Revised on 04/2021 (update the company list)NOTICE OF EXAMINATIONTITLE:Examination for Certificate of Fitness for Inspection, Cleaning & Testing of SmokeDetectors (F-78/S-78)Date of Exam: Written exams are conducted Monday through Friday (except legalholidays) 8:00 AM to 2:30 PM.REQUIREMENTS FOR WRITTEN EXAMApplicants must be at least 18 years of age and must have a reasonable understandingof the English language. Applicants must apply the exam in person and bring thefollowing materials and required fee:1. Identification. Applicant must provide two forms of identifications; at least one form ofidentification must be government issued photo identification, such as a State-issuedDrivers’ License or Non Driver’s License or a passport.2. Letter of Recommendation. Applicants must present a letter of recommendation fromhis/her employer. The letter must be on official letterhead, and must state the applicant’sfull name, experience and the address where the applicant will work. If the applicants areself-employed or the principal of the company, they must submit a notarized letterattesting to their qualifications. Applicants must present a completed application for certificate of fitness ds/pdf/business/cof-application-form.pdf4. Without Required Documentation. Applicants not currently employed may take theexam without the required documentation. If the applicants pass the exam, FDNY willissue a temporary letter with picture for the job seeking purpose. The C of F card will notbe issued unless the applicants are employed and provide all of the requireddocumentation. (Exception: If COF has a school requirement, applicant will not be allowedto take COF exam until school is completed.)5. Special requirements for the: S-78/F-78 C of F:F-78 C of F: The letter of the recommendation shall be from buildingowner which must state that the employee will have the necessary tools toperform the smoke detector inspection, testing and cleaning. S-78 C of F: The employer must be an FDNY approved Smoke DetectorCompany or an FDNY approved Fire Alarm Company. A letter requesting fee waiver on the Agency’s official letterhead statingapplicant full name, exam type and address of premises; AND Copy of identification card issued by the agencyEXAM INFORMATIONThe F-78/S-78 exam will consist of 25 multiple-choice questions, administered on a“touch screen” computer monitor. It is a time-limit exam. Based on the amount of thequestions and reference material provided, you will have 37 minutes to complete thetest. A passing score of at least 70% is required in order to secure a Certificate ofFitness.Call (718) 999-1988 for additional information and forms.Special material provided during the exam: The tables which appear in the bookletwill be provided to you as a reference material when you take the exam at MetroTech,however, the booklet will not provide to you during the exam.Please always check for the latest revised booklet at FDNY website before you take pdf/business/cof-f78-s78-noe-study-materials.pdfIf all the requirements are met and pass the exam a certificate will be issued the sameday. Applicant who fails the exam will receive a failure report. To retake the examapplicants will need to submit a new application and payment.RENEWAL REQUIREMENTSThis Certificate of Fitness must be renewed every THREE YEARS. The renewal fee is 15. FDNY also reserves the right to require the applicants to take a re-examinationupon submission of renewal applications.You will receive a courtesy notice of renewal 90 days before the expiration date. However,it is your responsibility to renew your Certificate. It is very important to renew your C ofF before it expires. Renewals submitted 90 days (up to one year) after the expiration datewill incur a 25 penalty in addition to the renewal fee. Certificates expired over one yearpast expiration date will not be renewed. New exams will be required.To change a mailing address: Submit a letter requesting the change of mailing address and a copy of your C ofF with 5.00 fee.To change a work location, Submit a letter from your current employer (on company letterhead) confirmingthat you are an employee and stating your new work location with a copy of yourC of F and a 5.00 feeTo request a replacement certificate: Submit a driver’s license or passport, social security number, mailing addressand a 5.00 fee.This book is provided to the public for free by the FDNY.4

Renewal onlineIf you are an individual, make sure you have your 12 digit Certificate of Fitness AccessID. This can be found on your Renewal Notice. If you do not have your Renewal Notice,your Access ID is your 8 digit Certificate of Fitness number and the last four digits ofyour social security number.
