The responding contractors included Construction 57, Construction Engineering &Management, Wilson Construction, DAF Concrete, SD&C, Ohmshiv Construction, Tri Scapes,Sol Construction, and Summit Construction. We have reviewed the submittals based on the ITB,22-001. Each firm submitted all the required information in the ITB.The low bidder, DAF Concrete, has provided a bid total of 386,728.00 and is qualified toperform the work. They have a good reputation and have satisfactorily completed similarprojects. Based on these submittals, staff recommends contract award to DAF Concrete for ITB22-001. The bid tabulation is attached for reference.11

STATE OF GEORGIACITY OF DORAVILLERESOLUTION NO. 2022-RESOLUTION OF THE MAYOR AND COUNCIL OF THE CITY OF DORAVILLE,GEORGIA APPROVING AND ADOPTING FINAL TERMS AND CONDITIONS FORIMPLEMENTATION OF EMPLOYEE HEALTH BENEFITSWHEREAS, the Mayor and Council of the City of Doraville are the duly electedgoverning authority of the City of Doraville, Georgia; andWHEREAS, medical, dental and vision insurance plans are offered to employees andofficials of the City of Doraville, Georgia; andWHEREAS, the Mayor and Council desire to fix terms and conditions for participation inthe City’s employee health benefits plan that are fair and proper and in the best interest of the City.NOW, THEREFORE, IT IS HEREBY RESOLVED by the Mayor and Council of theCity of Doraville, as follows:SECTION 1. The employee health benefit schedules attached hereto as Exhibit “A” arehereby approved and adopted.SECTION 2. The City Manager is hereby authorized to execute and deliver any and allpolicies, agreements and other documents and take such further action as necessary or appropriateto implement the City’s employee health benefits plan effective July 1, 2022.SECTION 3. If any section, part or provision of this Resolution is held invalid orunenforceable, such invalidity or unenforceability shall not affect any other portion or provisionof this Resolution, and all parts, sections, and other provisions of this Resolution shall beseverable.SECTION 4. Insights If 100% of employees choose PPO, at a 10% cost-share, the City will pay an estimated 1.46 million toward employee medical benefits (less than this year’s to date); but weexpect at least 50% of employees will choose the lower cost HMO option Employee cost-share on the high-end plans was 5% until 2021. One-third of our employees make less than 50,000 a year. With increases across theboard (rent, gas, food) a substantial increase to out-of-pocket medical can bedevastating for these families. Offering a reduced rate for the HMO plan will encourage employees who will benefit fromthis plan to try it. Increasing the number of HMO participants helps Kaiser control theircosts, improves employee health outcomes, and drives down renewal rates.21

STATE OF GEORGIADEKALB COUNTYCITY OF DORAVILLERESOLUTION NO. 2022-A RESOLUTION APPOINTING ALEN WENG TO SERVE ON THE PLANNING COMMISSION OF THE CITY OF DORAVILLE FORA TERM THAT WILL EXPIRE APRIL 1, 2025; TO PROVIDE FOR SEVERABILITY; TO PROVIDE FOR AN EFFECTIVE DATE ANDOTHER LAWFUL PURPOSES.WHEREAS, the Planning Commission considers and makes recommendations on zoning ordinance amendments,rezoning and land use changes, conditional use permits applications, planning studies, and other planning initiativesto the Doraville City Council; andWHEREAS, the Planning Commission is comprised of five (5) members and a vacancy currently exists on theCommission which must be filled by the City Council; andWHEREAS, appointees must be residents of the City of Doraville for at least one (1) year prior to serving on thePlanning Commission; andWHEREAS, the City Council shall select one (I) member from each Council District and two (2) at-large memberswho may reside in any Council District; andNOW, THEREFORE, BE IT RESOLVED the Mayor and Council of the City of Doraville, Georgia, do herebynominate, confirm and appoint Alen Weng to the Planning Commission.Section 1. Appointment. The City Council hereby appoint Alen Weng to serve on the Planning Commission commencingMay 25, 2022 and ending April 1, 2025.Section 2. Severability. If any section, part, or provision of this resolution is held invalid, unenforceable, suchinvalidity or unenforceability shall not affect any other portion of this Resolution, and all parts, sections, whereasclauses, and other provisions of this Resolution shall be severable.Section 3. Effective Date.
