About Us For over 30 years, Maxim Healthcare Services has been providing services forthe developmentally disabled population. Currently have 4 local offices within the Inland Regional Center catchment Specialize in offering non-medical care in the Inland Empire

What is Personal Assistance? The purpose of Personal Assistance is to promote independence Personal Assistants provide one-on-one care, assisting andplanning in day to day activities that promote independence athome and in the community. Personal assistance goal plans are individualized to fit the needsof the consumerPlease Note: No overlapping of services with another program

What are the differences between PersonalAssistance and Respite?

College Personal Assistance Scheduling DSPS meetings and class registration Assist with shopping for school supplies Assistance with navigating campus Note taking and page turning Encouragement and positive reinforcement Time management and organization Encourage socialization Transportation assistance or help accessing public transportation Mobility and transfer assistance Restroom assistance Assistance with meals and feeding Assist with finding tutoring

Daily Goals and referral Process Consumers receiving personal assistance must have an appropriate goal planfor services Maxim staff works with the consumer and or guardian in order to complete agoal plan and recommends hours based off consumer assessment. Goal plan and recommended hours are forwarded over to CSC and PM forrevision and approval Once approved the caregiver matching process begins Once we receive areferral weschedules anassessment.ReferralAssessment During theassessment apersonalized goalplan is created. Hours aredetermined basedoff of theassessment (goalplan).Request is sentApproved Once the request isapproved our staffcan work on findingan assistant for theconsumer.

Examples of Personal Assistance Goalsand recommended hoursPA Goal Plan:1.Assistance with personal care while out in the community2.Physical assistance and safety supervision while out in the community3.Transportation assistance in order to access the community4.Assistance with personal shoppingRecommended Hours:4hr shifts 3x per week 12hrs per week x 4.5 weeks 54 hours per month

Quarterly Progress Reports Consumers are scheduled for a quarterly progress report call every threemonths with Maxim staff. The purpose of these calls is to review current goal plans and verify if thecurrent goals and personal assistance hours are still appropriate If current goals have been met and new goals need to be added our staff willwork with the consumer or guardian to create a new goal plan if needed. This new goal plan is added to the consumers progress report and sent over tothe CSC for review and approval.
