Mentis Assist assists people to recognise theirvalues, strengths and goals, with an emphasis on improving health and wellbeing, independence, socialparticipation and enhancing meaningful relationships with significant others.Mentis Assist was established in 1987 and has evolved significantly to become a leader in the areas of deliveringflexible support packages tailored to the individual using strengths-based, recovery-orientated practice, quality andevidenced based service provision.Mentis Assist strives to provide opportunity for people living with mental illness to enjoy a meaningful life bystrengthening self-identity, personal responsibility and hope. Qualifications and Requirements Qualification in mental health, aged and disability care or related discipline – minimum Cert. The HACC Practitioner will use their skills, knowledge, case work and group work strategiesto support their clients and their family to develop the Individual Support Plan (ISPs), Wellness and Discharge(where appropriate) Plans that achieves the identified goal within set timeframes.Under general direction of the HACC Coordinator, the HACC Practitioner ensures a high standard of service deliverythat is consistent with the organisation’s values and service models. The HACC Practitioner is charged withproviding opportunities that assist with maintaining physical and daily living skills and personal development,community and family education, mental health and wellbeing coping strategies and community engagement inline with the client’s Individual Support Plan.HACC service delivery is complimented by drawing on external (informal and formal) support networks (SupportTeam) working collaboratively to ensure that when the client is ready for discharged, they are able to self-managewith these supports in place. Individual Support Plans and case/group work is regularly reviewed with the supportof the HACC Coordinator.The objectives of HACC service provision is to holistically assist clients to: Develop the knowledge, skills and confidence clients need to make decisions and choices about theirsupport needsLearn or relearn skills and develop confidence required for activities of daily living (e.g. budgeting, buyingand cooking food, healthy living skills, personal care, care for one’s home environment)Improve the individual’s social relationship skills and confidence (e.g. making and keeping social andrecreational contacts, and relationships with friends, family, neighbours and significant others)Access and engage with (public or private) mental health treatment services, including general practitionersas required to manage their mental illnessContribute to achieving client directed outcomes in regard to physical health, community connections andother relevant needs.HACC Practitioner13/07/2016Page 2 of 5

Finance and Performance Ensure all service expenditure has approval in line with Mentis Assist’ Delegation Schedule policy andguidelinesEnsure all accounting and documentation processes are accurate and within budgetWorking closely with the HACC Coordinator, reflectively review HACC practices in line with service model,performance targets and related policies, and practice guidelinesSupport the HACC Coordinator by ensuring Minimum Set Data (MDS) targets are metSupport the Service Manager by preparing draft ASM, Diversity Plan as directed by the HACC CoordinatorSupport the Service Manager by assisting with funding and reporting requirements and complianceobligation, as directed by the HACC Coordinator.Practice Knowledge General understanding of mental health and behavioural diagnoses and symptomsGood understanding of the causes of stigma and barriers faced by those with disability and mental ill healthSound understanding of age and disability servicesExperience in providing; strengths-based, person-centred group work practices.Professional Development Participate in fortnightly supervision with the HACC CoordinatorUndertake annual performance appraisals with the HACC Coordinator and the Service Manager anddevelop and implement own professional development plansActively take part in training and skill building opportunities.Administration Ensure the completion of Minimum Data Set (MDS) and other reporting requirements within settimeframes in readiness for reporting by the Service Manager, and completed to the highest standardRecord case notes as required in line with policies and guidelinesEnsure HR policies and procedures are adhered toEnsure adherence to Critical Incident ReportingEnsure HR responsibilities are met within set timeframes i.e. timesheets, personal and annual leave forms,50/52 requestsPossess good written and verbal communication and IT skills.Promotion Actively promote the HACC program within the community and with local service providersEnsure consistent number of clients are actively engage to meet funding targetsEnact strategies as directed, to ramp up client numbers as directed.Other Duties Conduct other duties as directed.HACC Practitioner13/07/2016Page 4 of 5

Key Selection Criteria1.Demonstrated practice knowledge of working with HACC eligible, older people with mental ill health and/orthose who present with behaviours that makes access to general services and community programs difficult.2.Demonstrated skills and knowledge in the area of planning, implementing, reviewing and evaluating effectiverecovery-orientated case work and group work.3.Proven ability to consider and develop and deliver effective annual program plans in-line with fundingdirection i.e. HACC Active Service Model, Diversity and Diversity Plan.4.Demonstrated skills in promoting programs and achieving referrals from clinical and general service providersand general and cultural aged community groups.5.Good understanding of the HACC Active Service Model and Diversity planning.6.Proven ability to have ‘courageous’ conversations with others including problem solving and conflictresolution.7.Demonstrated ability to work as an effective and collaborative team member, including demonstration of ahigh level of professional and ethical behaviour within an organisational setting.
