Section 1 – IntroductionThis directory provides a list ofKaiser Permanente’s plan providers.To get detailed information about your healthcare coverage, please see yourEvidence of Coverage.choosing a personal doctor to coordinate yourcare is easy. You can learn about eachdoctor’s certifications, specialties, languagesspoken, interests, and more on kp.org/scal tohelp you find the right fit for you.Your Primary Care ProviderIf you want to make a switch, you can changeyour doctor at any time and for any reason.You will have to choose one of our planproviders who are listed in this directory to beyour Primary Care Provider (PCP). The term“PCP” will be used throughout this directory.Generally, you must get most of your healthcare services from your PCP.Care for your unique needsYou can choose your personal doctor from oneof the departments listed next.Adult or Internal Medicine (Internists)Family Medicine (Family practitioners)Obstetrics-Gynecology (Ob-Gyn)Pediatrics For infants, children, adolescents,and teens.Nurse practitionersAt some facilities, you also have the option ofchoosing a nurse practitioner.Your PCP meets state standards and is trainedto give you primary medical care. He or she willusually practice general medicine, also calledinternal medicine or family practice. Womenmay also choose specialists in obstetricsgynecology whom the Medical Groupdesignates as Primary Care Providers. Atsome facilities, you also have the option ofchoosing a nurse practitioner (NP) to be yourPCP.Nurse practitioners are registered nurses whohave advanced education and training. Theycan diagnose and treat a wide variety ofmedical conditions, write prescriptions, orderand interpret lab and medical imaging tests,and educate and give advice to members andtheir families. They practice with a doctor’ssupervision and support according to carefullycontrolled procedures.Your PCP will give you most of your care. Ifyou need a specialist, your PCP will help youchoose a doctor in that field and work with himor her as part of your health care team.You can choose your PCP from any of ouravailable primary care doctors, nursepractitioners. The only servicesyou can get from non–plan providers are: Care at a Federally Qualified Health Center(FQHC) Care at an Indian Health Center Covered emergency ambulance services Covered emergency services and poststabilization care Covered family planning services Covered out-of-area urgent care Referrals to non–plan providers Some covered sensitive servicesType of careUrgent care appointmentRoutine (non-urgent)primary care appointment(including adult/internalmedicine, pediatrics, andfamily medicine)Routine (non-urgent)mental health careappointment with apractitioner other than aphysicianRoutine (non-urgent)specialty careappointment with aphysicianIf you obtain services that are not covered byMedi-Cal, you may have to pay for the cost ofthese services.The “plan providers” listed in this directoryhave agreed to provide you with your healthcare and vision services. You may go to any ofour plan providers listed in this directory;however, some services may require a referral.If you have been going to one plan provider,you are not required to continue to go to thatsame provider. In some cases, you may getcovered services from non-plan providers.AppointmentofferedWithin 48 hoursWithin 10business daysWithin 10business daysWithin 15business daysThe standards for appointment availability donot apply to preventive care services. Yourpractitioner may recommend a specificschedule for these types of services,depending on your needs. Preventive careservices may include physical exams, visionand hearing tests, immunizations, healtheducation, and prenatal care. The standardsalso do not apply to periodic follow-up care forongoing conditions or standing referrals tospecialists.To see a specialist, you usually need to getyour PCP’s approval first. (This is called gettinga “referral” to a specialist.) However, you donot need a referral to get specialty care fromplan providers in Ob-Gyn, optometry, mentalhealth, and chemical dependency.Timely access to telephoneassistanceThe following standards for appointmentavailability were developed by the CaliforniaDepartment of Managed Health Care (DMHC).In addition, the following standards foranswering telephone inquiries were developedby the DMHC. These standards require healthplans to answer the following telephoneinquiries within specified time frames.If you prefer to wait for a later appointment thatwill better fit your schedule or to see thepractitioner of your choice, we will respect yourpreference. In some cases, your wait may belonger than the time listed if a licensed healthcare professional decides that a laterappointment will not have a negative effect onyour health.This information can help you know what toexpect when you request an appointment.8 For telephone advice about whether youneed to get care and where to get care,plans must answer within 30 minutes,24 hours a day, 7 days a week. For customer service inquiries, plansmust answer within 10 minutes duringnormal business hours.

Interpreter services at no cost to youFor ease and continuity of care, we encourageyou to go to a Plan hospital EmergencyDepartment if you are inside our service area,but only if it is reasonable to do so, consideringyour condition or symptoms.If you need interpreter services when you callus or when you get covered Services, pleaselet us know. Interpreter services, including signlanguage, are available during all businesshours at no cost to you. We highly discouragethe use of minors or family members asinterpreters. For more information on theinterpreter services we offer in NorthernCalifornia and Southern California, call ourMember Services Contact Center: If you have an emergency, we will talk with thedoctors who are giving you emergency care tohelp manage and follow up on your care. If your emergencycare is provided by non-plan providers, we willtry to arrange for plan providers to take overyour care as soon as your medical conditionand the circumstances allow. It is veryimportant that your non-plan provider call us toget authorization for post-stabilization carebefore you receive the care from the non-planprovider. You will only be held financially liableif you are notified by the non-plan provider orus about your potential liability.Bills from non-plan providersIf you receive a bill from a non-plan provider,you must file a claim as soon as you can.
