UPON YOUR ARRIVALYour Rights as a PatientAt Stony Brook Medicine, we respect your rights as apatient and strive to provide considerate, respectfulhealthcare based on your individual needs. Whenyou are admitted to the hospital, you will receiveinformation that outlines your rights under New YorkState law. If you have a question about your rightsor need more information, please speak to yournurse, doctor or social worker. You can also call theDepartment of Patient Advocacy at 4-2880. Pleasesee pages 24 and 25 for more information.Positive IdentificationTo ensure patient safety and security while meetingstandards of compliance, when you are beingadmitted to Stony Brook University Hospital, youwill be asked to provide positive identification. Ingeneral, a government picture ID, such as a driver’slicense, state ID or passport serves to meet thisstandard. If you have not already documentedpositive patient identification at the hospital, pleasecontact a Patient Access Services representativeat 4-1870.Selecting a Caregiver:The New York State CARE ActDuring admission to the hospital, you will be askedif you would like to name a “caregiver” who can helpyou with tasks at home should you need assistance.This could be a family member, friend, neighboror anyone else who is significant in your life. Thisis not necessarily the family or friends who arehelping you with your medical decisions and doesnot have to be your healthcare proxy or next ofkin. Your caregiver will be included with you in anynecessary teaching regarding your discharge plan,and any other instructions and demonstrations byhospital staff related to things that may need to beprovided for you after discharge. This could includemedication, dressing changes and physician followup appointments. As your caregiver, your medicalinformation may be shared with them, as necessary.NY Care Information GatewayIf your doctors are not affiliated with Stony BrookUniversity Hospital, you can share your medicalinformation with them if you participate in theNY Care Information Gateway. The NY CareInformation Gateway is a regional health informationorganization (RHIO) that uses a secure computernetwork to store, protect and share patienthealth information electronically. You have theright to decide who can access your healthinformation — doctors, nurses and other healthcareproviders — which will help them make moreinformed decisions regarding your care. We willneed your consent allowing us to access yourhealth information, as well as to share your medicalrecords while you are being cared for at Stony BrookUniversity Hospital. One of our Patient AccessServices representatives will ask you for yourelectronic signature.Identification BandsAfter being admitted to the hospital, you will begiven a patient identification band, which willbe placed around your wrist. The band containsimportant information about you to help us meetyour individual needs. Please wear it throughoutyour stay. If your band is uncomfortable, is removedor falls off, let your nurse know immediately.Personal Items and ValuablesWe will supply hospital gowns, slippers and personaltoiletries, although you may be more comfortablehaving your own nightclothes. Please do notkeep personal items and valuables in your roomsince the hospital cannot be responsible for theirsafekeeping. Ask a relative or friend to keep theseitems and any cash over 25 for you. If you areunable to have someone take your items, we canhave them placed in the Cashier’s Office safe (Level5 of the main hospital) until you leave. To retrieveyour personal items and valuables, the Cashier’sOffice hours are: Monday - Wednesday, 8 am to 6 pm Thursday - Sunday, 8:30 am to 4:30 pm Emergency Department Cashier - 24 hours a day/seven days a weekPersonal medications and medical devices arepermitted only with authorization of your attendingphysician. Dentures, Eyeglasses and Hearing AidsWe will provide denture cups and patient belongingbags for your dentures, eyeglasses and/or hearingaids during your stay. Be careful not to wrap them intissue, store them in a glass, or leave them on a foodtray, your bed or on tabletops. Please keep theseitems in the drawer in your bedside table when youare not wearing them. If you wish to store them,please call Transport Services at 4-2980.Cell PhonesTo help promote a healing environment, please becourteous when using your cell phone. To protectpatient confidentiality, the use of camera phonesand/or visual/audio recording is strictly prohibited.Hospital Directory Opt OutAs a patient, you have the right to opt out and notbe listed in any Hospital Directory of patients. Thatmeans when someone asks for you by name, evenfamily and close friends, no information will beprovided, including your presence here.Patients who do not opt out of the HospitalDirectory may either limit the individuals who mayvisit, or indicate individuals who are not permittedto visit (whichever is fewer). For example, you canrequest that (name of person) may not visit.6help. If you are a smoker, ask your doctor or nurseabout nicotine replacement. The New York StateSmokers’ Quitline offers free counseling and nicotinereplacement. Call (866) 697-8487 (866-NY-QUITS)or visit nysmokefree.com online. The Suffolk CountyDepartment of Health offers a six-week program toassist you with quitting. The service is available Monday through Fridayfrom 8 am to 4:30 pm. The shuttle’s schedule isposted at all off-site locations and is available at theInformation Desk. For more information, callCourier Services at 4-2640.Hospital GarageTemporary parking is available in the hospital garagefor a fee based on an hourly rate (with a maximumper day rate), or with the purchase of a parking pass.Current rates are posted near the entrance gates tothe garage. Please call the garage office at 4-6607or 4-6608 for details. The office is open Monday toFriday, 6:30 am to 9 pm; Saturday, 1 to 7 pm; andSunday, 11 am to 5 pm.Your Hospital RoomParking PassesParking passes can be purchased allowing visitorsaccess to the hospital garage only. Passes are validfor 30 days from the date of purchase. Parkingpasses can be purchased at the parking garageoffice on Level 1 of the garage (see above forhours of operation). Passes may be purchasedwith cash, check or credit card. For current chargesand additional information, call the garage officeat 4-6607 or 4-6608.A remote control is available for you to call your nurse(known as a call button in the main hospital andpillow speaker in the Hospital Pavilion). When youpress the button, the nursing staff is alerted, and astaff member will respond as soon as possible.Valet Parking ServiceValet parking for visitors is offered for a fee at themain hospital entrance. The hours are Monday toFriday (excluding holidays), 5:30 am to 9 pm, andSaturday, Sunday and holidays, 9 am to 9 pm. Thefee is collected at the valet booth. A parking garagevalidation cannot be used for valet parking. You mustpresent your valet ticket to the attendant to claim yourvehicle. For current fees, call 4-9568. Valet parking atthe Emergency Department (ED) valet location is freefor patients only in the ED, Comprehensive PsychiatricEmergency Program (CPEP) and Maternity UrgentCare with validation from one of those three locations.The ED valet is open 24/7. All others must use thevalet at the main entrance.Our patient rooms are grouped by medicalspecialties, such as surgery, medicine and obstetrics.Every effort is made to place you in the unitcorresponding to your diagnosis. Private rooms maybe requested through the Department of PatientAccess for an additional charge. Medical insurancedoes not usually cover the cost of a private room.Call Button/Pillow SpeakerDuring the NightFor your safety, we ask that you stay in bed afteryou have been made comfortable for the night.Unfamiliar surroundings and medications maycreate a hazard if you get out of bed. For assistanceduring the night, use your call button/pillow speaker,and your nurse will help you.myStationPatients in the Hospital Pavilion have accessto myStation, a personal library of information,entertainment and education. This TV systemallows you to review your information, your room’sservices and take part in a satisfaction survey. Byusing the keyboard in your room, you can scrollthrough the system to explore available features. Ahow-to video is available to guide you through theprocess. Concierge Services/Information DeskThe Concierge Services staff is available from7 am to 8:30 pm to help you should you have anyquestions, need directions or need any help beyondthe routine of your medical care. To reach theConcierge Desk, call 4-7686.Interpreter ServicesStony Brook University Hospital offers free interpreterservices to you and your family during your stayor when visiting our hospital. Interpretive servicesinclude dual handset telephones, on-site Spanishinterpreters, American Sign Language interpretersand video interpretation. Please call Patient Advocacyat 4-2880 to make arrangements or speak with yournurse. See pages 24 and 25 for more information.Additional Services for Patients withHearing ImpairmentsStony Brook University Hospital provides manyresources for our patients with hearing impairments.Please contact Patient Advocacy at 4-2880 forassistance or ask your nurse.Quiet HoursTo provide our patients and visitors with a quieterenvironment, we are actively working to reduce noiselevels throughout the hospital. Studies show thatnoise levels directly affect physical and mental healing.So that our patients receive needed rest, Quiet Hourson the inpatient units begin at 2 pm for one hourand then again overnight from 9 pm to 6 am. As ourstaff makes every effort to keep noise levels to aminimum, we ask that you contribute by keepingconversations low, lowering TV and radio volumes,putting phones on silent or vibrate, and avoidcongregating in hallways outside of patient rooms.Thank you for helping to make our hospital a betterplace for all.DiningWholesome, nourishing and well-balanced mealsare important to your treatment and recovery.Your doctor has prescribed a diet suitable for yourmedical needs, and the hospital offers a variety offood choices within that framework. If you practicedietary restrictions for religious or personal reasons,be sure to tell your nurse. If at any time you havequestions about your diet, meals or nutrition, your8Stony Brook University Hospitalnurse will arrange for you to meet with one of ourregistered dietitians.Distinguished Dining Room ServiceThe Room Service Call Center is open from 7 am to7 pm and can be reached at 4-2900 or 8-3463.Meals for patients are generally served as follows:Breakfast . 7 to 10 amLunch/Dinner.noon to 7 pmDining Facilities Located in the Hospital The Market Place Café is located just off themain lobby on Level 5, and offers a variety offood and beverages. Monday through Friday: Midnight to 10:30 amand 11 am to 8 pmSaturday and Sunday: Midnight to 8 pm The Skyline Deli is located just off the main lobbyon Level 5, and offers sandwiches, snacks, and hotand cold beverages.Monday through Friday: 10:30 am to 3:30 pmClosed on Saturdays and Sundays Starbucks Café is located in the main lobby onLevel 5, and offers specialty coffees and teas,pastries, baked goods, hot breakfast items, andhot and cold sandwiches.Daily: 6 am to midnight Jamba Juice is located on Level 5 of the HospitalPavilion, and offers smoothies and fruit bowls.Open daily Panera Bread is located on Level 5 of the HospitalPavilion, and offers freshly baked breads, pastries,breakfast items, soups, salads and sandwiches.Open dailyFood for Patients from External SourcesStony Brook discourages visitors from bringingin food from any external source. The hospital isresponsible for providing our patients with foodthat is safe, sanitary and clinically appropriate. If youfeel it necessary to bring in food, please discuss itwith the patient’s nurse or doctor to make sure itconforms to the patient’s diet. The hospital does nottake responsibility for the wholesomeness, safetyor nutritional content of foods supplied by visitors.The hospital will not accept foods to store or reheat.Food items may not be left in the patient’s room.

Telephone ServiceAll rooms, except those in the intensive careand psychiatric units, have bedside telephonesthat provide complimentary telephone service. Ifthere is an issue with your telephone equipmentor the service, please notify your nurse, nursingassistant or another staff member.Stony Brook University has four major exchanges:444, 632, 638 and 216. You may see abbreviatedtelephone numbers such as 4-1234 (444-1234),2-1234 (632-1234), 8-1234 (638-1234) or6-1234 (216-1234). Videos areavailable in English and Spanish.Additional complimentary television servicesinclude regularly scheduled programs inCablevision’s Family Cable package. If thereis an issue with your television or the service,please notify your nurse, nursing assistant oranother staff member. See page 21 for a listing oftelevision channels.Wireless Internet Access forPatients and GuestsStony Brook operates a wireless (Wi-Fi) networkfor patients and guests, free of charge. Usingyour Wi-Fi-enabled device, you can access theinternet throughout our medical campus.To Set Up Your Connection1. On your device, access the Wi-Ficonnection, which is usually found withinthe device’s settings.2. Select our Patient/Guest Wi-Fi network.Name: SBMED-Guest.3. Once connected, open a web browser on yourdevice. The browser should automatically opena web page that will give you details on how toregister and connect to Wi-Fi.4. If the web page does not open, try opening anon-secure web page like neverssl.com to forcethe browser to open the registration portal.5. On the registration portal, select one of twooptions: One for those who have alreadyregistered and the other for those who need anaccount. If you have not registered, select theoption under “Need an account.”6. Provide your name, cell phone number andmobile carrier information. Once submitted, thepassword information is sent via text messageto the provided number.7. When you have registered and received theWi-Fi username/password via text message,select “Patient and Guest Login.”8. You can now log in using the telephone numberyou provided and the password you received viatext message.If you registered and did not receive the textmessage, do not have text messaging or needadditional assistance, please call the Stony BrookMedicine Help Desk at (631) 444-4357 (4-HELP).Be sure to secure all valuables, including laptopsand smartphones at all times. The hospital cannotbe responsible for loss or damage to such items.Please Note: The hospital reserves the right tomonitor Wi-Fi activity, filter content and otherwisecontrol or prohibit usage.Mail and FlowersConcierge Services delivers flowers and mail sentto you while you are a patient. Latex balloons arenot permitted in hospital rooms. YOUR HEALTHCARE TEAMMedical StaffYour doctor, who is on the faculty of theRenaissance School of Medicine at Stony BrookUniversity, manages your patient care team.Several doctors may be involved in your care, as wellas other staff members: Attending physicians or hospitalists areresponsible for patient care and overseeresidents.– House physicians are fellows, residents andinterns, who have finished medical school andare in training. They are directly involved inpatient care.– Fellows are fully credentialed physicians whohave completed their residency and elect tocomplete further training in a subspecialty.– Residents are physicians undergoing three toseven years training to become board certifiedin a specialty, such as pediatrics or oncology.– Interns are new physicians in the first yearof residency. Nurse practitioners, licensed practical nurses,physician assistants, nurse midwives, otherallied health practitioners and hospital clinicalassistants also provide care.Nursing StaffThere may be many nurses and nursing staffmembers involved in your care. The nursing staffworks closely with the medical staff and othermembers of the healthcare team. The nursing teamincludes the nurse manager, clinicians,nurse specialists, nurse educators, registerednurses, nursing assistants, technicians and nursingstation clerks. Nurse managers are responsible for thesupervision of the nursing staff. Nurse specialists have advanced training in aspecific nursing specialty and are responsible forassuring that quality care is received by patients. Nurse educators provide education and training tonursing staff. Clinicians are the nurses who are responsiblefor operational management of a unit during aparticular shift.10Stony Brook University Hospital A registered nurse plans and coordinates youroverall nursing care each shift and assigns tasksto other members of the nursing team such asclinical assistants, licensed practical nursesand technicians.Care Management Staff Case managers work with your team tocoordinate your care while you are in thehospital and assist with discharge planningfor those patients who need care aftertheir hospitalization. Social workers help you and your family tomanage the stress of illness, hospitalizationand treatment, and help plan for your careafter you leave the hospital.Your team strives to provide comprehensiveand coordinated care throughout your stay.As a major teaching resource, Stony Brooktrains health professionals, including doctors,nurses, medical students, physical therapistsand social workers. Should you wish to hire a privateduty nurse, speak with your nurse or contact theNursing Office for a list of approved agencies.Please note that private duty nurses are notmembers of the hospital staff. You will beresponsible for forwarding payment directly tothe agency you select.VolunteersVolunteers assist with a variety of servicesthroughout the hospital. You will be able to identifyvolunteers by their identification badges and theirroyal blue-colored jackets.Staff IdentificationEach staff member, student-in-training andvolunteer is required to wear a clearly visibleidentification badge bearing a photograph, nameand job title. The ID badge confirms his or herstatus at the hospital and specialty area. Donot hesitate to ask staff members to identifythemselves. If someone you do not know entersyour room, it is your right to ask who he or sheis. If you have any doubts about the identity orthe intentions of any visitor, call a member of thenursing staff immediately.

Coordination of Your CareYour healthcare team coordinates your care inmany ways, including discussions with your referringphysicians. You may also notice staff using mobiledevices since this is also an important way that yourcare team communicates with each other.QUESTIONS FOR MY DOCTOR
