Dear Friend of CRCH,Charles River Community Health is pleased to announce our Waltham fundraiser, Spring intoSummer, an event celebrating the health center’s commitment to the Waltham community formore than 12 years! This breakfast and groundbreaking event will be held on June 21st from8:30 to 10 am, at our new health center location, 43 Foundry Ave, Waltham.I’m writing to enlist your participation in this special event for Charles River Community Health, which will draw supporters dedicated to helping us with our life-changing work. Enclosed is asponsorship sheet detailing opportunities for individuals and businesses to support ourWaltham fundraiser.Since opening in September, 2004, Charles River Community Health’s Waltham health centerhas grown tremendously to meet the needs of the community. About half of the 12,750patients we see each year receive care at our Waltham site, 5,300 of whom live in Waltham.CRCH provides essential primary care services in a welcoming and inclusive manner to allcommunity members, including low-income and immigrant populations, many of whom wouldhave no other place to go for care. As we have outgrown our current facility, we have leasednearly 20,000 square feet of space at 43 Foundry Avenue in Waltham, which will offer a stateof-the-art and more spacious health center for the Waltham community. As you will see fromthe enclosed materials, we will be able to increase to 16 medical exam rooms, five dental examrooms, and three behavioral health counseling rooms to serve additional patients, and havemuch needed meeting space available for community events, and group visits for patients. Our mission and philosophy have always been focused on empowering patients to take chargeof their health and well-being. We hope to continue our mission in Waltham and open ourdoors to more patients from community than ever before, and helping to bring health andwellness to everyone in our neighborhood. Help us honor this important milestone at ourfundraiser in June. Many thanks for your consideration, and we look forward to seeing you!Kind regards,Caroline Grossman, ChairDevelopment & Capital Campaign CommitteeElizabeth BrowneExecutive Director495 Western Avenue, Brighton, MA 02135 PHONE: 617-783-0500 ADMIN FAX: 617-783-5514 CLINICAL FAX : 617-787-4359564 Main Street, Waltham, MA 02452 PHONE: 781-693-3800 FAX: 781-693-3817

With a strong focus on primaryand preventive care, Community HealthCenters are at the forefront of innovationsin healthcare that aim to eliminate healthdisparities and improve access to qualityand compassionate care. In Massachusetts,49 community health centers provide highquality, comprehensive care to nearly 935,000residents through 285 sites statewide. Our HistoryFounded in 1974 by a group of Allston-Brighton communityactivists under the leadership of Joseph M. Smith, our community health centerhas grown from a two-room clinic to a full-service, freestanding health centerwith four clinical sites located in Allston-Brighton and Waltham. In 1997, CharlesRiver Community Health (CRCH) moved to a stand-alone site located a block fromthe public housing complex where it originated. In 2004, CRCH opened a site indowntown Waltham to fill the void left by the closing of Waltham Hospital. Inthe spring of 2008, CRCH opened satellite sites at the Gardner Pilot Academy andJackson/Mann School to bring additional access to care to the community. In Fall2015, we moved into a new, 45,000 sq. ft. state-of-the-art facility in Brighton withenough space short- and long-term to meet growing community needs. Our MissionCharles River Community Health’s mission is to improve thehealth and well-being of the communities of Allston, Brighton, Waltham andsurrounding areas by providing quality, comprehensive, coordinated care that ispatient-centered, family friendly, and community focused. Our PatientsCRCH CY2016 Total PatientsServed: Approximately 12,700COUNTRY OF ORIGIN # of PATIENTSU.S.2,402Guatemala1,782Brazil533El Salvador324Mexico222Our 2016 Patient DemographicsThe cost of living in our serviceareas is well above the national average and our familes are struggling to keep up,many working two or more jobs.
