Dr. Joseph LevineOncology Medical Director, Program ChairDr. Joseph Levine holds board certifications in internalmedicine, hematology and medical oncology. He becamethe medical director of the cancer center and chair of thecancer committee in June of 2016. He is passionate about themultidisciplinary approach to cancer treatment and believesthat cancer care must be individualized to achieve optimalresults. Dr. Levine considers himself a general hematologist/oncologist but has specific interests in the treatment ofleukemia and lymphoma in addition to cancers of the uppergastrointestinal tract, colon and rectum, pancreas, andhepatobiliary system.Cancer Center HighlightsAs the Medical Director of the North Memorial Health Cancer Center and chair of the Cancer Committee,it is my privilege to present the 2016 report of our program.In spite of the many recent advances in understanding the biology of cancer and its treatment, thecancer diagnosis is still frightening and life altering. It is a diagnosis that requires a multidisciplinarytreatment approach to obtain successful outcomes, regardless of the specific goal of care. It is adiagnosis that requires a strong social support system to help people to stay steadfast during their timesof physical and emotional struggle. At the North Memorial Health Cancer Center, we are proud to offercancer treatment and support services that are on the cutting edge of modern medicine. Our medicaloncologists and nurse practitioners are skilled in the use of traditional chemotherapy, molecularlytargeted therapy, and the novel immuno-oncology therapies. We offer expert services in surgicaloncology, radiation oncology and palliative care. We are privileged to work with amazing primary careand subspecialty physicians across the North Memorial Health Care System who help us provide thecomprehensive medical care needed for successful treatment outcomes. The North Memorial HealthCancer Center is also proud to offer services in financial counseling, social work and psychosocialsupport, and cancer survivorship to help ease the emotional burden of the diagnosis for customers andtheir families.5 northmemorial.com

2016 Accomplishments: Commission on Cancer (CoC) awardedcertifications in the areas of Public Reportingof Outcomes, Pathology reporting and Datasubmissions. Celebration of Life Event. Speaker: RuthBachmann, author and cancer survivor. Preventive Care Team applied the “MemphisModel” and developed a partnership with acoalition of faith communities. The “CancerAwareness Sundays” event brought cancerprevention and screening awareness tounderserved neighborhoods in NorthMinneapolis. Our cancer center continues to host threeweekly multidisciplinary tumor boards thatare highly attended. Grand Rounds topics included “New Agentsand Clinical strategies in the systematictreatment of non-small cell lung cancer”presented byDr. Gregory Riely, M.D, Phd. in May 2016.And, Breast Cancer/Genetics presented byJoy Larsen Haidle, MS, CGC in November2016. The Cancer Center was awarded over 1,683,884 in grant money to help individualspay for their treatment. One hundred thirty people, includingcustomers and their families, received mealsthrough the Open Arms Meal Program. Our clinical trial enrollment reached 44customers. Over 31 customers received scrreningmammorams through the SageMammography screening program. The Cancer Center was awarded Sage ScopesColorectal Screening Grant. CoC Standard 4.1 Prevention/4.2 Screening.We continued our collaboration with thePrimary Care Preventive Care Team aimed atincreasing breast and colon cancer screeningrates within the North Memorial HealthSystem. We are committed to reaching 80%screening rate by 2018 and are currentlyalready at a rate of 79.1% breast cancerscreening. Our team created a survivorship care planand customized through the EPIC electronicmedical record so each patient’s cancer storyis more easily accessed by providers on thecare team. We created an electronic distress tooland implemented a policy to measure ourpatient’s psychosocial levels at every providervisit. This allows us to track the distresslevels in each of our customers and allowsour providers to easily place the necessaryreferrals for continued care. Our customers were awarded 89,000 incommunity grants for basic living expensessuch as rent, food, gas, utililities, child careand car repairs.6 northmemorial.com

Dr. Deepak Kademani, Oral SurgeonMulti-disciplinary cancer programDr. Deepak Kademani is board certified oral &maxillofacial surgeon whose clinical special interestsinclude: head and neck cancer, oral cancer, salivarygland pathology; facial trauma, reconstructive surgery,bone grafts, jaw tumors, TMJ disorders, obstructivesleep apnea, dental implants and dentoalveolar surgery.Oral and Maxillofacial Surgery program overviewThe Oral and Maxillofacial Surgery (OMFS) program was established at North Memorial Health in April2014, as an integral part of the hospital level I trauma services. Since that time, a full scope hospital-basedsurgical service has been designed to cover the specialized needs of trauma, oncologic, reconstructiveand elective head and neck surgical customers. Under the leadership of Dr. Deepak Kademani, the OMFSprogram has developed a comprehensive practice excelling in the management of complex head and necksurgery customers through all phases of their care. Their team remains driven to provide remarkable carewith advancements in clinical and surgical practice, education and research. They perform a full spectrumof specialized services including head and neck oncologic surgery which includes oral cavity, laryngeal,tracheal, thyroid and parotid tumors, microvascular reconstruction, orthognathic, TMJ surgery, dentoalveolarprocedures, implants and craniomaxillofacial trauma.Annually, the OMS service performs approximately 500 major patient procedures per year. This yearfrom January to July 2016, nearly 300 surgeries have been performed including 16 microvascular free flapcases. The clinical practice has maintained a 96% or higher level of willingness to recommend. The OMFSservice works closely with the North Memorial Health Cancer Center and has established a multidisciplinaryrelationship creating a seamless transition and integration of oncologic care for customers with head andneck tumors. It is because of this commitment to its patient population and oncologic services, NorthMemorial’s OMFS service and the North Memorial Health Cancer Center has been recognized as the thirdlargest head and neck practice in the State of Minnesota.In July 2015, North Memorial Health was proud to establish a CODA accredited Head and Neck Oncologicfellowship for the education and development of future Oral and Maxillofacial Surgeons to pursue postgraduate training in Head and Neck Oncologic and Reconstructive Surgery. The fellowship provides bothclinical teaching with didactic education and opportunities for research. Their training includes rotations withmedical and radiation oncology, pathology, and a variety of services to ensure a comprehensive educationalexperience. Upon completion of the fellowship, they are competent in performing a variety of head andneck oncologic procedures from resection of benign and malignant cancer lesions of the head and neck tomicrovascular reconstruction of head and neck defects. In addition, fellows also participate in managementof the craniomaxillofacial trauma customers from our Level I trauma center at North Memorial.7 northmemorial.com

Historically, oral cancer has been associated withmales older than the sixth decade who regularlyconsume tobacco and alcohol products. Statisticsnow show that in certain populations the incidenceof oral cancer is increasing especially in youngerages, white males, and oral cancer associated withthe human papilloma virus (HPV). There has beenan alarming and steady increase in the incidence oforal cancers occurring in customers below the ageof 40, and in particular females, without identifiablerisk factors. The role of viruses has become a focusof research in the development of oral cancer.Specifically, HPV (subtypes 16, 18, and 31), is nowconsidered an independent risk factor for thedevelopment of oral cancer. Ultimately, tumors ofthe head and neck are among the top ten mostcommonly occurring malignancies for both menand women in the US. For 2016, the AmericanCancer Society estimates that about 48,330 peoplewill develop oral cavity or oropharyngeal cancer,this has increased from an estimated 39,500 peoplein 2015. The SEER data currently reports there areapproximately 500 new head and neck cancersdiagnosed in Minnesota on an annual basis.Early stage cancer is typically managed witheither surgery or radiation therapy alone. Althoughthey have almost equal success in the control ofrecurrence, the side effects of radiation often makethis a less appealing option, particularly for oralcavity tumors. The goal of surgical treatment is toremove the primary site of the cancer includinga three-dimensional excision of uninvolved tissuesurrounding the cancer. Similar to other cancers,oral cancers tends to spread through the lymphaticsystem. Therefore, it may be necessary to removethe regional lymph nodes in the neck to preventspread of malignant cells. Finally, reconstructivesurgery after removal of an oral cancer lesion isoften desirable to minimize the functional andcosmetic changes. The goal of reconstruction isto reestablish normal function such as speaking,eating and appearance as quickly as possible. Useof additional modes of therapy is dependent onmultiple factors including the primary location ofthe cancer, the size of the tumor, and the possibilitythat the cancer may have spread into the lymphaticsystem. Advanced staged tumors may requireadditional modes of treatment, often combiningsurgery, radiation and chemotherapy. Each patientwho presents with head and neck cancer requiresan individualized and personalized approach withmulti-disciplinary collaboration to determine thebest and most appropriate treatment options.The primary mode for diagnosis of oral cancer isactually physical examination. Oral cancer is oftenpreceded by a precancerous lesion. These lesionsmay be asymptomatic and only identified on dentalexams or oral screening. Symptoms that shouldalert a patient or health care provider to a possibleoral cancer include jaw or dental pain, changingor growing lesion, loose teeth, bleeding, dysarthria(difficulty in speech articulation), dysphagia(difficulty swallowing), odynophagia (pain onswallowing), otalgia (ear pain), sensory and motornerve compromise, mass lesions at the primarysite or cervical lymphadenopathy. It is importantthat any lesion or sore in the mouth that has nothealed within two weeks be evaluated. Survivorship CareWe at North Memorial Health Cancer Center believe the survivorship visit is an essential element ofour customer’s care. Survivorship visits provide customers with a document which includes a concisesummary of their cancer diagnosis and treatments received. Education on prevention and follow-up, aswell as, getting connected to additional resources will all keep the customer understand their new normal.We have taken Survivorship to a whole new level! We have worked with the North Memorial Health ITdepartment to create a custom survivorship template built into the patient’s individual electronic healthrecord. Recognized on a national level, our survivorship program puts a patient’s cancer treatmentsummary only a click away from a patient’s primary care physician. This innovative approach tosurvivorship assures excellent communication to those physicians involved in the customers’ care.Survivorship visits are scheduled with our Clinical Nurse Specialists, NursePractitioners and Nurse Navigators. Psychosocial focusin cancer treatmentEveryone has a cancer story, or someone they know and love, or perhaps even their own life has beenimpacted by cancer. It is a feared diagnosis that affects those experiencing cancer and their familiesemotionally, spiritually, financially and physically. Customers and families, impacted by the weight of thediagnosis, often feel unprepared for the emotional rollercoaster, economic burden and life altering journeythat cancer brings to their lives.At North Memorial Health Cancer Center, we know it is not enough to just medically treat your cancer. Weunderstand the importance of addressing the practical, family, emotional, and spiritual aspects of cancer.Easing the burdens help the healing. Knowing this, we begin the conversation about how you are doing atevery physician visit.Addressing psychosocial aspect of care is the sixth vital sign, equally important as lab values and bloodpressures. We utilize an innovative tool to define your individual needs and address your psychosocialconcerns at every physician visit. Celebration of Life!North Memorial Health Cancer Centerannual Celebration of Life a huge success!On Wednesday, April 6, 2016, more than 165customers and their families attended the 2016Celebration of Life event for Cancer Survivors.The corridors were brimming with organizationsoffering products, services and informationaimed at supporting cancer customers. Thefood was delicious and the featured speaker,author and cancer survivor, Ruth Bachman, wasentertaining and inspirational! The guests ofhonor, however, were our customers and they arefabulous!!11

Palliative Care TeamPalliative care services are an essential component of cancer care. Palliative Care includescomprehensive care and management of the physical, psychological, emotional and spiritual needs of allcustomers with serious illness including those with cancer. The goal of our Palliative Care program, here atNorth Memorial Health, is to improve quality of life for both the patient and the family.Customers identified with a need for Palliative care services will be referred to the Palliative Care team.This team will comprise a strategy to meet the needs of the patient and family as they work to live wellwith a diagnosis of cancer or other serious illness. North Memorial Health Breast CenterNorth Memorial Health Breast Center may seem like a spa with its warm décor, soft terry robes, relaxingmusic, aromatherapy, and soft spoken, caring team. While it is all of those things, it is also a state of the artbreast care center. North Memorial Health Breast Center has the digital equipment to aid breast cancerscreening and diagnostic and surgical follow up.Because we understand the busy lives of our customers, in 2016, North Memorial Health Breast Center andaffiliated North Memorial Health Mammography locations, implemented the same day scheduling optionfor those wishing to drop by for a quick screening mammogram. This customer-centered change has beenhugely popular by increasing convenience to breast cancer screening.Knowing there are many obstacles which prevent our customers from getting an annual breast cancerscreening mammogram done, North Memorial Health Breast Center and its affiliated North Memorial HealthMammography locations has partnered with Minnesota Department of Health’s Sage Screening program.On June 23, 2016 a Sage Breast Screening Event at North Memorial Health Breast Center was held to bringin low income women from our service area. Eight customers either uninsured or under insured were able toreceive complimentary cervical cancer screening exam, a mammogram and gain additional colon and uterinecancer prevention information. Additional Sage Breast Screening Events have been held at North Memorial’sBlaine and Downtown Clinics, serving an additional 23 customers, collectively. Interpreters were on site andtransportation was provided to customers.Leaders from the North Memorial Health Breast Center collaborate with the North MemorialHealth Preventive Care Team. This partnership has committed to reach 80% Breast CancerScreening rate for North Memorial Health customers by 2018. This initiative will make adifference in catching cancers in early stages when it is more curable.13 northmemorial.com

PharmacyNorth Memorial Health Cancer Center pharmacy is located in the heart of the infusion center. The locationwas intentionally placed in effort to be a central location for all cancer patient needs. Our specially trainedOncologic Pharmacists are only a few steps away from customers receiving chemotherapy and always readyto answer questions and offer encouragement. In addition to providing on site prescription counseling andeducation, North Memorial Health Cancer Center Pharmacy offers retail medications eliminating the need forcustomers to stop at another location for over the counter medications to aid symptoms. In 2016 we haveupdated our electronic pharmacy system in effort to improve refill efficiency and continuity of care.14 northmemorial.com14

Radiation Therapy and ImagingNorth Memorial Health Breast Center/North Memorial Health Cancer Center customers needing radiationtherapy or follow up imaging exams enjoy the convenience of these services in the beautiful, sun soakedNorth Memorial Health Outpatient Center. Located a few steps down the hall from North Memorial HealthCancer Center, customers often schedule their appointments in the same day and same location. Thisconvenience allows for efficient scheduling and often results in more restful days at home to recover.15 northmemorial.com

Physical and Occupational TherapyProgramRehabilitation is a growing area in cancer care due to the increase in cancer survivorship — customerstend to do better when they are stronger. Because of the advancement in medical technology, treatmentand benefits of early detection, more and more customers are utilizing services such as physical andoccupational health as part of their cancer recovery plan.North Memorial Health has programs in place to meet cancer recovery needs. Whether it is to restoremobility after surgery, improve cancer-related Fatigue (CRF) or reduce symptoms of lymphedema, carenavigators will help facilitate a recovery schedule that works for you.Most people don’t think speech therapy is part of cancer treatment but, at times, there is a need for thisservice. North Memorial Health has a speech pathology department to evaluate and treat customers whomay have conditions that cause difficulty with communication, thinking and swallowing.Additionally, our cancer program offers weekly fatigue prevention exercise classes in the beautiful atriumin the North Memorial Health Outpatient Center. These very popular complimentary classes are opento cancer customers as a means of proactively combating fatigue that sometimes accompanies cancertreatment. Taught by specialized trainers and modified to appeal to all levels of mobility. A variety of agegroups attend, even one patient who is 101 years young! She attends every week! Preventive Care TeamNorth Memorial Health Preventive Care team, a multi-disciplinary team, is responsible for developingstandard processes and initiatives to increase breast and colon cancer screening rates for customers acrossthe North Memorial Health system.North Memorial Health has experienced a steady increase in both breast cancer and colon cancer screeningrates since the team was assembled in late 2014.A positive outcome of our increased screening rates has led to an increase in the diagnosis of stage ! andstage II cancers. Barriers such as financial, transportation, language, orcultural concerns may delay customers obtaining these important preventative care procedures.In light of these issues, the team strengthened our partnership with the Minnesota Department of HealthSage program to offer no cost breast and cervical cancer screening services. North Memorial Health is nowable to serve Sage mammogram participants at 14 North Memorial Health clinic locations. During 2016, wereceived approval to also begin to offer the Sage Scopes program, which provides colonoscopies at no costto customers who are financially eligible.Additionally, the Preventive Care team applied the “Memphis Model” and developed a partnership withthe coalition of faith communities. They committed to present information on Cancer Awareness Sundaysregarding the importance of early detection and cancer screenings before and after church services inunderserved areas of Minneapolis. During 2016, members of the Preventive Care team volunteered their timeto participate at 8 church Cancer Awareness Sundays. Also two North Memorial Health physicians presentedat 4 faith communities on the topic of breast and colon cancer screening and fielded questions from thecongregations. We estimate that we delivered the message about the importance of preventative care andearly detection to over 1000 members of these faith communities.In recognition of our 2015-2016 efforts to increase the overall colon cancerscreening rates and to reduce cancer screening disparities in underservedcommunities in the service area, North Memorial Health received two awardsfrom the American Cancer Society in 2016.17 northmemorial.com

Additional 2015-2016 initiativesled by the Preventive Care team: Implemented a standardized process to offerless invasive colon cancer screening options,when a patient does not wish to schedule the“gold standard” of colonoscopy. We developeda FIT kit/Follow Up process. We offer patientinstructions in multiple languages, eliminate thebarrier of transportation by offering a postagepaid mailer to return the sample for testing. Weuse a tracking tool to monitor when we providea stool sample collection kit to customers andmonitor for sample return for testing. Weroutinely follow-up with customers when theydo not send in a sample, in order to respond toquestions or offer additional supplies. During2016, we increased our average sample returnrate from 10% to 70%. Created an awareness campaign during thenational breast cancer awareness month whichincludes multiple methods of communicationto customers regarding the importance of earlydetection and mammograms. Implemented Fast Track Mammogramscheduled for North Memorial Healthemployees. Over 20 women scheduled theirmammograms via the quick and confidentialmethod to choose their appointment times. Partnered with a health plan to createawareness and leverage the gift card incentivesthat the health plan offers to members whocomplete a breast or colon cancer screeningtest. This partnership incentivized 69 women toget mammograms and 12 others to complete acolon cancer screening test. Cancer Center Highlights As the Medical Director of the North Memorial Health Cancer Center and chair of the Cancer Committee, it is my privilege to present the 2016 report of our program. In spite of the many recent advances in understanding the biology of cancer and its treatment, the cancer diagnosis is still frightening and life altering.
