2018 NORTH MEMORIAL HEALTHMESSAGE FROM OUR LEADERSJoseph Levine, MDOncology Medical Director, Program ChairAs the Medical Director of North Memorial Health CancerCenter and chair of the Cancer Committee, it is myprivilege to present the 2018 report of our program. Wehope you will use this an introduction to our commitmentto provide the best care for our customers and theirfamilies. The diagnosis of a cancer or a blood disorder is alife changing event.At North Memorial Health Cancer Center, we focus onempowering you to help make the most informed decisionsabout your care. Our physicians and nurse practitionersare skilled in the use of traditional chemotherapy,molecularly targeted therapy and the novel immunooncology therapies. We offer expert services in surgicaloncology, radiation oncology and palliative care, and wewill work alongside your primary care provider to makesure your needs are met. North Memorial Health CancerCenter is also proud to provide access to a multitude ofservices such as medication support, financial counseling,survivorship, and other social services and are fortunatebe a part of our customer’s care team.Kate Sullivan, RN, BSNDirector, Specialty ClinicsAs the Director of North Memorial Health Cancer Center and a nurse with over20 years of hematology/oncology experience, I am very proud of the care weare able to provide to those customers that have chosen North Memorial Health.North Memorial Health has been accredited by the American College ofSurgeons Commission on Cancer since 1987. This means that for over 30 years,our program has been taking a multidisciplinary approach to cancer care thatis driven by the highest standards and continuous clinical and programmaticimprovement. Our team has a long tenure in taking care of customers withcancer and other blood disorders and strives to provide the best experiencefor both customers and their families during what can be a very difficult time.2

2018 NORTH MEMORIAL HEALTH2018 YEAR IN REVIEWTumor Conference Presentations by Primary Site at North Memorial Health3 TUMORCONFERENCESPER WEEKOur cancer center continues to hostthree weekly multidisciplinary tumorboards that are highly attended.PROGRAMMATIC GOAL:CLINICAL GOAL:Working on EMR integrationwith North Radiation.Integrated Health Programat Cancer CenterAdded free yoga and exercise classesand massage services for our customers.PAY IT FORWARD:Supported the Pay It Forward fundraisingevent. Grants help customers with breastand other women’s cancers pay for basicliving expenses so they can focus on whatis really important – getting healthy.QUALITY:1. Studied the need for an improvedpsychosocial/legal distress screeningprocess and partnered with CancerLegal Line.2. Partnered with Care Access to improvecustomer access for scheduling and triage.PARTNERED WITHTRELLIS Rx:For increased access tooral medications for our customersat the Cancer Center Pharmacy.OPEN ARMS:3NEW PHYSICIAN:Rena Shah, MDContinue to partner with Open Arms ofMinnesota for increased access to food forour customers.

We ValueQualityAccredidations from: Det Norske Verita (DNV) American College of Surgeon’s Commission on CancerCertified Services and Programs in: Surgery Radiology Breast Oral and Maxillofacial Surgery Palliative CareMembers of: The Association of Community Cancer CentersStudies of Quality and ImprovementEach year, the Cancer Committee selects goals toimprove care delivery at North Memorial Health CancerCenter. 2018 goals included: Clinical goal to implement integrated health servicesat the Cancer Center for our customers. Free yogaand exercise classes, as well as massage services forcustomers were provided. Programmatic goal of integrating North RadiationEMR with North Memorial Health Epic wasinvestigated with discussions between both facilities. Cancer Legal Line study found that there is a needfor an improved psychosocial/legal distress screeningprocess, which we have implemented at the CancerCenter. Lynch Study for genetic referrals following colontumor resections. The study found that there isopportunity to improve the referral process followingpathology findings. Our pathology reports have beenupdated with appropriate referral prompts. Care Access addition to increase customer accessto scheduling and triage. Care Access took overmanaging incoming calls in July 2018, and wepartnered with them on the development and processimprovement efforts of triaging and schedulingcustomers calling in to the clinic.4

2018 NORTH MEMORIAL HEALTHPREVENTION & SCREENINGWanting to help ourcustomers achieve theirbest healthPrevention Head and neck cancer awarenessNorth Memorial Health co-sponsored with DeltaDental of MN a conference for medical and dentalprofessionals and customers. The conference wasan urgent call to collaborate due to the epidemic ofHPV-associated head and neck cancer. Family Night at North MarketNorth Memorial Health sponsored an evening at NorthMarket for a family-friendly night with kids activities,wellness and cancer education and fitness classes.— Breast health and cancer prevention education— Diabetes education— Nutrition information— Resources for mental health— Free blood pressure readingsScreening Breast cancer screeningNorth Memorial Health Clinic – Minnetonka held aSage screening mammography event in February. InOctober, a breast cancer awareness table was set upin the hospital atrium to provide breast health andbreast cancer prevention information to the public. Oral, head and neck cancerTeam members from OMFS (Oral and MaxillofacialSurgery) provided oral, head and neck cancerscreening at the conference in April. They alsoparticipated in the Mission of Mercy event inMinneapolis in September to provide cancerscreening and free dental care.5
