[Nursing Admissions & OrientationInformation].Please submit all requirements in separate PDF files to psahhs@tncc.edu; do not send several requirementscombined into one PDF. Do not separate multiple pages of a document into separate PDF files.*** PROGRAM CHANGE FORM ***To change your program of study to A.A.S. Degree Nursing for FA22, please fill in your personal info at the top, add yourcurrent program of study in the first box (pre-nursing, social science, etc.) and sign/date the bottom of the form[Program Change Form].  Please email this form to PSAHHS@tncc.edu by Wednesday, May 11, 2022.1. Complete a drug screen and background check through Castle Branch. You must register by Wednesday,May 11, 2022, and submit your sample to Lab Corp. by Monday, May 16, 2022. The package code forRegistration will be posted here after Orientation. The nursing office will receive your report; no need to submita copy.2. Order uniforms -- Processes for ordering uni forms are changing. Uniforms can now be purchased throughthe Bookstore where financial aid may be used, or scrubs can be purchased from any supplier. Moreinformation will be posted as it becomes available.[Editor Note: Add scrub description.color, pockets, lab coat, etc. Lab supplies/equipment in bookstore too?]The Uniform Connection is the long-standing scrub/uniform provider and are taking orders from Monday, May 9

through Monday, May 16 between the hours of 10:30 AM and 4:30 PM (Closed on Sundays). Payment is due atthe time of the order by Cash or Credit Card; personal checks are not accepted.   Masks and social distancing arerequired. Please bring this order form for your sizing and order placement. [Uniform Order Form]An all-in-one cost-effective package of your clinical equipment (Stethoscope, Blood Pressure Cuff, Watch, PenLight, Hemostat, and Bandage Scissors) is offered through The Uniform Connection and will be available to orderwith your uniform order.PN-RN Students must purchase uniforms through the Uniform Connection by May 4.3. Provide a copy of your AHA BLS Certification for Healthcare Providers. BLS for Healthcare Providercertification MUST be through American Heart Association or through one of their approved agencies only. (RedCross certification is not accepted.) The handbook will be revised this summer and postedbefore classes begin in August. Please sign and upload the Acknowledgment form for the Nursing StandardsAffirmation Statement (Page 6) found in the FA22 Nursing Forms Packet [FA22 Nursing Forms Packet] toCastleBranch by by Friday, June 24 and send the entire completed packet to PSAHHS@tncc.edu as well. HepB isthe only requirement that can be waived. (PN-RN Students by May 2nd.)TB Requirements:Negative IGRA test results from either (1) QuantifFERON -TB Gold In-Tube test (GFT-GIT) or (2) T-SPOT Btest (T-Spot)ORMantoux Tuberculin Skin Tests (TST)/2-step PPD -- Complete the survey portion at the top. Your provider willcomplete the middle box for Step 1, and then return to your provider 2 weeks later to complete Step 2. Yourprovider will need to complete the Thomas Nelson form [TNCC PPD Form]If you have a positive TST/PPD or IGRA, documentation of a negative chest x-ray after the positive TST isrequired along with the completed survey at the top of the TNCC PPD Form.Upload all TB documentation to CastleBranch AND email to PSAHHS@tncc.edu by Friday, June 24th. (PN-RNStudents by May 2nd.)2022-23 Flu Shot:RN Students - A flu shot is not due at this time. A 2022-23 flu shot will be due in September when the newstrain becomes available.PN-RN Students - A 2021-22 flu shot is required for your SU22 NSG200 clinical day. COVID vaccinations are required by our clinical sites.All students attending clinical rotations will need tocomplete COVID vaccination series and boosters as necessary in order to attend clinical. This requirement cannotbe declined, and only approved medical or religious exemptions will be accepted. A medical exemption form canbe obtained through your medical provider. Please visit 12VAC5-110-80. Exemptions from immunizationrequirements. (virginia.gov) for more information on a religious exemption.   If you have a medical exemption forthe COVID shots, please complete the RHS Vaccine Exemption Form [RHS Vaccine Exemption Form].The COVID-19 Assumption of Risk form must also be completed and signed/dated. The first-semester book package is estimated at 1,300.00 Please contact Financial Aid Office andthe Bookstore for additional textbook information.PN-RN CSC students must purchase both 1st and 2nd semester book packages before Summer 2022 classes begin. Concurrent and Traditional BSN Programs at ODU – The concurrent program allows students enrolled in ThomasNelson’s RN program to take on-line courses at ODU toward their BSN at the same time.After Thomas Nelsongraduation and program completion, you are eligible to take NCLEX, and then a few months and final ODU projectlater, a BSN from ODU is earned. This is an aggressive educational track, so it is highly recommended that studentsdo not take on the extra work required by this double major until the 2nd semester at Thomas Nelson; adjust to thenursing course/workload before you begin the concurrent program.If you prefer a traditional track for your BSN, ODU also offers an RN to BSN program that starts after ThomasNelson’s program is complete.
