SINCE 2006ANNUAL REPORT 2016KEYS TO SUCCESS: Collaboration and creative problem solvingThe year 2016 launched us into our second decadeserving the uninsured and underserved residentsof King, Kitsap and Snohomish counties. Whilethe national health care debate rages on, we arefocusing on simple, innovative solutions that removebarriers to care for our patients and deliver measurableresults for our health care partners. We are buildingon the vision of our founders and embracing newopportunities to collaborate and serve emerging needs.We continued to offer our core Care CoordinationProgram in three counties, helped low-incomeWashington residents to acquire private healthinsurance through our Premium Assistance Program,connected emergency room and hospital patients tohealth care homes through ourPrimary Link Program, andlaid the groundwork for ournew Health Home Program,which provides intensive,home-based, care coordinationservices for individualswith one or more chronicconditions. With the supportChrissy Yamada,of 1,640 volunteer providers,BOARD PRESIDENTwe served 6,304 patientsin 2016 alone, bringing ourtotal to almost 40,000!As this report goes to press, our organizationis undergoing a leadership transition, as foundingexecutive director Sallie Neillie retires and GaryRenville takes the helm. Producing measurable resultsStudy reveals powerful impact of simple patient interventionsThe encouraging results arein from our Primary Linkpilot project with five of theSwedish Emergency Departments.The project targeted Apple Health(Medicaid) and uninsured SwedishMedical Center emergencydepartment (ED) users who hadhealth care needs that could beappropriately addressed in primarycare. Primary Link staff connectedthese patients to a primary careprovider in one of the communityhealth centers and providedother resources depending onthe stated needs of the patients.The study assessed whetherthere were reductions in ED useresulting from the Primary Linkprogram. We used a comparisongroup of patients who met thecriteria for being contacted by theprogram, but were not contacteddue to program capacity.While there was little or nochange in ED visits in the PrimaryLink-contacted groups, therewere significant increases in thecomparison group. The PrimaryLink program appears to haveprevented an increase in EDvisits that would have occurredin the absence of the program.Translating the reduction in EDvisits into cost savings showed that 284,000* was saved in avoidedvisits in the six months followingour intervention, for MolinaHealthcare patients alone. Removing barriers to carePATIENT-CENTERED CARE: Molina Medical Group Clinic in South EverettFamily practice physicianMarcia M. Wharton, MDunderstands the manyobstacles her patients mustovercome to obtain health care.Many are low income or uninsured.Some struggle with addiction ormental illness. Almost all need helpnavigating the system. So whenshe got the chance to assist intransforming the way health carewas delivered in Molina MedicalGroup’s only outpatient clinic inthis state, she jumped right in.As associate medical directorfor Washington state outpatientclinics at Molina Medical Group,Dr. Wharton has been workingwith her staff and the MolinaHealthcare administration toredesign the care delivery modeland create a welcoming, patient-centered health care home, withcomprehensive care on site—fromsocial work and mental healthservices to primary care andchronic disease management.“We want our patients to feelthat this is their medical home,”explains Dr. Wharton. She hopes theclinic can eventually become a onestop shop for most patients, or atleast serve as a central touch pointfor patients who may need supportfrom other partner agencies.The clinic also will be usingpatient navigators to meet patientswhere they live and try to solvetheir most urgent needs—whichoften mean the basics of survival:food, clothing and shelter.“We will put the patient at thecenter of the care plan and start byidentifying each person’s primaryMarcia M. Wharton, MDneeds,” Dr. Wharton explains. “Theymay need to move to permanenthousing, or they may need foodstamps, addiction medicine servicesor mental health care. They may becovered by Medicaid but not reallyknow what that means,” she adds.“It’s completely different thanour traditional care model, whichwe know—for many patients—doesn’t work,” she says. “We’reexcited to see the results!”A POWERFUL PARTNER IN CARE: Molina Medical GroupMolina Medical Group is a critically important partner in our mission to open doors to quality health care for lowincome individuals — as it both receives referrals for primary care and sends referrals for specialty care.Through the Hospital Inpatient Discharge Program at Providence Regional Medical Center in Everett, ProjectAccess Northwest places many low-income uninsured and Medicaid patients who are soon to be released frominpatient care into follow-up primary care appointments within 12 days of their discharge. This program isdesigned to reduce the incidents of follow-up care taking place in the emergency department and to reduce thenumber of readmissions to the hospital for the same health issue.Project Access Northwest also partners with Molina Medical Group by helping its patients receive specialty careservices. Molina Clinic can refer patients to Project Access Northwest for care in more than 40 specialties.
