Treatment and Management Guidelines for von Willebrand DiseaseType of vonWillebrandDiseaseType 1 or Type 2The type 2 VWDknown as ‘pseudoVWD or VWFplatelet type' willonly respondto a platelettransfusion - calla hematologist.Major life-threatening bleeds(ex. - head injury, GI bleeding, severemenorrhagia, etc.)Factor concentrate containing bothFVIII (8) and von Willebrand factor(eg. Humate P ):60-80 Ristocetin cofactor units/kg IVPackage insert will instruct as to rate pervolume.Note: monoclonal or recombinantfactor VIII (8) products do NOT havevon Willebrand factor in them andwill not stop the bleeding.Other bleeds(ex. - sutures, nosebleed, mouth bleed, dentalextractions etc.)Known to respond to desmopressin(DDAVP ):Desmopressin 0.3 mcg/kg IV in 50 ml ofNormal Saline over 30 minutes orsubcutaneously if volume can be givensafely. Recommendation: a maximum doseof 20 mcg.Mucosal bleeding - anti-fibrinolytics (pg. 9)For patients who do not respond to desmopressin:Give a factor concentrate containing bothFVIII (8) and von Willebrand factor(eg.
