IntroductionThis booklet provides a list of BlueAdvantage’s network pharmacies. To get a complete description of yourprescription coverage, including how to fill your prescriptions, please review the Evidence of Coverage andBlueAdvantage’s formulary. If you have additional questions, please call our Member Services at 1-800841-7434, 7 days a week, from 8 a.m. to 9 p.m. Eastern Time. TTY/TDD Users should call 1-888-423-9490.Or, visit www.bcbst-medicare.com.We call the pharmacies on this list our “network pharmacies” because we have made arrangements withthem to provide prescription drugs to Plan members. A network pharmacy is a pharmacy wherebeneficiaries obtain prescription drug benefits provided by BlueAdvantage. In most cases, your prescriptionsare covered under BlueAdvantage only if they are filled at a network pharmacy or through our mail orderpharmacy service. Once you go to one, you are not required to continue going to the same pharmacy to fillyour prescription. You can go to any of our network pharmacies. We will fill prescriptions at non-networkpharmacies under certain circumstances as described in your Evidence of Coverage.BlueAdvantage’s Network PharmaciesOur pharmacies are listed alphabetically by county then city. Under each pharmacy we list the PharmacyType so you know what type of service each pharmacy offers. The key is as follows:R Retail. We have a statewide network of pharmacies where you can get covered prescription drugs as amember of our plan.LT Long Term Care are pharmacies that service long-term care nursing facilities. Residents of a long-termcare facility may access their prescription drugs covered under BlueAdvantage through the facility’s longterm care pharmacy or another network long-term care pharmacy.H Home Infusion. Pharmacies that provide infused (IV) medicine in your home per doctor’s order.E Extended day supply. The pharmacy provides an extended day supply of medications.IN Indian Health Service/Tribal/Urban Health Program. Only Native Americans and Alaska Natives haveaccess to Indian Health Service/Tribal/Urban Indian Health Program (I/T/U) Pharmacies throughBlueAdvantage’s pharmacy network. Those other than Native Americans and Alaskan Natives may be ableto access these pharmacies under limited circumstances (e.g., emergencies).24 24 Hours. The Pharmacy is open 24 hours a day.S Specialty. Pharmacies that can provide specialty prescription drugs. These medications may have specialstorage and handling requirements.Note: National Specialty Pharmacies and Mail-Order Pharmacies are listed on page iii of this directory.The key is also listed at the bottom of each Pharmacy Directory page for your reference.Network pharmacies outside the service areaWe have network pharmacies outside of the service area where you can get your drugs covered as a memberof our plan.
