, AR, AR/AE, AR/AH,AR/A1-A30, AR/A, AR/AO; or(2) Below the base flood elevation adjusted toinclude the effect of wave action in Zones VEor V1-V30.The lowest floor levels are based on the bottomof the lowest horizontal structural member of thefloor in Zones VE or V1-V30 and the top of thefloor in Zones AH, AE, A1-A30, AR, AR/AE,AR/AH, AR/A1-A30, AR/A, AR/AO.6.A manufactured home or a travel trailer as describedin the Definitions section (see II.B.6.b. and II.B.6.c.).If the manufactured home or travel trailer is in aspecial flood hazard area, it must be anchored inthe following manner at the time of the loss:a.By over-the-top or frame ties to ground anchors;orb.In accordance with the manufacturer's specifications; orc.In compliance with the community's floodplainmanagement requirements;Page 3 of 19

unless it has been continuously insured by the NFIPat the same described location since September 30,1982.7.The following items of property which are coveredunder Coverage A only:a.b.c.d.e.Awnings and canopies;Blinds;Built-in dishwashers;Built-in microwave ovens;Carpet permanently installed over unfinishedflooring;f. Central air conditioners;g. Elevator equipment;h. Fire sprinkler systems;i. Walk-in freezers;j. Furnaces and radiators;k. Garbage disposal units;l. Hot water heaters, including solar water heaters;m. Light fixtures;n. Outdoor antennas and aerials fastened tobuildings;o. Permanently installed cupboards, bookcases,cabinets, paneling, and wallpaper;p. Plumbing fixtures;q. Pumps and machinery for operating pumps;r. Ranges, cooking stoves, and ovens;s. Refrigerators; andt. Wall mirrors, permanently installed.8.(14) Water softeners and the chemicals in them,water filters, and faucets installed as anintegral part of the plumbing system;(15) Well water tanks and pumps;(16) Required utility connections for any item inthis list; and(17) Footings, foundations, posts, pilings, piers,or other foundation walls and anchoragesystems required to support a building.Items of property in a building enclosure below thelowest elevated floor of an elevated post-FIRMbuilding located in Zones A1-A30, AE, AH, AR,AR/A, AR/AE, AR/AH, AR/A1-A30, V1-V30, or VE, orin a basement, regardless of the zone. Coverage islimited to the following:a.b.B. COVERAGE B - PERSONAL PROPERTY1.If you have purchased personal property coverage,we insure against direct physical loss by or fromflood to personal property inside a building at thedescribed location, if:a.The property is owned by you or your householdfamily members; andb.At your option, the property is owned by guestsor servants.Personal property is also covered for a period of 45days at another location as set forth in III.C.2.b.,Property Removed to Safety.Personal property in a building that is not fullyenclosed must be secured to prevent flotation out ofthe building. If the personal property does float outduring a flood, it will be conclusively presumed that itwas not reasonably secured. In that case there is nocoverage for such property.2.Any of the following items, if installed in theirfunctioning locations and, if necessary foroperation, connected to a power source:(1) Central air conditioners;(2) Cisterns and the water in them;(3) Drywall for walls and ceilings in a basementand the cost of labor to nail it, unfinished andunfloated and not taped, to the framing;(4) Electrical junction and circuit breaker boxes;(5) Electrical outlets and switches;(6) Elevators, dumbwaiters, and related equipment, except for related equipment installedbelow the base flood elevation afterSeptember 30, 1987;(7) Fuel tanks and the fuel in them;(8) Furnaces and hot water heaters;(9) Heat pumps;(10) Nonflammable insulation in a basement;(11) Pumps and tanks used in solar energysystems;(12) Stairways and staircases attached to thebuilding, not separated from it by elevatedwalkways;(13) Sump pumps;Clean-up.Coverage for personal property includes the followingproperty, subject to B.1. above, which is coveredunder Coverage B only:a.b.c.d.e.f.g.3.Air conditioning units, portable or window type;Carpets, not permanently installed, overunfinished flooring;Carpets over finished flooring;Clothes washers and dryers;"Cook-out" grills;Food freezers, other than walk-in, and food inany freezer; andPortable microwave ovens and portabledishwashers.Coverage for items of property in a buildingenclosure below the lowest elevated floor of anelevated post-FIRM building located in Zones A1A30, AE, AH, AR, AR/A, AR/AE, AR/AH, AR/A1-A30,V1-V30, or VE, or in a basement, regardless of thezone, is limited to the following items, if installed intheir functioning locations and, if necessary foroperation, connected to a power source:a.b.c.Air conditioning units, portable or window type;Clothes washers and dryers; andFood freezers, other than walk-in, and food inany freezer.Page 4 of 19

4.If you are a tenant and have insured personalproperty under Coverage B in this policy, we willcover such property, including your cooking stove orrange and refrigerator. The policy will also coverimprovements made or acquired solely at yourexpense in the dwelling or apartment in which youreside, but for not more than 10 percent of the limit ofliability shown for personal property on theDeclarations Page. Use of this insurance is at youroption but reduces the personal property limit ofliability.2.Loss Avoidance Measuresa.Sandbags, Supplies, and Labor(1) We will pay up to 1,000 for costs you incurto protect the insured building from a floodor imminent danger of flood, for thefollowing:(a) Your reasonable expenses to buy:(i)5.6.7.If you are the owner of a unit and have insuredpersonal property under Coverage B in this policy,we will also cover your interior walls, floor, and ceiling(not otherwise covered under a flood on) for not more than 10 percent of the limitof liability shown for personal property on theDeclarations Page. Use of this insurance is at youroption but reduces the personal property limit ofliability.(ii) Fill for temporary levees;(iii) Pumps; and(iv) Plastic sheeting and lumber used inconnection with these items.(b) The value of work, at the Federalminimum wage, that you or a member ofyour household perform.Special Limits. We will pay no more than 2,500 forany one loss to one or more of the following kinds ofpersonal bilia, including but not limitedporcelain or other figures, and sports cards;b.Rare books or autographed items;c.Jewelry, watches, precious and semipreciousstones, or articles of gold, silver, or platinum;d.Furs or any article containingrepresents its principal value; ore.Personal property used in any business.fur(2) This coverage for Sandbags, Supplies, andLabor applies only if damage to insuredproperty by or from flood is imminent, andthe threat of flood damage is apparentenough to lead a person of commonprudence to anticipate flood damage. Oneof the following must also occur:orto,(a) A general and temporary condition offlooding in the area near the describedlocation must occur, even if the flooddoes not reach the insured building; orwhich(b) A legally authorized official must issuean evacuation order or other civil orderfor the community in which the insuredbuilding is located calling for measuresto preserve life and property from theperil of flood.We will pay only for the functional value of antiques.C. COVERAGE C - OTHER COVERAGES1.Debris Removala.We will pay the expense to remove non-owneddebris on or in insured property and owneddebris anywhere.b.If you or a member of your household performthe removal work, the value of your work will bebased on the Federal minimum wage.c.This coverage does not increase the Coverage Aor Coverage B limit of liability.Sandbags, including sand to fillthem;This coverage does not increase the Coverage Aor Coverage B limit of liability.b.Property Removed to Safety(1) We will pay up to 1,000 for the reasonableexpenses you incur to move insured propertyto a place other than the described locationthat contains the property in order to protectit from flood or the imminent danger offlood.Page 5 of 19

(a) 80 percent or morereplacement cost; orReasonable expenses include the value ofwork, at the Federal minimum wage, that youor a member of your household perform.(6) To the extent that payment under this policyfor a condominium building loss, incombination with any recovery available toyou as a tenant in common under any NFIPcondominium association policies for thesame building loss, exceeds the amount ofinsurance permitted under the Act for asingle-family dwelling.This coverage does not increase the Coverage Aor Coverage B limit of liability.Loss assessment coverage does not increase theCoverage A limit of liability.Condominium Loss AssessmentsIf this policy insures a unit, we will pay, up to theCoverage A limit of liability, your share of lossassessments charged against you by thecondominium association in accordance withthe condominium association's articles ofassociation, declarations and your deed.D.COVERAGE D - INCREASED COST OF COMPLIANCE1.GeneralThis policy pays you to comply with a State or localfloodplain management law or ordinance affectingrepair or reconstruction of a structure suffering flooddamage. Compliance activities eligible for paymentare: elevation, floodproofing, relocation, or demolition(or any combination of these activities) of yourstructure. Eligible floodproofing activities are limitedto:The assessment must be made as a result ofdirect physical loss by or from flood during thepolicy term, to the building's common elements.b.We will not pay any loss assessment chargedagainst you:(1) And the condominium association by anygovernmental body;(2) That results from a deductible under theinsurance purchased by the condominiumassociation insuring common elements;(3) That results from a loss to personal property,including contents of a condominiumbuilding;(4) That results from a loss sustained by thecondominium association that was notreimbursed under a flood insurance policywritten in the name of the association underthe Act because the building was not, at thetime of loss, insured for an amount equal tothe lesser of:full(5) To the extent that payment under this policyfor a condominium building loss, incombination with payments under any otherNFIP policies for the same building loss,exceeds the maximum amount of insurancepermitted under the Act for that kind ofbuilding; orAny property removed, including a moveablehome described in II.B.6.b. and c., must beplaced above ground level or outside of thespecial flood hazard area.a.its(b) The maximum amount of insurancepermitted under the Act;(2) If you move insured property to a locationother than the described location thatcontains the property, in order to protect itfrom flood or the imminent danger of flood,we will cover such property while at thatlocation for a period of 45 consecutive daysfrom the date you begin to move it there. Thepersonal property that is moved must beplaced in a fully enclosed building orotherwise reasonably protected from theelements.3.of2.a.Nonresidential structures.b.Residential structures with basements thatsatisfy FEMA's standards published in the Codeof Federal Regulations [44 CFR 60.6 (b) or (c)].Limit of LiabilityWe will pay you up to 30,000 under this Coverage D- Increased Cost of Compliance, which only applies topolicies with building coverage (Coverage A). Ourpayment of claims under Coverage D is in addition tothe amount of coverage which you selected on theapplication and which appears on the DeclarationsPage. But the maximum you can collect under thispolicy for both Coverage A - Building Property andCoverage D - Increased Cost of Compliance cannotexceed the maximum permitted under the Act. We donot charge a separate deductible for a claim underCoverage D.Page 6 of 19

3.Eligibilitya.elevations are being increased, and a flooddamaged structure must comply with thehigher advisory base flood elevation.)Increased Cost of Compliance coveragedoes not apply to situations in B, C, X, or Dzones where the community has derived itsown elevations and is enforcing elevation orfloodproofing requirements for flooddamaged structures to elevations derivedsolely by the community.A structure covered under Coverage A - BuildingProperty sustaining a loss caused by a flood asdefined by this policy must:(1) Be a "repetitive loss structure." A repetitiveloss structure is one that meets the followingconditions:(a) The structure is covered by a contract offlood insurance issued under the NFIP.(3) Elevation or floodproofing above the baseflood elevation to meet State or local"freeboard" requirements, i.e., that astructure must be elevated above the baseflood elevation.(b) The structure has suffered flooddamage on two occasions during a 10year period which ends on the date ofthe second loss.c.Under the minimum NFIP criteria at 44 CFR 60.3(b)(4), States and communities must require theelevation or floodproofing of structures inunnumbered A zones to the base flood elevationwhere elevation data is obtained from a Federal,State, or other source. Such compliance activitiesare also eligible for Coverage D.d.This coverage will also pay for the incrementalcost, after demolition or relocation, of elevating orfloodproofing a structure during its rebuilding atthe same or another site to meet State or localfloodplain management laws or ordinances,subject to Exclusion D.5.g. below.e.This coverage will also pay to bring a flooddamaged structure into compliance with State orlocal floodplain management laws or ordinanceseven if the structure had received a variancebefore the present loss from the applicablefloodplain management requirements.(c) The cost to repair the flood damage, onaverage, equaled or exceeded 25percent of the market value of thestructure at the time of each flood loss.(d) In addition to the current claim, the NFIPmust have paid the previous qualifyingclaim, and the State or community musthave a cumulative, substantial damageprovision or repetitive loss provision inits floodplain management law orordinance being enforced against thestructure; or(2) Be a structure that has had flood damage inwhich the cost to repair equals or exceeds50 percent of the market value of thestructure at the time of the flood.
