CCAC Coordinator completes EvaluatorQuestionnaire and determines a client isincapable of making LTC decisions. 2. CCAC Coordinator determines that thereare no other Substitute Decision Makerswho could make LTC decisions on theclient’s behalf.3. Complete the CCAC PGT Confirmation Form Any Power of Attorney documentation must be explored and exhaustedAll possible family or other Substitute Decision Maker’s must be exhausted anddetermination that there is no one who can make decisions on the client’s behalf Complete the full application and all fields – including current location of the clientat the time of the applicationProvide any names of contacts such as friends even if not considered being an SDM– PGT may be able to obtain information about the client’s wishes, values and beliefto assist with decision makingClear contact information of the CCAC Coordinator is required for communicationand follow upThe CCAC Coordinator who is familiar with the client should be the contact personfor follow up 4. Gather all necessary documents for faxingto the PGT office. The TDC will contact the Coordinator to confirm receipt and discuss any gaps orissuesConfirmation of when the TDC will meet with the client to allow for a decisionrelated to LTC 7. TDC visits the client and makes LTCdecisions On site visit is made with the client and LTC decisions are made supported bydocumentationCompleted choice sheet and consent will be provided by PGTCoordinator may be involved during or after the meeting with the client.8. Coordinator forwards the application & Coordinator forwards the application as per the local process to the LTCHs selected9. Ongoing monitoring and communication bythe Coordinator to the PGT. Office of the Public Guardian & TrusteeProperty / Finances Decisions ‐ Application Process:Should an individual be a patient in hospital for the care, treatment or observation of a mental disorder and a physician finds them to beincapable of managing their finances; the PGT must be advised of this as per Sec 15 of the Substitute Decisions Act. The PGT would thenbecome the guardian of property.For all other individuals, if it is suspected that the individual may be incapable of managing their finances, a capacity assessment can berequested. If the individual is found to be incapable by a capacity assessor, the PGT will become the guardian of property.A list of names of capacity assessors may be obtained from the:Capacity Assessment Office:April 4, 2011, JG&JS(416) 327‐6766 or 1‐800‐521‐10335

Office of the Public Guardian & TrusteeStandards for PGT Response Times (TDCs)1. Treatment Decision Consultants (TDCs) will contact the CCAC Coordinator immediately upon receipt of the application. Any gaps ininformation or issues will be discussed during this time. A scheduled client visit will be confirmed during this discussion.2. The TDCs will see the client’s face to face for LTC decisions within 7 business days post receipt of the application.3. In exceptional circumstances where a face‐to‐face visit cannot be completed within 7 days, the PGT will contact the Coordinator toarrange for Long Term Care Home selection and decision making remotely. This is at the discretion of the TDC and is related tosignificant delays most likely due to geography.4. If the TDC does not see the patient or respond by day 7, the Coordinator will contact the PGT Treatment Decisions Unit for escalation(416‐314‐2788 or 1‐800‐387‐2127).NOTE: In more rural jurisdictions where travel to clients may be lengthy, the response times may be longer. TDCs will communicatetimeframes with the CCAC Coordinator for client visits.Standards for CCAC Coordinators1. When assessing capacity using the Evaluator Questionnaire, all responses to questions must be clearly documented to substantiate adetermination of incapacity.2. Clients deemed incapable must be informed of their finding of incapacity and provided with the Rights Information Sheet.3. The Coordinator must ensure that all possible Substitution Decision Makers are explored and exhausted before applying to PGT.4. The full PGT application package must be completed and faxed to the PGT office.5. If the TDC does not visit the client within 7 business days, the Coordinator will escalate the issue to the PGT Treatment & DecisionsUnit for intervention.April 4, 2011 , JG&JS6
