Trade-in ProgramPolycom Conference PhonesTerms and Conditions1. The equipment being traded in must be in use and in working condition prior to destruction to be eligible for thisrebate program. See Claim Form.2. End Users must purchase a new qualified Polycom voice conferencing unit from a Polycom Channel Partner and trade inan eligible voice conferencing phone to be eligible for this rebate program. One trade-in unit must be traded in for everyqualifying new unit purchased.3. Polycom must receive a trade-in Claim Form with all required documentation within 90 days of purchase. Note: If thetrade-in documentation, Recycler Certification or digital photos are not received within 90 days from date of purchase,program participation will be declared incomplete, and the End User customer will not receive the rebate.4. Required documentation includes: completed Claim Form, Letter of Authentication and Destructionor RecyclerCertification, reseller invoice. Options for trade-in equipment are – 1) a receipt showing donation of the oldequipment to a charitable organization, 2) recycle the old equipment and submitting a receipt validating proper disposal,3) send equipment back to Polycom at the address below.5. If trading in a SoundStation IP 4000, invoice with purchase date of conference phone being traded in must also besubmitted, showing purchase date greater than 12 months prior to purchase date of new conference phone. This is notrequired if trading in any other approved Polycom or competitive conference phone.6. Rebates checks will be issued and sent directly to the End User. If a customer, such as an education or stateentity, is unable to accept rebate checks directly from Polycom, a check may be issued to the reseller. In this case, thereason must be documented on the Claim Form and authorized by the End-User customer. The reseller must provide therebate in advance to the End User customer. The reseller invoice must include a line item deduction offthe total invoice amount for the full rebate amount to validate the up front rebate was given.7. The End User will be sent an email confirmation when their claim is received. Polycom is not responsible for lost ormisdirected shipments, email, mail or faxes.8.Polycom has the right to call the End User at any time for verification purposes.9.This offer is only available to End User customers who purchase from a Polycom Channel Partner.10. This offer is void where prohibited or otherwise restricted by law. Rebates will be sent within 6 weeks of receiving all thenecessary documentation (completed Claim Form, Letter of Authentication and Destruction or Recycler Certification andproof-of-purchase (reseller’s customer invoice) of the trade-in equipment.11.
