It is important that You readand understand this Certificate of Insurance, as Yourcoverage is subject to limitations and exclusions.All benefit amounts stated in this document are inCanadian currency.The enclosed Certificate of Insurance provides asummary description of the principal provisions of theinsurance coverages and assistance services availableto eligible Bank of Montreal Corporate AccountCardholders.This Certificate of Insurance contains a provisionremoving or restricting the right of the insured todesignate persons to whom or for whose benefitinsurance money is to be payable.PLEASE READ YOUR CERTIFICATE CAREFULLYBEFORE YOU TRAVEL.The Common Carrier Accidental Death and Dismembermentbenefits with 24 Hour Extension and Car Rental Accidental Deathand Dismemberment benefits described in this Certificate ofInsurance are underwritten by Allianz Global Risks US InsuranceCompany (“Allianz”) under Group Policy No. FC310000-C for aBMO Corporate Card account; or Group Policy No. FC310000-Dfor Diners Club Corporate Card account, Diners Club EventCard account and Diners Club Travel Account. Group Policy No.FC310000-C and FC310000-D are referred to herein as (the “Policy”)issued to the Bank of Montreal (the “Policyholder”, “BMO”). Allother benefits, such as Car Rental Collision Damage Waiver, CarRental Personal Effects, Lost or Stolen Baggage, Baggage Delay,Hotel/Motel Burglary, Unexpected Return Home and Flight Delay/Missed Connection insurance are offered by Allianz to You under anindividual policy. Your credit card number is Your policy numberwith respect to any such individual insurance.The insurance described in this Certificate of Insurance is for eligibleCardholders whose Accounts are in Good Standing and, wherespecified, their Spouses, Dependent Children and/or certain otherpersons (referred to herein as “You” or “Your”).Only BMO may determine who is a Cardholder, whether an Accountis in Good Standing and whether the insurance provided for hereinhas come into or is in force.No person is eligible for coverage under more than one policy orcertificate of insurance providing insurance coverage similar tothat provided hereunder. In the event that any person is recordedas an “Insured Person” under more than one such policy orcertificate of insurance, such person shall be deemed to be insuredonly under the policy or certificate of insurance which affords that

person the greatest amount of insurance coverage. The insurancecoverages made available to the Cardholder by BMO pursuantto this Certificate of Insurance supersede any other certificates ofinsurance previously issued to the Cardholder for the insurancecoverages described herein.Assistance Services are provided and administered by AllianzGlobal Assistance. EFFECTIVE AND TERMINATION DATEExcept as otherwise stated herein, the insurancecoverages shall come into effect on May 1, 2014 andshall terminate on the earliest of:1. The date of termination of the Corporate Card orCentral Account Program to which the Cardholderbelongs;2. The date the Cardholder is no longer eligible toparticipate;3. The date the Cardholder’s Account ceases to be inGood Standing;4. The date the Policy is terminated.ELIGIBILITYTo be eligible for this insurance You must be an InsuredPerson as defined in the definitions in each benefitsection.CAR RENTAL COLLISION DAMAGEWAIVER AND PERSONAL EFFECTSBENEFITSThe coverage outlined in this individual policy ofinsurance is provided to all eligible Cardholdersby Allianz Global Risks US Insurance Company(Canadian Branch). Your individual policy numberis the last four digits of your credit card number.SECTION 1 – DEFINITIONSIn this Certificate of Insurance, certain terms havedefined meanings. Defined terms are capitalizedthroughout this document.Accidental Bodily Injury means bodily injury causeddirectly and independently of all other causes byexternal violent and purely accidental means. Theaccident must occur during the Coverage Period andthe loss to which the insurance applies must resultwithin three hundred and sixty-five (365) days of thedate of the bodily injury and must not result from anyof the exclusions.Account means the Cardholder’s Corporate Card orCentral Account.Cardholder means the business owner or anyemployee ordinarily residing in Canada who has beenissued a Corporate Card by BMO; or the individualtraveling on a Central Account issued by BMO.Central Account means a Diners Club Event Card,Diners Club Corporate Travel Account, or BMO2

Corporate Travel Account/Department Card issuedby BMO, which is in Good Standing and for whichBMO includes this insurance as a feature of the cardor account.Corporate Card means a Diners Club Corporate Card,or a BMO Corporate Card issued by BMO with theCardholder’s name embossed on such card, and forwhom the Corporate Card account is established andin Good Standing and for which BMO includes thisinsurance as a feature of the card or account.Coverage Period means the time insurance is in effect,as indicated in this Certificate of Insurance.Dependent Child means a person who is: an unmarriednatural or adopted child or stepchild of a Cardholder;under twenty-one (21) years of age (under twentysix (26) years of age if a full-time student attending arecognized college or university); and is dependent onthe Cardholder for maintenance and support.Dollar or means Canadian dollars.Exotic Vehicle means automobiles manufacturedby Aston Martin, Bentley, Bricklin, Daimler, DeLorean, Auburn, Excalibur, Ferrari, Jensen, BMW,Lamborghini, Lotus, Jaguar, Maserati, Porsche, RollsRoyce or any similar automobile.Good Standing means being in full compliance withall of the provisions of the Corporate Card or CentralAccount account agreement, as amended from time totime.Insured Person means the Cardholder, the Cardholder’sSpouse, his/her Dependent Child or a person employedin Canada by the same employer as the Cardholder whois travelling with the Cardholder and who is otherwiseauthorized to operate the Rental Car.Mysterious Disappearance means when the articleof personal property in question cannot be located,and the circumstances of its disappearance cannot beexplained and do not lend themselves to a reasonableinference that a theft occurred.Operations Centre means the Operations Centremaintained by Allianz Global Assistance. From Canadaand the U.S. call 1-877-704-0341. From elsewhere callcollect 1-519-741-0782.Pick-up Truck means a four-wheel passenger vehicleequipped with a rear cargo area separate from thepassenger cab not exceeding a “3/4 ton” rating.Rental Car means a land motor vehicle with four (4) ormore wheels, that is designed for use mainly on publicroads and that You have rented from a commercialrental agency for Your business use for the period oftime shown on the Rental Car Agreement.3

Rental Car Agreement means the entire writtencontract that You receive when renting a Rental Carfrom a rental car agency that describes in full all ofthe terms and conditions of the rental, as well as theresponsibilities of all parties under the Rental CarAgreement.Spouse means the person who is legally marriedto the Cardholder; or if there is no such person, theperson who has been living with the Cardholderfor a continuous period of at least one (1) year in aconjugal relationship and who resides in the samehousehold as the Cardholder. For the purposes of thisinsurance, You may only have one (1) Spouse.Travel Advisory means a formal statement issued bythe Canadian government, advising Canadians not totravel to that country, region or city during the time ofYour insured trip.We, Our, Us means Allianz Global Risks US InsuranceCompany (Canadian Branch).You or Your means the Insured Person.SECTION 2 – BENEFITS – COVERAGEPERIOD AND DESCRIPTION OF COVERAGES2.1 – CAR RENTAL BENEFITSCoverage EligibilityThe Car Rental Benefits apply when the Cardholderenters into a non-renewable Rental Car Agreement,where the total rental period does not exceedforty-eight (48) days, subject to the exclusions andlimitations (as outlined in Section 3.1) and thefollowing requirements:1. The Rental Car must be rented by the Cardholder;and2. The Rental Car must be rented from a commercialcar rental agency; and3. The rental of the Rental Car must be for businesspurposes only; and4. The full cost or portion of the rental cost must becharged to: a Corporate Card, where the Cardholder can rentonly one Rental Car during a rental period; or a Central Account, where a Central Account canrent multiple Rental Cars during a rental period;and5. The Cardholder must decline the collision damagewaiver (CDW) benefits (or similar provisions, suchas “loss damage waiver”) offered by the rentalagency (when not prohibited by law). If such4

coverage is not available from the rental agency,then CDW benefits are not available under thisCertificate of Insurance; and6. The Rental Car must have been operated by theInsured Person under the Rental Car Agreementand in accordance with its conditions, when theloss occurs; and7. There must be a full calendar day waiting periodbetween rentals.Coverage PeriodInsurance coverage begins as soon as the Cardholder,or other person authorized to operate the Rental Carunder the Rental Car Agreement, takes control of theRental Car, and ends at the earliest of:1. The time when the rental agency assumes controlof the Rental Car, whether it be at its place ofbusiness or elsewhere;2. The end of the chosen rental period; or3. The date on which the Cardholder’s coverage isterminated in accordance with the “Effective andTermination Date” provision set out above.2.1.1 – COLLISION DAMAGE WAIVER(CDW) BENEFITSCoverage BenefitsSubject to the terms and conditions, You are coveredfor:1. Damage to the Rental Car; and2. Theft of the Rental Car or any of its parts oraccessories; and3. Rental agency charges for valid loss-of-use whilethe Rental Car is being repaired; and4. Reasonable and customary charges for towing theRental Car to the nearest available repair facility.Up to the full value of the rental vehicle.There is no coverage for additional rental fees chargedby the rental agency for a replacement vehicle ifrequired by You for the remainder of the originalrental period.This coverage does not provide any form of thirdparty automobile property damage or personalinjury liability insurance.It is the responsibility of the Cardholder to haveadequate third party insurance.The amount of the benefit payable will be equalto the cost of the repair (including loss-of-use) orreplacement cost of the Rental Car which has beendamaged or stolen, less any amount or portion of the5

loss assumed, waived or paid by the car rental agency,its insurer, or a third party insurer.In the event of a claim, the Insured Person mustcontact the Operations Centre as soon as possible orwithin forty-eight (48) hours.Please see Section 3 for applicable exclusions andlimitations.2.1.2 – CAR RENTAL PERSONAL EFFECTSBENEFITSCoverage BenefitsThis personal effects insurance covers loss, theft ordamage to personal effects belonging to an InsuredPerson while such personal effects are in transit orin any hotel or other building, en route during a tripwith the Rental Car for the duration of an eligiblerental period.Maximum coverage during such rental period is 1,000 for each Insured Person, per rental period.Total benefits during each rental period are limited to 2,000 per Account, per rental period.Please see Section 3 for applicable exclusions andlimitations.SECTION 3 – CAR RENTAL EXCLUSIONSAND LIMITATIONS3.1 – GENERAL EXCLUSIONS ANDLIMITATIONSThis insurance does not cover certain risks. We willnot pay any of the Car Rental Collision Damage Waiverand Personal Effects Benefits if a claim is directly orindirectly a result of one or more of the following:Damage – wear and tear, gradual deterioration,mechanical breakdown, insects or vermin, inherentflaw or damage;Violation of Rental Car Agreement – operation of theRental Car in violation of the terms of the Rental CarAgreement;Intentional Acts – damage due to intentional acts,while sane or insane;Off-road operation – damage caused to the Rental Carby use off of publicly maintained roads;Intoxication – any event which occurs while theconcentration of alcohol in the Insured Person’sor driver’s blood exceeds eighty (80) milligrams ofalcohol in one hundred (100) millilitres of blood; or6

while the Insured Person or driver is intoxicated dueto the voluntary taking of drugs;Drugs or Poison – any voluntary taking of poison,toxic substances or non-toxic substances or drugs,sedatives or narcotics, whether illicit or prescribed,in such quantity that they become toxic, or voluntaryinhalation of a gas;Disease – sickness, illness, bodily or mental infirmityor disease of any kind;Medical Complications – medical or surgicaltreatment or complications arising therefrom, exceptwhen required as a direct result of an AccidentalBodily Injury;Suicide – suicide, attempted suicide or self-inflictedinjury while sane or insane;Illegal Trade – transporting contraband or illegaltrade;Criminal Offence – committing or attempting tocommit a criminal offence, or committing or provokingan assault;War or Insurrection – declared or undeclared war,or any act of war, riot or insurrection; or service inthe armed forces of any country or internationalorganization;Confiscation – confiscation by order of any governmentor public authority;Seizure or destruction – seizure or destruction undera quarantine or customs regulation;Contamination – contamination or poisoning by nuclearand/or chemical and/or biological substances; orFinancial collapse or default of any transport, tour oraccommodation provider;Liability – other than for loss of, or damage to, theRental Car;Epidemic or pandemic damage caused by an epidemicor pandemic during the coverage period;Expenses – assumed, waived or paid by thecommercial car rental company or its insurers orpayable under any other insurance;Sanctions – any business or activity that would violateany applicable national economic or trade sanctionlaw or regulations;Trip Advisory – any expenses incurred, if You chooseto travel to a country, region or city, if before Youreffective date, a formal Travel Advisory was issued.3.2 – COLLISION DAMAGE WAIVEREXCLUSIONS AND LIMITATIONSIn addition to the General Exclusions and Limitations,these specific exclusions and limitations apply toCollision Damage Waiver benefits:7

1. This coverage does not apply to rental vehicleseffectively rented for a period exceeding fortyeight (48) days under a single contract or severalconsecutive contracts.2. This coverage does not apply to rental vehiclesused to transport property or passengers for hire orcompensation.3. This coverage will not pay for the cost of anyinsurance offered by or purchased through the carrental agency, even if such cost is mandatory orincluded in the price of the vehicle rental.4. Vehicles that belong to the following categories arenot covered: vans (except as defined below); campers or trailers; off-road vehicles (Sport Utility Vehicles arecovered, provided they are not used as off-roadvehicles and driven on maintained roads); motorcycles, mopeds or motorbikes; Exotic Vehicles; antique vehicles; recreational vehicles, or vehicles not licensedfor road use; and leased vehicles.Vans are not excluded provided that they:1. are for private passenger use with seating forno more than eight (8) occupants including thedriver; and2. do not exceed a “3/4 ton” rating; and3. are not designed for recreational use; and4. are not used for hire by others.An antique vehicle is one which is over twenty (20)years old or when its model has not been manufacturedfor ten (10) years or more.Limousines are not covered. However, standardproduction models of these vehicles that are not usedas limousines are not excluded.Trucks (other than Pick-up Trucks) are not covered.However, if the car rental agency provides You with atruck because the rental agency runs out of vehicleswhich are covered under this Certificate of Insuranceand which are not otherwise excluded from coverage,then that truck will be covered.3.3 – CAR RENTAL PERSONAL EFFECTSCOVERAGE EXCLUSIONS AND LIMITATIONSIn addition to the General Exclusions and Limitations,these specific exclusions and limitations apply to theCar Rental Personal Effects Benefits:8

1. Personal Effects do not include money (whetherpaper or coin), tickets, consumable or perishablegoods, bullion, banknotes, negotiable instrumentsor other numismatic property.2. Benefits are not paid if loss results from MysteriousDisappearance.3. Personal effects coverage is in excess of allother applicable valid insurance, indemnity orprotection available to the Insured Person inrespect of the item subject to the claim. We willbe liable only for the excess of the amount of theloss or damage over the amount covered undersuch other insurance, indemnity or protectionand for the amount of any applicable deductible,only if all other insurance has been exhaustedand subject to the exclusions and limitationsset out in this Policy of Insurance. This coveragewill not apply as contributing insurance and this“non-contribution” shall supersede despite any“non-contribution provision” in other insuranceindemnity or protection policies or contracts.4. Reasonable effort must have been made by theInsured Person to protect their Personal Effects(e.g. locking Your personal items in the trunk ofthe Rental Car instead of the front or back seat). Ifclaiming as a result of theft, evidence of forcibleentry into the vehicle while all the doors, windowsand other openings are closed and locked must besubmitted.SECTION 4 – CLAIM FILING PROCEDURESPlease contact Us at 1-877-704-0341 or 1-519-741-0782or visit www.allianzassistanceclaims.ca to obtain aclaim form. This insurance will not pay for any interest.As a condition to the payment of benefits under thisinsurance, We will need certain information from Youif You need to file a claim. This documentation willinclude, at a minimum and is not limited to, thefollowing:1. General Documentation Receipts and itemized bills for all expenses. Original of any refunds or expense allowancesreceived from Your tour operator, travel agency,common carrier or other entity.2. Collision Damage Waiver Benefits The fully completed claim form supplied to Youby the Operations Centre. The police report when the loss results indamage or theft over 1,000. Th
