ConnectorCareShopping GuideUse this guide to help you choose a ConnectorCare health insuranceplan through the Massachusetts Health Connector.Step 1: Know which plans you qualify forFirst, you’ll need to know which plans are available to you. You will have upto 5 plans to choose from, depending on where you live. If you appliedonline, you’ll be able to see your choices after you complete your applicationand then click the Find a Plan button. If you applied by paper application orby phone, you’ll need to call Customer Service.Step 2: Compare premium costsYour ConnectorCare plan choices have different monthly premium costs,depending on the insurance company offering the plan. Covered benefitsand out-of-pocket costs are the same for all plans. You can see a list ofcovered benefits for all plans on page 3, in question 1.Step 3: Find out whether the providers you wantare in a plan’s networkIf there are providers you want to use for care (such as doctors, hospitalsor health centers), make sure they are in a plan’s provider network before youenroll. Use the Find a Provider tool at ProviderDirectory.MAhealthconnector.orgto easily find plans that have the providers you want. Learn more about providernetworks on page 2.Step 4: Choose your new plan and enroll!Once you choose a plan, you can enroll online at MAhealthconnector.org,or call Customer Service to enroll by phone. You’ll need to pay your firstpremium (if you have one) to complete enrollment. Payment is always due bythe 23rd of the month before your coverage begins.1

Questions to help you get startedWrite down your answers to the questions below before you start shopping for healthinsurance. Your answers will help you decide which ConnectorCare health plans are best.1. Do you have prescription medications that youand anyone on your plan take regularly? Do you have a hospital or health center that youor anyone on your plan will want to be able to goto if you need care? If so, what is the name of thehospital or health center?Before you enroll in a plan, check to make surethe providers you want are in the plan’s network.Glossary of TermsBelow is a list of terms you will see and hear when choosing your health plan. These terms maybe new to you. Learning them will help you better understand your health insurance.Co-insuranceIf a health care service has co-insurance, you pay apercentage (part) of the cost for that service. Usually, youstart to pay co-insurance after you meet your deductible.It is not a fixed cost like a co-pay. The amount you paydepends on the total cost of the service.Co-payPremiumThe amount you pay each month for your healthinsurance. You must pay your premium every month,whether or not you use health care services. Yourpremium will always be due by the 23rd of the month.Primary Care Physician (PCP)A fixed price you pay when you get certain health careservices. Not all services require a co-pay.The primary (main) doctor that you go to for care.If you need a specialist, your PCP will coordinate thatcare and give you a referral.Health care servicesProvider networkHealth care services are things like going to the doctor,getting a prescription filled, or having an operation.Maximum out-of-pocket cost (MOOP)The most you pay in one year for health care services.Once you pay this, your plan pays for all of your coveredservices for the rest for the year.Plan TypeA provider network is a group of doctors, hospitals andother providers that works with the health plan to giveyou care. Before you enroll, make sure any providers youwant to use are in the plan's provider network. If youget care from a provider that is not in the plan's network,you could have to pay out-of-pocket for all or part ofthe costs.There are different ConnectorCare Plan Types thatyou may qualify for, based on your household size andincome. All of the plans in each Play Type have thesame costs for covered benefits.2

What benefits do the health plans cover?All of the ConnectorCare health plans you can chooseoffer these health benefits: Outpatient care – Treatment you get withoutbeing admitted to a hospital Inpatient care – Treatment you get inthe hospital Emergency room treatment, includingemergency dental Prenatal and postnatal care – Care before andafter your baby is born Mental health and substance use disorderservices – This includes behavioral healthtreatment, counseling, and psychotherapy Prescription drugs Services and items to help you recover ifyou are injured, or have a disability or chroniccondition – This includes physical andoccupational therapy, speech-language pathology,psychiatric rehabilitation, and more Lab tests, such as blood work or Pap smears Preventive services – Services to help keep youhealthy, like counseling, screenings, vaccines,and routine eye exams Pediatric services – Services for children, includingvision care2. How do the health plans differ?Differences between plans include: Which health insurance company offers the plan Provider networks (the doctor, hospitals, andother providers in the plan) Monthly premium Prescription drugs covered by the planIn Questions to help you get started, you wrote thenames of doctors and hospitals you want to be sure arein the plan you choose. Before you enroll in a plan,go to ProviderDirectory.MAhealthconnector.org. Use theFind a Provider tool tomake sure the providersyou want are in theplan’s network. Or, youcan call the insurancecompany directly. Youcan also go to theplan’s website and do a“provider search.”When you search or callan insurance companyto ask about a provider,be sure to give the plan’s full name.3. What kinds of costs do health plans have?There are four kinds of costs in health plans: Premium: This is the cost you pay each month,whether or not you use health care services. Youwill pay your monthly premium bill to the HealthConnector. The premium is always due by the 23rdof the month. Co-pay: This is the cost you pay at the time youget certain health care services. Co-insurance: If a health care service hasco-insurance, you pay a percentage (part) of thecost for that service. Usually, you start to payco-insurance after you meet your deductible. It isnot a fixed cost like a co-pay. The amount you paydepends on the total cost of the service.For ConnectorCare plans, you only pay co-insurancefor medical equipment such as crutches,wheelchairs, or respiratory (breathing) equipment. Maximum out-of-pocket cost (MOOP): This is themost you pay in one year for health care services.Once you pay this amount, your plan will pay for allof your covered services for the rest of the year.For ConnectorCare, there is a MOOP for the cost ofprescription medications and a separate MOOP forthe costs of other health care services.3
