INSTRUCTIONS FOR ONLINE REPORTING OF DOTFMCSA DRUG AND ALCOHOL TESTING RESULTSThe Federal Motor Carrier SafetyAdministration (FMCSA) collectsdrug and alcohol testing resultsin order to quantify the incidenceof drug and alcohol abuse in thecommercial motor vehicle industry;this data is used to determineFMCSA’s random drug and alcoholtesting rates for the next year.Each year FMCSA collectsdata from a random sample ofcompanies with fewer than 1,000drivers, and from all companieswith 1,000 or more drivers.Instructions: Employer SectionDrug and Alcohol TestingManagement Information SystemData is Incomplete01/31/XXXX 07:31:54 AMFMCSA notifies companies ofreporting requirements by email(and physical mail if emailbounces back undeliverable).Your notification includes ausername and password that allowsyou to complete the requiredsummary report online (or using adownloadable form). Instructions: Employer SectionDrug and Alcohol TestingManagement Information SystemSafety sensitive categories are jobpositions that require participationin a drug and alcohol testing program.FMCSA has only one safety-sensitivecategory, which is “drivers”. Anybodythat operates a vehicle that requiresa commercial driver’s license onpublic roads in intrastate or interstatecommerce, including a part-time oroccasional employee, is considered adriver. For example, if the number ofdrivers varied for your company during2017, you will need to calculatethe average number of positionsthroughout the year.A selection period reflects how oftena motor carrier conducts random drawsamong drivers for testing – typicallymonthly or quarterly. If you conductquarterly random draws, you wouldneed to determine the number ofdrivers in your company during eachquarter. For example, if your companyhad 30 drivers in quarter 1, 20 driversin quarter 2, 40 drivers in quarter 3,and 25 drivers in quarter 4, you woulddivide the total of 115 drivers by4 – resulting in an average of 28.75.Always round up the average results.Therefore, in this instance, the averagenumber of driving positions to beentered is 29.If you conduct monthly random draws(or more frequent random draws),add the total number of drivers for allselection periods, and then divide bythe number of selection periods to getthe average number of drivers. Thus,for monthly random draws, you wouldadd all drivers, and then divide by 12for the average number of drivers.Enter the total (or total average)number of drivers.Enter the total (or total average)number of drivers again.3. Instructions: Drug Testing Data SectionDrug and Alcohol TestingManagement Information SystemPre-employment testing (column 1).This column is the total for columns2-12; the system will complete itautomatically, so skip to column 2 tobegin entering data.In column 2, enter the number ofverified negative results.In column 3, enter the number ofpositive results.If your company had any positive testresults, you must enter the totals incolumns 4 through 8 for each drugfor which DOT requires testing (e.g.,Marijuana, Cocaine, etc.). Columns 4through 8 must equal the value youentered in column 3. For example, ifyou indicated that you had 5 positiveresults, you must ensure that columns4 through 8 add up to 5.If there are zeros for all columns,please use the button at the bottomthat will fill the remaining columnswith zeros for this row. Please followthe same procedure for the remaining5 columns (pertainingto refusals).For column 11, enter the numberof donors who did not provide anadequate specimen in 3 hours,and went through the Shy Bladderprocedure, but were determined bythe medical review officer to not have avalid medical condition that precludedgiving an adequate specimen. Thisis considered a refusal per 49 CFR §40.193 (d) (2) (i).For column 12, enter the appropriatenumber of refusals, based on refusalinformation in 49 CFR § 382.107 and§ 49 CFR 40.191.
