STATEMENT OF WORKREPLACE SLIDING GLASS DOORSGOVERNMENT OWNED RESIDENCE X-189, OLIVOS 21.PRICES AND PERIOD OF PERFORMANCEThe contractor shall replace 8 sliding glass doors in the living room of a residence occupied by the U.S.Consulate General located in Urbanizacion Olivos 2 Club Mz.D solar 5-6, Guayaquil. The price listed belowshall include labor, materials, transportation and any additional fee included on this service. TheGovernment will pay the Contractor the fixed price for standard services that have been satisfactorilyperformed.1.1VALUE ADDED TAX.Value Added Tax (VAT) is not included in the CLIN rates. Instead, it will be priced as a separate Line Itemin the contract and on Invoices. The amount of VAT to be charged is 12%. Local law dictates the portionof the contract price that is subject to VAT; this percentage is multiplied only against that portion. It isreflected for each performance period. Full VAT amount is charged on all aspects of the contract.2.BASE PERIODThe firm fixed price for the period of the contract is:WORK DESCRIPTION1. Remove sliding glass doors and frames2. 3. EXISTING CONDITION.The currently sliding glass doors installed at the CGR are in very bad condition due to the normalwear and tear. The aluminum frames that support the glass doors are deteriorated and no longercan adjust the glass. Additionally, the sliding glass doors are not tempered glass.4. SCOPE OF WORKThe Contractor shall perform the following activities:1. Contractor shall decommission 8 old sliding glass door in the living room2. Contractor is responsible to move blinds and relocate them and the end of the project.3. Contractor is responsible for disposal of all materials.NOTE: Once the sliding glass doors are removed, the space must be cleaned, sealed andsecured if to remain open after work hours.4. Install eight (8) new sliding glass doors as per described below: see attachment #1 for details,measures are only for reference. Contractor shall perform measures. Ten (10) millimeters laminated (structurally resistant) tempered glass bronze color.Extreme weather resistant.Interior opening system, sliding system screens.Frames shall be made with rigid aluminum (8 centimeters perfil europeo) bronzecolor. Weatherproof aluminum structure is required.Sliding screen mesh is required.5. Contractor shall anchor the doorframes with 2 inches length stainless steel screws.6. Seal frames with high resistance (exterior type) anti-fungus silicon.7. Contractor shall install the eight (8) new sliding glass doors from the interior; security grillshall not be removed.8. Patch and paint affected areas.At the end of each work day, the Contractor shall remove all garbage from the premises after work isfinished. The contractor shall make efforts to protect floors, carpets, furniture and landscape fromdamaged caused by scaffolding, ladders, paint, debris and traffic.5. SCHEDULEThe Contractor shall submit to the COR a schedule and description of all tasks which the Contractorplans to provide, includes drawings. Submittal of the type windows and frames must be provided tothe COR for review and approval prior to installation.6. SITE VISIT AND QUOTATION SUBMITTALA site visit will be held on Wednesday, June 26, 2019 at 10:00, in Urbanizacion Los Olivos II Mz. Dsolar 5-6Guayaquil.

HOURS OF PERFORMANCEThe chosen vendor will transport all materials and personnel on his own; this means, contractor willtransport the materials and personnel to the house located in Urbanizacion Olivos 2 Mz. D solar 5-6 ,Guayaquil. Available times for services required are from Mondays through Fridays, from 08h30 to12h00 and 13h00 to 16h30, prior coordinating with Consulate for entry permission.The Contractor will also be responsible of any damage of premises resulting of this job, and restorationof the damages will be expected to occur within the 20 calendar days of the period given to completethe job.Performance Schedule Project must be finished and completed within fifteen days (15) calendar daysafter the Notice to proceed is given.Once job is finished, contractor will inform the U.S. Consulate to inspect and approve the workperformed.8. CONSTRUCTION PERSONNELA. Removal of PersonnelThe Contractor shall:(1)(2)(3)maintain discipline at the site and at all times;take all reasonable precautions to prevent any unlawful, riotous or disorderly conduct byor amongst those employed at the site; andtake all reasonable precautions for the preservation of peace and protection of personsand property in the neighborhood of the project against unlawful, riotous, or disorderlyconduct.The Contracting Officer may require in writing that the Contractor remove from the work any employeethat the Contracting Officer determines:

e orotherwise objectionable, orwhose continued employment on the project is deemed by the Contracting Officer to becontrary to the Government's interests.Construction Personnel SecurityAfter award of the contract, the Contractor has five (5) calendar days to submit to the ContractingOfficer a list of workers and supervisors assigned to this project for the Government to conduct allnecessary security checks. It is anticipated that security checks will take 5 days to perform. For eachindividual the list shall include:ooooFull NamePlace and Date of BirthCurrent AddressIdentification numberFailure to provide any of the above information may be considered grounds for rejections and/orresubmittal of the application. Once the Government has completed the security screening and approvedthe applicants a badge will be provided to the individual for access to the site. This badge may be revokedat any time due to the falsification of data, or misconduct on site. These passes must be displayed visiblyby all Contractor personnel working on site. The Contractor shall inform its employees to be used underthis contract that they may be subject to search by the Government when entering or leaving work. TheContractor shall return all passes upon conclusion of the contract.9. MATERIALS AND EQUIPMENTA. General. The Contractor shall provide all necessary supplies and equipment necessary toperform the work. No materials will be furnished by the U.S. Government.B. Selection and Approval of Materials1. Standard of quality. All materials and equipment incorporated into the work shall be new and forthe purpose intended, unless otherwise specified, and all workmanship shall be of good quality andperformed in a skillful manner as determined by the Contracting Officer.2. Selection by Contractor. Where the contract permits the Contractor to select products, materialsor equipment to be incorporated into the work, or where specific approval is otherwise required bythe contract, the Contractor shall give the Contracting Officer, for approval:(a) the names of the manufacturer;(b) model number;(c) source of procurement of each such product, material or equipment; and

(d) other pertinent information concerning sions,performance,capacity, andrating,unless otherwise required by the Contracting Officer.3. The Contractor shall provide this information in a timely manner to permit the Government toevaluate the information against the requirements of the contract. The Contractor shall provide asubmittal register five (5) days after contract award showing when shop drawings, samples, orsubmittals shall be made. The Contractor shall submit samples for approval at the Contractor'sexpense, with all shipping charges prepaid, when directed to do so by the Contracting Officer or COR.Installation or use of any products, materials or equipment without the required approval shall be atthe risk of rejection.C. Custody of MaterialsThe Contractor shall be responsible for the custody of all materials received for incorporation intothe project, including Government furnished materials, upon delivery to the Contractor or to anyperson for whom it is responsible, including subcontractors. The Contractor shall deliver all such itemsto the site as soon as practicable. If required by the Contracting Officer, the Contractor shall clearlymark in a manner directed by the Contracting Officer all items of which the Contractor has custodybut which have not been delivered or secured at the site, clearly indicating the use of such items forthis U.S. Government project.10. SCHEDULEThe objective of scheduled routine-services is to assure continuous, safe, and satisfactory operation ofthe Consular Residence. The Contractor shall submit to the COR a schedule and description of all taskswhich the Contractor plans to provide.11. CONTRACTOR RESPONSIBILITIESThe contractor is responsible for any damage, theft or loss caused by him and/ or his team at thework site. The contractor shall ensure that the work site is cleaned up every day and free of safetyhazards as a result of the contractor’s action or inaction.The contractor shall be responsible for safeguarding all U.S. government property which he or hisemployees, come into contact with during the work period.

The Contractor is responsible for employees’ safety and should follow the safety regulations of theEmbassy.The contractor must submit a chronograph work schedule (construction schedule) with theproposal for the project. Any delay in the time schedule will cost to the Contractor 200 per day forthe first five days and 500 per day for everyday past the fifth day. If a stop work is issued, thecontractor will have five calendar days to commence work again once a notice to proceed has beenissues not including the initial notice to proceed (NTP).ATTACHMENT #1
