To obtain aCDL, the driver must pass knowledge tests, a skills test anda DOT physical examination. This manual is a studyguide for passing the knowledge tests and the CDLskills test. The booklet and a physical examinationform may be obtained at any Indiana Bureau of MotorVehicles (BMV) l icense branch or online at myBMV.com.1. A driver must have a valid Indiana operator’slicense.2. A driver must have a valid United States Social Securitycard. Copies of a Social Security card are not valid andwill not be accepted. Other acceptable documentsverifying a Social Security number are listed online atmyBMV.com or can be obtained by calling 888- 6926841.3. A driver must pass a DOT physical examination prior toapplying for a CDL. The driver must file a valid DOTphysical examination with the BMV to maintaincommercial driving privileges.4. A driver must obtain a CDL learner permit from the Bureauof Motor Vehicles. To obtain a CDL learner permit, adriver must pass one or more of the knowledge tests:A. The general knowledge test for all drivers.B. The passenger transport test by all busdrivers.C. The air brakes test by any driver of a vehiclewith air brakes.D. The combination vehicle test forcombination vehicles.E. The hazardous material test if a driver isrequired to haul hazardous waste or hazardousmaterials requiring vehicle placards.F.The tanker test for drivers required to haulliquids in bulk.G. The doubles/triples test for a driver requiredto pull double or triple trailers.H. The school bus endorsement test by all schoolbus drivers. (School Bus means a CommercialMotor Vehicle used to transport pre-primary,primary, or secondary school students fromhome to school, from school to home, or to andThe Federal Motor carrier Safety Regulations Part383.93 states: “Only drivers actually transportingpre-primary, primary, or secondary school studentsfrom home to school, from school to home, or toand from school sponsored events in a school busare required to have both the “P” (Passenger) and“S” (School Bus) endorsements. Only a “P”endorsement is required by drivers deliveringschool buses, and by drivers transporting studentsand/or adults to and from events that are notsponsored by the school.”5. After a driver has acquired a CDL learner permit, thedriver then must pass a skills test in a vehiclerepresentative of the class of license which thedriver will receive. The skills test must be taken atan approved state test site and will consist of threeparts:A. Pre-trip inspection test.B. Basic control skills test.C. Road trip test.If a driver fails the skills test, the driver may take thetest again the next day. A driver must only test once aday. A driver will be responsible for all costs each timethat driver has to take a skills test. After a driver haspassed the skills test, the examiner will validate acertification form which is to be taken to any BMV licensebranch.6. After a driver has passed all tests, the driver must takehis/her operator license and CDL learner permit toany BMV license branch.Note: If the Commercial Driver’s License has been expired,disqualified, canceled, revoked or invalidated for more than oneyear, the holder must pass all knowledge and skills exams priorto issuance of the CDL license. (140 IAC 7-3-17.5) CDL tests(knowledge and skills) are good for 180 days, and only three permitsare allowed in a two-year period (140 IAC 7-3-10).Page 3

Review of Steps to Obtain an IndianaCDL:The applicant must:1. Have a valid Indiana operator’s license.2. Pass a DOT physical examination. (See explanationbelow.)3. Pass the knowledge CDL test at any Indiana BMVlicense branch.4. Obtain a learner’s permit.5. Successfully complete a skills test at a CDL skills testsite.6. Submit the Skills Test Certificate within 30 days to anyIndiana BMV license branch, if applicable.3. All pages of the examination form and the medicalexaminer’s certificate with the medical examiner’ssignature, phone number and medical examiner’s statelicense number.4. The driver’s signature and the driver’s SocialSecurity number.5. Out-of-state physicals will be accepted if all otherrequirements are met.If a physical form is missing any of this information,a Commercial Driver’s License may not be issued.DOT Physical Examination forms, may be submitted:1.Providing Physical ExaminationTo protect the integrity of medical reporting and to monitorthe medical status of individuals who hold CDLs, theIndiana BMV and the Federal Highway Administrationhave established a medical review program. This programwill allow the BMV to track medical histories to ensurecompliance with federal and state medical requirements.The program requires a CDL applicant or a CDL holderto:1. Provide a copy of the DOT Physical Examination formwhen making application for a CDL permit and everytwo years thereafter or sooner if medically required.2. Sign an information release for the BMV and supply thephysician’s address, phone number and state licensenumber3. Click on All Regulations &Guidance, Part 383 and then 383.51.Exemption of Certain Physical DefectsA person who is not physically qualified to drive underFMCSR 391.41 (b)(1) or (2) and who is otherwisequalified to drive a commercial motor vehicle, may drivea commercial motor vehicle if the Federal Motor CarrierSafety Administration Regional Director of Motor CarrierSafety has granted an exemption to that person.For example, a person requiring a prosthetic or orthoticdevice is not automatically disqualified from operating acommercial motor vehicle. The State of Indiana, inconjunction with the Federal Highway Administration,will conduct a physical exemption evaluation, providedall exemption requirements are met.For specific information regarding the application, callthe Federal Highway Administration (Indiana office) at317-226-7474. Please contact the BMV for additionalinformation or assistance at 888-692-6841.Implied Consent to Alcohol TestingFederal Regulations Part 383.72 states “Any person whoholds a CLP or CDL is required to hold a CLP or CDL isconsidered to have consented to such testing as isrequired by any State or jurisdiction in the enforcementof 383.51 (b), Table 1, item (4) and 392.5 (a)(2) of thissubchapter. Consent is implied by driving a commercialmotor vehicle.”“Driving a commercial motor vehicle while under theinfluence of alcohol.” This includes:(A) Driving a commercial motor vehicle while theperson’s alcohol is 0.04 percent or more; or(B) Driving under the influence of alcohol, asprescribed by State law; or(C) Refusal to undergo such testing as is required by anyState or jurisdiction in the enforcement of 383.51 (b),or 392.5 (a)(2).Part 392.5 (a)(2) states “No driver shall: Use alcohol, beunder the influence of alcohol, or have any measuredalcohol concentration or detected presence of alcohol,while on duty, or operating, or in physical control of acommercial motor vehicle.”Source: Federal Motor Carrier Safety Regulations, Parts383, 390, 391, 392. A person required to have a CLP or CDL who isdisqualified must not drive a CMV.2. An employer must not knowingly allow, require, permit orauthorize a driver who is disqualified to drive a CMV.3. A holder of a CLP or CDL is subject to disqualificationsanctions designated in paragraphs (b) and (c) if thissection, if the holder drivers a CMV or non-CMV and isconvicted of the violations listed in those paragraphs.4. Determining first and subsequent violations. For purposesof determining first and subsequent violations of theoffenses specified in this subpart, each conviction for anyoffense listed in Tables 1 through 4 to this section resultingfrom a separate incident, whether committed in a CMV ornon-CMV, must be counted.5. The disqualification period must be in addition to any otherprevious periods of disqualification.6. Reinstatement after lifetime disqualification. A State mayreinstate any driver disqualified for life for offensesdescribed in paragraphs (b)(1) through (8) of this section(Table 1 to 383.51) after 10 years, if that person hasvoluntarily entered and successfully completed anappropriate rehabilitation program approved by the State.Any person who has been reinstated in accordance with thisprovision and who is subsequently convicted of adisqualifying offense described in paragraph (b)(1) through(8) of this section (Table 1 to 383.51) must not bereinstated.(b) Disqualification for major offenses. When it is safe to do so, thedriver may drive the vehicle across the tracks in a gearthat permits the vehicle to complete the crossing without achange of gears.”3. Look for a second train. After the last car of the trainpasses, check both directions to see if another train isapproaching.4. Never drive around crossing gates. If the gates aredown, do not cross the tracks.5. Never shift gears on the crossing.6. Watch for vehicles that must stop at a crossing.Be prepared to stop when following buses or truckswhich are required to stop at railroad crossings.7. Never race a train to the crossing.Safe driving practices on the road can help to avoidaccidents. Drivers on Indiana roads and highways, have aresponsibility to practice safe driving. Expect a train on any track at any time, day or night.2. Never get trapped on a grade crossing. When traffic isheavy, wait on the approach to a crossing until youare sure you can clear the crossing.Page 11
