The tests will be performed at Munson Medical Center and can be used toidentify potential convalescent plasma donors.Important recommendations when considering this test:While this test is available, we still question the clinical utility of the antibody testing result outside ofconvalescent plasma donor identification for these reasons.The understanding of the immune response to SARS CoV-2 and its implications for diseasemanifestation and for immunity to reinfection is in its infancy. Given the uncertainty surroundingthese antibody tests, to date they have no clinical utility outside of approved uses, such asidentification of potential convalescent plasma donors.A positive does not allow one to conclude immunity and a negative does not rule out active norrecent infection. For use in a specific patient outside the context of a study or donor identification,the test has a real capacity to cause harm in that if positive, there is the potential for a false sense ofsecurity of immunity. No prudent advice about patient behavior or risk can be given based on thesetests. The tests are not far enough along in development to have any clinical utility in that soundinterpretation is impossible and therefore the capacity for patient harm is real.In addition, insurances may not cover the cost of these studies, causing additional financial impact tothe patient and the healthcare system when ordered.It is Munson Healthcare’s recommendation that SARS CoV-2 antibody tests not be ordered outsideof identification of potential convalescent plasma donors.Additional Test Details:Test Name:SARS CoV2 IgG ANTIBODYSample Type:Serum. You should expect a result by the next day in mostinstances.Test results will be reported as below: Negative. No IgG antibodies to SARS CoV2 detected, Negative results may occur in serumcollected too soon following infection, in immunosuppressed patients, or in some individualswith prior mild illness. Follow up testing with a molecular test is recommended in

symptomatic patients. This test should not be used to exclude active or recent COVID 19disease. Testing approved under the FDA emergency use authorization. Equivocal. Please retest patient in 10 to 14 days. Positive. SARS CoV2 IgG antibodies detected. Results suggest recent or prior infection withSARS CoV2. Correlation with epidemiologic risk factors and other clinical and laboratoryfindings is recommended. Serologic results should not be used to diagnose recent SAR CoV2infection. Protective immunity cannot be inferred based on these results alone. Infrequently,false positive results may be due to prior infection with other human coronaviruses. **As of 9/2/20Protecting Access to Medicare Act (PAMA) Education and Testing PeriodExtended to 1.1.22On January 1, 2020, CMS implemented a new mandate – section 218 of Protecting Access to MedicareAct (PAMA) – that changes the prior authorization process for ordering certain advanced diagnosticimaging studies. The education and testing period for PAMA has now been extended until January 1,2022.When ordering an advanced diagnostic imaging service for a Medicare beneficiary, a provider is nowrequired to consult a qualified Clinical Decision Support Mechanism (CDSM). CDSMs are electronicportals through which appropriate use criteria (AUC) is accessed. The CDSM provides a clinicalDecision Support Number (DSN) denoting if the order adheres to the AUC. The CDSM replaces theprior authorization process for Medicare patients, is faster, and provides a more specific diagnosis aswell as more information on the order.EMRs should have prepared to meet this mandate by embedding a qualified decision supportmechanism within their software. Please check with your EMR to see how they will be supportingyour office to meet this mandate.
