This mustbe recorded, a dollar amount applied,and reported to Medicareas a savings tothe hospice on an annual basis. During2016, CHC hospice volunteers workedOur TeamonUnreimbursed services – discounted andfree care, write-offs, etc. – was up againduring 2016 and hit another record level.Center for Hospice Care gave away servicesvalued at 2,222,810 during the year. Thisis a 4% increase from 2015. In addition, 2016was also the ninth consecutive year thatwe gave away more than we took in withfundraising.Our VisionTo be the premiere hospice and palliative careorganization for all end-of-life issuesAdministratiA key, historic fundraising / developmentbreakthrough was reached in 2016. It wasthe first time in 36 years that we raisedover 2 million in cash through fundraisingin a single year. The Hospice Foundationreported that 2016 development incometotaled 2,018,631. This number representscash received and does not include pledgesto any campaigns.2016 was another wonderful year for Centerfor Hospice Care and Hospice Foundation.The successes of last year will help us toaddress the future with confidence andenergy. Our goal each day is to have thecapacity to provide the next best day of careto anyone who needs something from CHC.Thank you for your interest and support.Compassion, Dignity, Innovation, Integrity,Quality, Service, StewardshipngIn the wake of ongoing Medicare Hospicereimbursement cuts that began in 2009,CHC diversified its revenue stream byacquiring Milton Adult Day Services. TheMilton programming is entirely directedtoward adults with dementia. Costs are paidfor by an Indiana Medicaid Waiver program,the Veterans Administration, and privatepay. We secured a new company name forownership purposes, Center for AdultDay Services, LLC. Last year, 13% of CHCpatients had a primary diagnosis ofAlzheimer’s, dementia, or anotherneurological disease. Since 2000, deathsfrom Alzheimer’s has increased by 89%and this is not expected to slow down.We believe Milton Adult Day Services is anOur new Veterans Memorial on the MishawakaCampus was dedicated on October 19 at aceremony attended by approximately 200people. This continues our efforts as anational We Honor Veterans partner program.Our Core Values17,574 hours, an increase of 6.4% from2015. These volunteers drove 52,633 miles,up 16.9% from 2015. Volunteers provideda total savings to CHC during 2016 of 435,617, an increase of 9% from 2015. Oursincere appreciation goes out to our manydedicated CHC volunteers.linseCouThe Center for Palliative Care, the region’sfirst freestanding palliative care outpatientclinic, opened to see outpatients onSeptember 19. Our consultation program,performed by nurse practitioners and ourboard-certified medical directors, isbeneficial for patients who require specializedmanagement of uncontrolled symptomscaused by advanced serious illness or theside effects of treatment.important component in CHC’s comprehensiverange of caring services.PatAt Center for Hospice Care (CHC) during2016, meeting our patient care missioncontinued to be our fundamental andprimary focus. The total number of patientsserved was 2,109, an increase from the prioryear. It includes investment increases during 2016.It should be noted that the combined 2016 operationalrevenue outpaced operational expenses and the combinedentities showed a positive net gain from operations alone.
