About ESIC!ESI Corporation (ESIC) is a statutory organization established under ESIC Act 1948. ESICprovides various benefits to its beneficiaries who are registered through their employers.Out of different benefits being provided, Medical Benefits (MB) are delivered through achain of ESIC / ESI Scheme (ESIS) run 155 hospitals, 1300 dispensaries and 950 empaneledInsurance Medical Practitioners (IMPs). Secondary/Tertiary medical services which are notavailable in ESIC/ESIS institutions are provided cashless through tie up arrangement withmore than 1000 public/ private hospitals.II.How many beneficiaries are enrolled in the ESI Scheme?ESIC caters to about 3.4 Crore Insured Persons (IPs) who pay contributions to the ESIScheme, every month through their Employers. Considering also the families of these IPsabout 13 crore beneficiaries are enrolled for receiving services at present.III. What is the old IMP Scheme?In an area where ESI does not have its medical establishment the Primary Medical Care isprovided to beneficiaries, through an arrangement with the locally available Private medicalPractitioner called Insurance Medical Practitioner (IMP) against defined remuneration. TheIMP is required to provide consultation services to each of the family members of theInsured Person of ESI Scheme. The IMP also needed to carry out certain basic LaboratoryTests and distribute certain medicines within the remuneration defined (i.e. Rs. 500/per IPFamily/Year).IV.What is a mIMP (Modified IMP) Scheme?In an area where ESI does not have its medical establishment, or in newly implementedarea, Primary Medical Care is provided cashless, through tie up arrangement with InsurancePage 1 of 18

Medical Practitioner (IMP). An IMP is a private Medical Practitioner who is registered withESI to provide medical treatment to the Insured Person & their Family members againstdefined remuneration. The selected Private Medical Practitioner shall be henceforth knownas the registered ‘Insurance Medical Practitioner’ for ESIC. He shall be free to continue withhis private practice but shall have to offer cashless priority services to the attached IP or hisfamily members. The IMP shall be selected based on the rebate he should offer on theupper ceiling limit is Rs.500/per IP Family / Year. He shall only provide consultation servicesand not required to carry out any lab test or distribute medicine against this remuneration.Non-attached IP or his family member may be treated as a non-IP or private patient. Also,the new concept of empaneled Chemist and empaneled Diagnostic Centre, hitherto notavailable in the old IMP Scheme shall be part of the mIMP scheme for cashless services.V.How is an IMP under the mIMP Scheme empanelled?An Expression of Interest (EOI) / Notification will be floated in its website (www.esic.nic.in)or otherwise, by the Regional Director of ESIC Regional Office, available in each State/ UnionTerritory, for the area where IMPs are required.VI.How is an IMP under the mIMP Scheme selected?Private Medical Practitioners (Doctors) not attached to any organization with Degree inModern System of Medicine (MBBS - Allopathy) from recognized Medical Colleges of Indiashall be eligible to participate in the mIMP Scheme. Additional qualifications such asMS/MD/Diploma, etc., shall be desirable but not mandatory. Desirous eligible PrivateMedical Practitioners shall be applying online / as directed in the notification furnishingrelevant details, for scrutiny and selection by the competent authority. Selection shall be onthe basis of Merit, Educational qualifications, Experience, Proximity to desired location,Reputation, Rebate/discount offered on the upper prescribed limit set by ESIC, Facility inprivate Clinic, Approachability, Timings of clinics vis-à-vis suitability/requirement by ESIC andother objective factors as decided by authorities. Depending upon the number of IPs in anarea, more IMPs may be appointed by the Regional Office of ESIC.VII. How much remuneration is paid to an IMP under the mIMP Scheme?The IMP shall be remunerated the agreed upon amount subject to ceiling limit of Rs. 500/-,per IP-Family Unit, per year, for providing medical treatment/consultation to an attached IPand their dependent family members. However, the amount payable is irrespective ofnumber of consultations / treatment provided to / or count of episodes of sickness forwhich attended to the IP-Family unit. One IMP shall be entitled to receive remunerationupto 2000 IP family-units attached to his/her jurisdiction, i.e., remuneration shall be madeas per actual number of IP-Family Units attached, or 2000 IP-Family Units, whichever is less.1) Example 1: At most, One IMP shall be eligible for upto 2000 x Rs.500 Rs. 10 Lakh peryear, if on an average 2000 or more IP-Family Units are tagged to him. This shall beirrespective of whether these IP-Family Units shall fall sick and seek medical help orPage 2 of 18

not. Moreover, this shall be an added income to the already practicing doctor withoutrequiring to invest in kind or cash, extra.2) Example 2: Similarly, If on an average 200 IP-Family Units are tagged, the IMP shall beeligible for upto 200 x Rs.500 Rs. 1 Lakh per year.VIII. How the payment of remuneration is made?ESIC will reimburse the remuneration to IMP on monthly or quarterly (every three months)basis against the claim raised by IMP based on number of eligible IPs tagged to him/her.Since attachment of IP Units to IMP is highly dynamic, a common rule shall be applied toarrive at the figure. The number of eligible IP-Family Units attached to the said IMP shall bedecided as average of numbers attached on the first days of current and subsequentmonths, or 2000, whichever is less, for a month for the purpose of payment. However, thisshall be governed by the other terms and conditions as in the MoU/agreement and in caseof any discrepancy, the contents of the MoU shall be final and binding. This payment will bemade on-line through ECS by Dispensary cum Branch Office (DCBO) in the district/ nearestBranch office. Hence Bank Details are mandatory while registering as IMP.IX. Who shall disburse the payment to IMP?IMP shall submit the claim monthly or quarterly for reimbursement/remuneration as peragreed terms on a specified format issued by Regional Office / DCBO.X.How an IP-family unit is tagged to an IMP?Tagging of IP to an IMP or Dispensary shall be done by the Employer/ESI Office/Dispensaryor by IP through IP portal, subject to a limitation of number of times the same can betagged/ de-tagged. An IP can be attached to an IMP and his family members can also beattached to the same IMP. However, if an IP is attached to an IMP, his family memberscannot be attached to other IMP, but to a dispensary or DCBO. Irrespective of an IP Familyattached to any IMP or dispensary, the IP-Family shall also be eligible to visit a DCBO of thesame district and for availing administrative and some specified services.XI. What is an empanelled Chemist (eC)?A private or public local chemist shop/pharmacy of a defined service area registered by ESICfor serving Insured Persons (IPs) is called an empanelled Chemist (eC). The chemistshop/pharmacy should be complying with the statutory rules, regulations and licenses.XII. What is an empanelled Diagnostic Center (eDC)?A private or public local Diagnostic Center of a defined service area registered by ESIC forserving Insured Persons (IPs) is called an empanelled Diagnostic Center (eDC). TheDiagnostic Center should be complying with the statutory rules, regulations and licenses.The Diagnostic Center having facility for Lab., X-ray, ECG, etc. are preferred.Page 3 of 18

XIII. How is an empanelled Chemist (eC) selected?ESIC Regional office in coordination with DCBO/BO will empanel locally available JanAushadhi Kendra, or a local Chemist Shop based on discount offered on MRP of drugs,through EOI/Notification. Interested shops/establishments shall apply online through thespecified Web App (www.esic.nic.in) for enrolment. Selection of suitable Chemist shall bebased on various factors such as suitability, service availability, proximity to a registered IMPor desired location, discount offered, conformity to statutory licenses, laws andauthorizations, quality of drugs, etc.XIV.How is an empanelled Diagnostic Center (eDC) selected?ESIC Regional Office in coordination with DCBO/BO will empanel a local Diagnostic Centrefor basic investigations based on discount offered on CGHS rates for such tests, throughEOI/Notification. Interested shops/establishments shall apply online through the specifiedWeb App (www.esic.nic.in) for enrolment. Selection of suitable Diagnostic centre shall bebased on various factors such as suitability, service availability, proximity to a registered IMPor desired location, discount offered, conformity to statutory licenses, laws andauthorizations, quality of diagnostic services, etc.XV.How much remuneration is paid to eC/eDC?There shall not be any remuneration. Once enrolled, the eCs and eDCs shall have additionalbusiness with no extra investment. There shall be no capping on number of IP-Family unitsthat can be tagged to each; hence business potential shall increase many folds.XVI.How the eC/eDC are tagged to an IMP?The Regional Office/DCBO, at the time of registering an eC or eDC, shall be tagging an IMPClinic so as to help beneficiaries avail services from designated centres. More than one eCand/or eDC can be attached to each IMP Clinic or vice versa to bring in fairness andcompetitiveness.XVII. Whom to approach in case of grievances?Normally the DCBO Medical Officer in Charge shall be able to redress any issue or grievanceand provide guidance on Rules / Regulations. However, if not resolved within reasonabletime, the same can be escalated to the Regional Director in the ESIC Regional Office of theState / Union Territory. IMP may Download ‘UMANG – ESIC Chinta Se Mukti” Mobile appfrom Google Play store for gathering certain additional information.XVIII. What is a Health Passbook?A Health Passbook is a small booklet of A5 size approx. containing about 30 leaflets withsystem generated beneficiary credentials affixed on it. This booklet needs to be carried bythe beneficiary every time he visits a doctor. The booklet serves as a tool for identifying thebonafideness of the ESIC beneficiary and meant for recording clinical findings andPage 4 of 18

consultation advices by the doctor. The Booklet can be obtained from open market by theIP or shall be provided by the Employer. The credentials generated from the system containthe demographic details of a member of the IP-family and Unique Health Identification(UHID) Number. In addition it contains e-mail Id, mobile no., passport size colouredphotograph and QR Code.XIX.How a Health Passbook is issued to a beneficiary?Along with the ESI e-Pehchaan Card issued to the IP / Insured Women (IW), the Employershall also provide a printout of system generated Health Passbook containing Credentials, toeach of the family members of the IP, at the time of Registration of IP and/or familymembers (i.e. each IP-Family unit shall have one common e-Pehchaan card but eachmember of the family including IP shall have separate Unique Health Passbook containingUnique Health Id). In due course of time, a provision shall be made so as to empower the IPto print Health Passbook for himself / Family Members from IP Portal at www.esic.in or itcan be issued by the officials in ESI Dispensary/ Hospital/ Branch Office/ DCBO. TheEmployer will be responsible for mapping and entering IPs and family members credentials(UHID & Mobile No., etc.) into the system and takes out the print. A photograph of thebeneficiary needs to be affixed on the QR Coded Health Passbook and to be countersignedby the issuing authority. Similarly, if the health passbook credential is issued by the ESIDispensary/ Hospital/ Branch Office/ DCBO, the same process is to be followed.XX.What is the validity of Health Passbook?Normally, the validity of the Health Passbook is till the last day of the current eligible BenefitPeriod corresponding to the previous Contribution Period. A new Health Passbook needs tobe issued within upto 28 days prior to the expiry of the validity date on the request of thebeneficiary and to maintain continuity and prevent hassles.XXI.How to access DHAWNATRI App?Once selected and registered successfully, the IMP shall be provided with user credentialsthrough valid e-mail ID and/or working Mobile number to access Dhanwantri App and workfor ESIC. IMP has to download ESIC Dhanwantri Mobile App in his/ her Android smart phonefrom Google Play store & log-in into it.XXII. How to Register an IP in the IMP clinic?At the time of visit of bonafide IP/beneficiary, subject to prior tagging of IPs & family, theIMP will enter the Insurance/UHID Number of the beneficiary to check eligibility andproceed further to capture certain clinical information including provisional ailments fromInternational classification of disease (ICD) in drop box as given in the App, based oncomplaints and examination of patient. This shall be over and above the prescriptions to bewritten on the Health passbook (a booklet with patient credentials with QR code pasted onit), manually.Page 5 of 18

XXIII. How to make a prescription using the DHANWANTRI App & Health passbook?IMP will prescribe medicines in the Health passbook manually as per prescribed medicinelist, basic investigations and also capture corresponding basic information in the MobileApp. This includes capturing of information about the count of drug or investigationprescribed. When medicines or investigations are prescribed from the specified list, thebeneficiary gets the services cashless from the empanelled Chemist (eC) or Diagnosticcentre (eDC). The medicines are to be prescribed in standard Generic Name, other than inexceptional or unavoidable circumstances. Prescribing in brand names is highly discouragedfor administrative and clinical reasons. All drug and test names are to be prescribed in clearlegible writing, preferably in capital letters. The consequences of ineligible prescriptionwriting shall be that of the IMP.XXIV.How to issue Medical/Regulation Certificates to IPs?Through Mobile App the IMP shall capture information about Regulation / “SicknessCertificate”, “Maternity Certificate” and other stipulated Certificates that has beenprescribed in the specified Certificate Booklet provided/issued by DCBO/ RD Office.XXV.What are Medical/Regulation Certificates?Sickness/Maternity certificate, unlike ordinary medical certificate is a highly regulateddocument based on defined logic and nomenclature. Issuance of Sickness/MaternityCertificate needs knowledge about ESI Scheme rules and must be learnt from the ESICOffice. Misuse or wrong use shall bring in complications and legal /administrativeconsequences. Issuance of regulated Sickness / Maternity Certificate is permissible toInsured Person (including Insured Women) only and not to the Family Members. However,normal medical certificate may be issued on the letterhead of the IMP to family members,which has no bearing on cash benefits and liability on ESIC. Issuance of Sickness / Maternitycertificate shall be subject to remote verification by Medical Referee/ DCBO doctor, or otherofficer of ESIC/ESIS. The resulting Cash benefit, if any shall be directly deposited in the bankaccount of the IP and shall be regulated. Issuance of lax/bogus certification by IMP is acriminal offence and all certifications shall be under constant scrutiny.XXVI.What to do in case of Occupational Disease and Employment Injury?The IMP shall also capture information in the Dhanwantri app about a suspectedOccupational Disease (OD) (as per the definition of Occupational Disease) and EmploymentInjury (EI) (As per definition of Injury sustained during in-job performance), over and abovedocumenting the same in the health passbook. A prior knowledge on OD and EI as per ESIContext is not only essential but also has legal and administrative consequences.Page 6 of 18

XXVII. How a patient can be referred?In case referral is required to higher centre (except in emergency) for secondary or tertiarymedical care, IMP has to refer the patient to Dispensary cum Branch Office (DCBO) of thedistrict or nearest ESI Hospital for further necessary action. The same has to be recorded inthe Health Passbook and also in the mobile App in the remarks field.XXVIII.What is to be done in case an unlisted drug/diagnostic test is required to beprescribed?In case of medicines/lab tests is required to be prescribed outside the prescribed list by IMP,the same is to be captured in Health Passbook as well as in the corresponding fields in theMobile App.XXIX.What if an unlisted drug/diagnostic test is prescribed by IMP?In this case, the patient has to visit DCBO for availing medicines /services or he has anoption to procure from the empanelled chemist/diagnostic centre paying outright from hispocket. Cosmetic items, food supplements and unnecessary vitamin supplementations arenot to be prescribed as these shall not be reimbursed. Later on he may visit DCBO forreimbursement of paid amount for self-purchase of medicine/ investigation performedsubject to stipulated terms, exclusions and guidelines. However, it is advisable to prescribefrom the listed items to facilitate cashless services to IP.XXX.How to complete consultation & documentation?At the end of activity, the Health Passbook must be signed and stamped. Oncedocumentation in the health passbook is complete, IMP shall capture the image of theprescription page in the Mobile App and click complete to end the process. The check-innumber shall be generated at this stage in the App which needs to be recorded in the HealthPassbook. This number shall also get reflected for further action by the empanelledchemist/diagnostic centre.XXXI.How issuance of Medicine and provisioning of Diagnostic facility made?Once IMP has prescribed Medicines or Investigations, the IP/Beneficiary shall visit thedesignated eC / eDC respectively for availing services. Prescriptions from the specified listpublished by ESIC shall entitle the beneficiary to avail Cashless services from these centres.Similar to that of IMP Clinic, both eC and eDC will also have to download the ESIC“Dhanwantri“ Mobile app to log-in with the ESIC issued user credentials. At the time of visitby patient (ESI Beneficiary), Chemist will check ‘health passbook’ where the drugs areprescribed/written by hand and ascertain bonafide status of the ESIC beneficiary. He willlog-in to ESIC Dhanwantri App, feed in the beneficiary’s credential to view registered patientand the Check-in number issued by the IMP Clinic. Once the genuineness of ESIC Beneficiaryhas been ascertained, he shall dispense prescribed medicine and prepare bill through hisown system. Against a Check-in number of a patient in the Mobile App, the eC shall enterPage 7 of 18

the Bill date, bill No, Total amount of the dispensed medicine; upload image of bill showingreceipt signature of the patient, as well as the prescription page of the health passbook.Similarly, Diagnostic centre will perform the prescribed test from the provided list. He willprepare the bill for the investigations and enter Bill date, bill no, amount, upload image ofbill & prescription in the Dhanwantri App. He will also keep the photo/ scanned copy ofprescription booklet and Bill/test report and get it signed by patient/ attendant for futureclaim for reimbursement. The eC / eDC shall provide cashless services to the ESICBeneficiary when the Drugs issued / investigation carried out from the listed items by theIMP Clinic and raise bills to ESIC later every month for reimbursement.XXXII. What is to be done in case an unlisted drug/diagnostic test has been prescribed?If unlisted drugs or investigation is prescribed, the beneficiary shall have either of the twooptions: To pay from pocket and avail the drugs / services from the eC/eDC and later claimreimbursement from ESIC/DCBO producing copies of bills, proof of receipt and prescriptionon the Health Passbook, subject to exclusions; OR, avail these from DCBO, free of cost. Thisimplies that the eC/eDC shall charge the cost from the patient as per the agreed upon rate(Discount on MRP/CGHS Rate) upfront when purchased by the beneficiary and issue the Billand capture information in the relevant fields of Mobile App.XXXIII.How the settlement payments to Chemist & Diagnostic centre are made?The claim for the cashless services provided during previous month by the empaneledchemist/diagnostic centre will required to be submitted in Hard Copy (as per prescribedproforma) at DCBO/BO in the 1st week of every subsequent month. The claim should besupported with summary statement (may also be generated through mobile app),medicines/laboratory test bill(s) and the proof of receipt of medicines /laboratory testsreport by the patient on a monthly basis.
