MIHS reserves the right to award more than onecontract based upon the Proposal(s) most advantageous to the Maricopa Integrated Health System,price and other factors considered. The contract is scheduled for award for the initial term of three (3)years with the option to extend for additional periods up to a maximum contract term of five (5) years.3

REVENUE AND REIMBURSEMENT COLLECTION SERVICES 90-15-170-RFP1.0EXECUTIVE SUMMARY1.1 General MIHS InformationMaricopa Integrated Health System (MIHS), a special healthcare district and politicalsubdivision of the State of Arizona, includes Maricopa Medical Center (MMC), the Arizona BurnCenter, the Comprehensive Healthcare Center, the Arizona Children’s Center, the 7th AvenueWalk-In Clinic, 11 community-oriented family health centers, and an attendant care program.MIHS also is a premier training center for Arizona’s physicians. Our medicine, surgery,pediatrics, and OB/GYN programs, in particular, contribute to the body of knowledge of patientcare.MIHS is located in central Arizona, and serves as the health care safety net for citizens ofMaricopa County. The health system serves people of many races and nationalities who comefrom diverse cultures and speak several different languages. Many of the patients face majorchallenges, such as lack of health insurance, complex medical problems, and difficultsocioeconomic situations. Caring for these patients demands special knowledge and sensitivity.MIHS is committed to giving culturally appropriate, sensitive medical care and helping itspatients live healthier lives. Annually, MIHS has over 17,000 inpatient admissions and over450,000 outpatient and ambulatory visits.Maricopa Medical Center (MMC) is a 325-bed licensed, full-service hospital, which includes alevel one adult and pediatric trauma center. Over 67,000 adults and children are treatedannually in the Adult and Pediatric Emergency Departments. The Arizona Burn Center,Arizona's only regional burn center and the second largest in the nation, provides world-classcare for critically injured burn patients from across Arizona and the Southwest. MIHS’ ArizonaChildren’s Center features a 31-bed Neonatal Intensive Care Unit that provides critical inpatientservices for babies transported across the Southwest. Specialty care is offered at ourComprehensive Healthcare Center, a multi-specialty care clinic located at the MMC campus.MIHS has 11 Family Health Centers throughout the Valley. The professional medical staff at all11 centers are trained and certified in primary care and focus on the treatment of both adultsand children. Many of MIHS’ Family Health Centers provide all health care needs in onelocation, including primary care, dental care, and pharmacy services.Desert Vista Behavioral Health Center is a 122-bed licensed hospital that provides behavioralhealth care and psychiatric services. Additional behavioral health services are provided on theMMC campus in a 68 bed licensed facility.5

REVENUE AND REIMBURSEMENT COLLECTION SERVICES 90-15-170-RFP3.0WORK STATEMENT3.1 SERVICE GOALTo provide accounts receivable services for third party payers including Medicaid with theintent to provide the following major activities relative to the collection of outstandingbalances.Provide self pay collection services including bad debt.Contractor may bid on all or portions of AR collections services within this RFP.3.2 UNIT OF SERVICEOne unit of service equals the resolution of one account.3.3 MIHS AND CONTRACTOR REQUIREMENTS3.3.1MIHS requires a cap on the fee associated with any single account where the fee paidto the agency will be no greater than 25,000.3.3.2.MIHS will not pay commissions on accounts if payment is received within five workingdays of placement.3.3.3If additional insurance is identified the contractor may pursue follow up activities forthat insurance.3.3.4MIHS will provide suitable work space within the MIHS Business Office for agencypersonnel to access related documents such as payer remittance advice detail, akaEOBs, imaged medical records, payment batches and access to MIHS patientaccounting systems as needed.3.3.5.Remote access to MIHS patient accounting systems is available.3.3.6Vendor must provide collection notes activity, letters and call notes in the properformat (will be supplied by MIHS) to upload notes into the Epic Patient accountingsystem.3.3.7Listing files, return files and inventory files will be exchanged using established FTPprotocols as defined by MIHS.3.3.9Vendor will provide MIHS with inventory and payment files in a specified format forperformance measurement and payment reconciliation monthly.3.3.10Contractor shall provide status reports on a regular schedule (no less than monthly)reflecting the performance of all payments, compromises including active / current7

REVENUE AND REIMBURSEMENT COLLECTION SERVICES 90-15-170-RFPstatus, account touches and liquidation rate and defaults and complete inventory.3.3.113.3.123.3.13Vendor will place appropriate adjustments into the MIHS Patient accounting systemand pass through the appropriate signatory authority (as defined by MIHS).Vendor will provide in detail the overall collection techniques which shall include butnot be limited to the following: Approach, initial actions following placement,frequency of follow up efforts, supervisory review, and management of problemaccounts.Vendor must have prior experience with collection activities for clients using Epicclinical, patient accounting and electronic health records.3.3.14 Penalties.3.3.14.1 MIHS will assess a penalty for accounts with no follow up activity by theContractor for more than 30 days that due to the inactivity, causes the account tobe adjusted off due to timely filing and/or appeal deadlines. The penalty will beequal to the value of the Contractor’s fee for the said balance that must beadjusted. Vendor will touch no less than 90% of the accounts in 30 days. This willbe assessed quarterly. A reduction of the contingency rate of .25% will be appliedthe following quarter invoices.3.3.14.2 Vendor will supply MIHS with at least one performance incentive to be applied ona quarterly basis based on prior quarter performance review. Incentive willinclude performance measurement methodologies.3.3.14.2 Vendor will provide at least one Epic client reference and three total references.Pricing provided will be based on placement of Commercial, Managed Care, Medicaid, Medicare,Self Pay and Bad Debt accounts at 31, 61 and 90 days from initial claim date. Pricing willinclude rates for patient balances after insurance, Medicare or Medicaid payments. Self Payand Bad Debt may include a single blended rate or self pay early out, and/or bad debt rates.The District reserves the right to add or delete services associated with this solicitation, and thesubsequent contract(s), as needed by the District.Nothing prohibits the addition of supplemental services, not identified in this Contract and deemednecessary by MIHS and agreed to by the Contractor.8

REVENUE AND REIMBURSEMENT COLLECTION SERVICES 90-15-170-RFP4.0EVALUATION CRITERIA AND PROCESS4.1Proposal Review ProcessA committee comprised of various representatives from MIHS departments will evaluateresponsive and responsible proposals. The Committee may request clarifications and/oradditional information from any Proposer through written correspondence. At MIHS’option, Proposers may be shortlisted and invited to make presentations to the Committee.The Committee will prepare an objective ranking of the proposals. MIHS may, at its solediscretion, reject any or all proposals submitted in response to the Request for Proposal.4.2Proposal EvaluationMIHS’ Evaluation Committee will evaluate all proposals based on the criteria describedbelow: Firm’s Qualifications (Attachment B) (Maximum 20 Points)This will include a thorough and detailed review of the submitted OrganizationalInformation. Quality of Professional References (Attachment C) (Maximum 10 Points)This will include a thorough and detailed review of the submitted ProfessionalReferences. Pricing (Attachment E) (Maximum 20 Points)This will include a thorough and detailed review of the Proposer’s pricing. Althoughprice will be a factor in proposal evaluation, MIHS reserves the right to accept otherthan the lowest priced proposal. Response to RFP Requirements (Attachment F) (Maximum 40 Points)This will include a thorough and detailed review of the responses to the WorkStatement specifications, the quality, completeness, accuracy and level of detail of theProposal and that the Proposer demonstrates and understands the concepts andrequirements of the RFP. Clarity of expression, succinctness of response will also beevaluated. Compliance with RFP Requirements and Contract Provisions (Attachment G)(Maximum 10 points)This will be reviewed and scored based on the Proposer’s exceptions to the RFPRequirements and Contract Provisions. Proposals that accept the RFP Requirementsand Contract Provisions as is will receive the maximum points allotted for thiscriterion during proposal evaluation. Proposals that reject and state exceptions to theRFP Requirements and Contract Provisions will receive no points.9

Only these shortlisted Proposers will be invited for interviews/presentations.4.3Shortlisted Proposer Evaluation (If necessary)The Evaluation Committee will evaluate finalist Proposers through inviting the finalistProposers to attend team oral presentations/interviews and evaluating the finalistProposers based on the criteria described below. MIHS reserves the right to requestadditional information from Proposers prior to final selection, and to consider informationabout the Proposer other than that submitted in the proposal.4.4 Finalist Proposer Team Interview (Maximum 60 Points)MIHS may provide interview questions in advance to Proposers. MIHS’ EvaluationCommittee will evaluate interviews based on the team’s responses to questions,ability to effectively communicate, and the Committee’s assessment of the team’sability to work successfully with each other and MIHS staff. MIHS may also askProposers to submit written responses to some questions in advance of theinterviews. Strategic Fit (Maximum 40 Points)MIHS will evaluate proposed solutions based on overall best fit with MIHS businessgoals and objectives. The Committee will consider solution simplicity, overallalignment with the requirements set forth in the RFP, as well as compliance withcontract terms and conditions and any and all additional findings from MIHS’ duediligence process. MIHS’ due diligence may include client references, site visits, andindependent evaluations and rankings for the Proposer from industry referencesincluding, but not limited to Gartner Group, KLAS, and MD Buyline.Competitive NegotiationMIHS retains the right to negotiate the final contract terms and conditions, to bepresented to the Maricopa County Special Health Care District Board of Directors forapproval, with one or more of the apparent most responsive proposers as solelydetermined by MIHS.MIHS reserves the right to request clarification, to conduct discussions with proposers, torequest revisions of proposals, and to negotiate price changes or waive minorinformalities. During the discussion period, no information will be disclosed regardingeither the contents of proposals or discussions. When the Board of Directors makes anaward, the solicitation file and the proposals are a matter of public record.4.5Best and Final OfferMIHS may issue a written request for Best and Final Offers (BAFO). The request shall setforth the date, time and place for the submission of the BAFO. BAFOs shall be requested10

The request for aBAFO shall inform Proposers that if they do not submit a notice of withdrawal or a BAFO,their immediate previous offer will be construed as their Best and Final Offer.4.6Award of ContractSubject to the Board of Directors approval, award will be made to the proposer whoseproposal has been deemed most advantageous to MIHS in accordance with theevaluation criteria contained in this RFP.11

Proposers are encouraged to review the RFP packagein detail prior to commencing work.Any person, firm, corporation or association submitting a proposal shall be deemed tohave read and understood all the terms, conditions and requirements in thespecifications. Conditional proposals will not be considered. All proposals must be signedby an authorized signatory; unsigned proposals may be rejected.All responses and accompanying documentation will become the property of MIHS at thetime proposals are opened. Proposals deemed to be non-responsive will be returned tothe Proposer.5.2Required Response FormatTo assist in the evaluation process, all proposals must follow the same format. Proposalsin any other format may be considered informal and may be rejected.One (1) soft scanned copy (in a single linear file) of the completed and signed originalProposal in Adobe PDF format on CD or flash drive with one original and four (4) hardcopies of the Proposal must be submitted with the attachments in the following orderand labeled as follows.
