Inova TransitionalServices helps discharged patients transition back intothe community with a phone support program as wellas community-based discharge clinics that providefollow-up care and bridge the gap between hospitaland home.This report introduces a vision fast becoming realitythat will benefit and be a transformative fixture to ourcommunity for decades. INOVA ALEXANDRIA HOSPITAL WOMEN’S TOWERA Dedicated, Nurturing Environment Enhances Women’s Health ServicesWomen throughout the Alexandria community nowhave a comprehensive, state-of-the-art center devotedexclusively to their healthcare needs. Completed in2014, Inova Alexandria Hospital Women’s Towerfeatures all private rooms, expanded services,advanced technology and updated décor to promotea more comfortable, healing environment.The renovations are part of IAH’s long-range plan toinvest in its environment, meet community needs andmake the patient experience the best it can be. “Withnearly 4,000 births each year, a large percentage ofhigh-risk pregnancies and a growing aging population,we understand the critical need for women’s healthservices in the region,” said Susan Carroll, CEO, IAH.“The new space represents a 5.2 million investmentto transform the hospital’s existing patient tower intoa contemporary center dedicated solely to the careand support of women.”Featuring enhanced privacy and comfort, the Women’sTower offers the best possible pre- and post-birthingexperience, as well as excellent recovery followingbreast and gynecologic surgeries. INOVA DWIGHT AND MARTHA SCHAR CANCER INSTITUTEComprehensive Cancer Care Close to HomeWith the opening of the Inova Dwight and Martha ScharCancer Institute (ISCI) at Inova Fair Oaks Hospital (IFOH),residents in the region now have access to coordinatedcancer services at their doorstep. All components aretogether in a beautiful new facility where high-techscience flourishes alongside compassionate care andtreatment.Every effort is directed at enhancing the patientexperience. The space includes state-of-the-artradiation oncology services, Inova Breast CancerProgram and a patient education center. In addition,numerous counseling and support services areavailable on site, including: A new satellite location for Inova’s acclaimed Lifewith Cancer program, offering education andsupport groups for cancer patients and their familiesfree of charge Cancer rehabilitation services from STAR-certified,licensed therapists who meet the nation’s premierstandards in oncology rehabilitation care“Each individual’s physical, emotional and spiritualneeds are addressed, from diagnosis through treatmentto follow-up care,” said Ann Miner, Director of CancerServices at IFOH. “This higher level of coordinated carenot only improves the patient experience, but is actuallyproven to result in better outcomes for patients.”Northern Virginia’s growing aging population meanscancer incidence will accelerate in the near future. ROOM TO GROWConstruction Begins on New Inova Women’s and Inova Children’s HospitalsFormer patients designed the high-risk perinatal unitto support women during long-term hospitalization forcomplicated pregnancies. The unit promotes familybonding and offers a soothing atmosphere to meetemotional, physical and spiritual needs. A separatewomen’s surgery center, staffed by female clinicians,enhances privacy and comfort. It streamlines the outpatientdischarge process, while conveniently consolidatingexisting programs and services for inpatients.Inova Children’s Hospital2014 marked a milestone for the future of women’s andchildren’s health. Construction began on the buildingthat will house the new Inova Women’s Hospital andInova Children’s Hospital on Inova Fairfax MedicalCampus. When complete in 2016, these state-of-the-artfacilities will provide critically needed services closeto home for patients and families throughout ourcommunity.Inova Women’s HospitalFeaturing 192 private rooms, a dedicated women’ssurgery center, 33 labor and delivery suites andsix cesarean section operating rooms, InovaWomen’s Hospital will be the mid-Atlantic’s premiercomprehensive health resource for women.To increase childbirth choices, couples can opt tohave a doula present during labor and delivery.Doulas are specially trained to support birthingmothers, and recent research shows they mayhelp lower the C-section rate. To encourage breastfeeding, the new hospital will provide a relaxed,welcoming environment with a full-service lactationcenter and inpatient and outpatient lactation consults.“We want to do all we can to enhance the birthingexperience and advance maternity care practicesthat protect, promote and support breastfeeding,”explained Patricia Schmehl, RN, Administrator,Inova Women’s Hospital.10I Advancing the Future of Health in Our CommunityDesigned with physician, nurse and parent input, InovaChildren’s Hospital will have 116 private pediatric rooms,five playrooms, two parent lounges and a RonaldMcDonald Family Room to complement the RonaldMcDonald House already on campus. T IETAL LETGHHOCEASR EH EORNE T H E F R O N T L I N E SMobile Health Services Program Keeps Loudoun Residents HealthyThroughout 2014, Inova Loudoun Hospital’s MobileHealth Bus was out and about in the community,providing health screenings, health education andpreventive services to more than 12,500 people.“The Mobile Health Services program began morethan 15 years ago to improve access to care for theunderserved,” said Stacey Miller, Executive Director,Inova Health Foundation. “Over time, it has grown tooffer a wide range of wellness services to residentsof greater Loudoun County. It’s part of our hospital’soutreach efforts to promote and create good health.”Staffed by RNs, the bus is a fully equipped,self-contained health clinic that offers: Blood pressure checks Blood sugar screenings Cholesterol screenings Body mass index (BMI) screenings Bone density tests Sun damage assessments Hearing and vision screeningsMobile Health Services delivers care directly to thecommunity at schools, churches, businesses, shoppingcenters, libraries and senior centers. The bus travels tofamily-centered events such as health fairs, communitygatherings and sports tournaments. FAMILIES TAKING CARE OF FAMILIESLocal Residents Rally to Meet Community Healthcare NeedsAt Inova Mount Vernon Hospital, 2014 was a watershedyear in its ongoing quest to expand resources to addressthe region’s growing healthcare needs. In December, thehospital officially dedicated the new Mark and BrendaMoore Patient Tower. The space is designed to improvethe patient experience while offering enhanced privacyand safety, in an aesthetic, healing environment. Itfeatures new, all-private rooms, a greatly expandedInova Joint Replacement Center and Inova RehabilitationCenter, a dedicated gymnasium, new cognitive suites,and other state-of-the-art patient care areas.Mark and Brenda Moore were motivated to give backafter Mark suffered a massive stroke in 2007. Just46 years old, he lost the use of his left side and wasunable to walk. Caregivers at Inova RehabilitationCenter guided his remarkable recovery and forgedlasting bonds with the couple. The Moores considerthe physicians, nurses and staff part of their extendedfamily and dedicated the new tower in their honor.Mount Vernon Hospital Emergency Department (ED).The upgrade is urgently needed to accommodatethe area’s growing population and rapidly increasingnumber of ED visits—estimated to be more than 41,000annually. INOVA CENTER FOR WELLNESS AND METABOLIC HEALTHA New Name, a Wider ReachDiabetes is an epidemic of increasing severity in theUnited States. The number of people diagnosed withdiabetes increased 176 percent between 1980 and2011, according to the Centers for Disease Control andPrevention (CDC).Effective self monitoring is key to successful diabetesmanagement, and Inova Diabetes Center was createdto help educate patients to do just that. But what aboutother diseases that share similar underlying metaboliccomponents, such as congestive heart failure, obesityand thyroid disorders? Those patients can also benefitfrom education, counseling and support as they workto take control of their health through nutrition, exerciseand medication.To address the needs of a wider population, InovaDiabetes Center became Inova Center for Wellnessand Metabolic Health in 2014. Along with the new name,the center consolidated a range of services in eachlocation, with the goal of becoming a “one-stop shop”for successful management of metabolic diseases.“In previous years, the center’s primary goal waspatient education,” said Marc Bailey, Director, InovaCenter for Wellness and Metabolic Health. “We havenow evolved into a more comprehensive medicalmanagement model, and we have worked to bring ahost of services for our population under one roof.”The multidisciplinary services now available at thecenter mean that patients no longer need to travel todifferent offices and locations for disease education,endocrinology, blood draws, nutrition counseling andprescription medications. By providing access to allof these services at outpatient facilities throughoutNorthern Virginia, Inova makes it easier for patientsto manage their diseases and live healthy lives.Education remains an important piece of the center’sofferings: the program is recognized by the AmericanDiabetes Association, and its staff of nurses andregistered dieticians are certified diabetes educatorswho empower patients with information and strategiesfor managing their disease. Personalized counselinghelps each patient establish health goals and an actionplan for achieving them. The center’s experts also offerclasses covering nutrition, medication, monitoring andmanaging setbacks.Continuing to expand the center’s services andcapabilities, a board-certified endocrinologist and anurse practitioner will join the team in 2015. In addition,medical nutrition services and counseling will beextended to patients with diabetes and other disordersas well.Inova designates funding for each location so that allcommunity members with metabolic disorders canaccess the center’s multidisciplinary services, whetheror not they are able to pay. In doing so, they helpthousands of people each year—the center’s locationshad 5,188 patient visits in 2014. INOVA TRANSITIONAL SERVICESBridging the Gap Between Hospital and HomeBeing discharged from the hospital should be causefor celebration, but oftentimes a patient’s eagernessto leave the hospital is tempered by uncertaintyabout how to manage medical conditions and toavoid ending up back in the hospital. When patientsare readmitted to the hospital within 30 days of beingdischarged for the same illness, they are at greaterrisk of poor health outcomes.To give patients the best chance of transitioninghome successfully, Inova developed the TransitionalServices department. Launched in 2011 and expandedsignificantly in 2013, the two-pronged program bridgesthe gap between the hospital encounter, whetherthrough the emergency department or inpatientadmission, and home.Inova Transitional Services offers patients key supportservices to help them navigate their medical needspost-hospitalization, regardless of their insurancestatus or ability to pay.The first part of the program, Transitional CareManagement (TCM), is a free, 30-day, telephone-basedcase management program for patients with high-riskdiagnoses such as chronic obstructive pulmonarydisease (COPD), diabetes, congestive heart failure(CHF) and pneumonia. TCM health coaches and casemanagers call patients once a week, beginning 24 to48 hours after hospital discharge. During these calls,staff members talk with patients, helping them tounderstand their disease diagnosis and learn whatsymptoms to watch out for as they recover. In addition,they complete a medication reconciliation and ensurepatients have a follow-up appointment with theirprimary care physician within three to five days.“In addition to providing disease education andmedication management counseling, the calls also14I Advancing the Future of Health in Our Communityhave a significant psychosocial component,” saidDana Green, MSW, LGSW, ACM, Director, InovaTransitional Services. “They are an opportunity toconduct depression screening and make sure patientsare connected to community resources relevant totheir specific condition.”The program’s second component is a network ofdischarge clinics located in Fairfax, Herndon andLeesburg. These clinics serve a broad range of patientswho need intensive post-hospitalization follow up forany of several reasons. Their condition may requireintensive management, or perhaps they can’t get anappointment with their primary care physician rightaway. The program also serves as a temporary medicalhome for patients who do not have established accessto primary care.In 2014, the discharge clinics served more than 2,000patients with 7,000 initial and follow-up encounterswith a physician, nurse practitioner, pharmacist or otherdischarge clinic healthcare providers. TCM staff servedmore than 5,000 patients in 2014.Inova Transitional Services has a significant impact onreducing 30-day hospital readmission levels, whichare 15 to 20 percent nationwide. In contrast, in 2014nine percent of TCM patients, and only six percent ofdischarge clinic patients, were back in the hospitalwithin 30 days.Whether in person or by phone, InovaTransitional Services helps set patients up for success,keeping them out of the hospital and helping them getback to their lives.For referral information or other program details,please contact Inova Transitional Services at571.623.3360.

INOVA KELLAR CENTEREmpowering Youth and Families Through Behavioral Health ServicesAlthough mental illness is not visible to the eye, it hasthe power to devastate a family—and it’s more commonthan you might think. According to the National Allianceon Mental Illness (NAMI), four million children andadolescents in the United States suffer with severemental illness, and one in five children between theages of nine and 17 have a diagnosable condition thatcould benefit from treatment.Yet it can be difficult to find help—lack of access toservices is a major risk factor for poor outcomes.Each year, only 20 percent of children who needmental health services receive them, according toNAMI.
