Total of twenty-one switches, one each intwenty-one heating coils (thirteen - notsafety-related; eight - safety-related, only four ofwhich have been determined reportable; designated by*).See attached list.SUPPLIEDBrasch Manufacturing Company, Inc., Maryland Heights,BY:Missouri. Air Flow Switches manufactured by DwyerInstruments, Inc., Michigan City, Indiana, assubvendor to Brasch Manufacturing.NATURE OFIn October, 1984, Brasch Manufacturing advised CP&LDEFICIENCY:that previously shipped electric heating coilscontained air flow switches with acrylic scaleenclosures which limits the airflow switch maximumtemperature for reliable operation to 130*F whereasthe same switch with glass scale enclosure has a180*F rating. The switch was specified by Brasch tohave glass enclosures, however, they wereinadvertently ordered and shipped with acrylicenclosures. Acrylic scale enclosures, when exposedto excessive temperatures as may be experienced in apost accident environment or during testing, couldwarp resulting in a leak path. This would cause theairflow switch to sense ambient pressure on bothsides of the diaphragm when installed in negativepressure portions of systems, which in turn woulderroneously indicate loss of air flow through theelectric heating coil / air cleaning unit. Sucherroneous indication would de-energite the electricheating coil.DATE PROBLEMCONFIRMED TOEXISTBased on a letter from the vendor, datedOctober 22, 1984, NCR-84-2332 was issuedDecember 5, 1984. -.,--- - .,.,.DATE PROBLEMREPORTED:,On December 20, 1984, Mr. F. E. Strehle notified theNRC (Mr. A. Hardin) that the subject item was 'potentially reportable. On January 17, 1985 Mr. N.J. Chiangi notified the NRC (Mr. A. Hardin) that theDecember 20, 1984 potentially reportable item hadbeen evaluated and found to be reportable per thepnovisions of 10CFR50.55(e) and 10CFR Part 21.SCOPE OFPROBLEM:The deficiency involves the twenty-one identifiedheating coils for Unit I shipped on Furchase OrderNY-435152. These heating coils have the subjectairflow switches installed.(See attached list.)SAFETYIMPLICATION:.The four electric heating coils denoted by * areprovided to limit effluent humidity through the RABand FilB emergency exhaust systems to * ess than 70%RH.Upon failure of the subject heating coils, theeffluent could reach 100% RH thereby saturating thecharcoal adsorbers. Excessive relative humidityseverly reduces the charcoal adsorber efficiency.During applicable post. accident scenarios, the aircleaning units associated with the four subject coilswould discharge to the plant vent stack and couldrelease uncontrolled effluents.qREASONDEFICIENCYIS REPORTABLE: If left uncorrected,' failure of the air flow switchscale enclosure conld result in off-site exposures ofradioactive effluent in excess of 10CFR100 limits.CORRECTIVEACTION:.Brasch Manufacturing indicated in their letter datedOctober 22, 1984 that they would provide and installreplacement glass scale enslosures as originallyspecified for SHNPP Unit One affected components. OnFebruary 21 6 22, 1985, scale enclosures per attachedlist were changed from Acrylic to glass by BraschManfacturing in accordance with their proposedresolution and therefore no further action isrequired.L!rWKJD04 1!. . . . . . .
