Signage must belegible from a minimum 40 feet away. 37 No employee shall ride on the sides, running board or tailgate of transport vehicle. Personnel Accountability During Emergencies . 39 Operation of any of the evacuations is a signal for all non-critical personnel to leave the unit orplant in an orderly and controlled manner. 39 Evacuation is to be opposite or at right angles to the prevailing wind direction (as determinedby observing windsocks or water vapor). . 39 Evacuate using “No Hardhat Required” route approved for entry/exit where possible. . 39

No running, evacuate in an orderly manner. . 40 Evacuation by vehicle is not permitted. All vehicles are to be pulled off the road, shutdown andabandoned. The operator should then evacuate on foot. If multiplecompanies are required to enter the excavation, each company will furnish a Competent Person tomonitor their respective personnel. . 61ii. For excavations outside the plant perimeter, or inside the plant perimeter where anothercompany’s pipeline crosses near the area, a notification to LA One Call, formerly known as DOTTIE(811 or 1-800-272-3020) must be made 48 hours prior to digging. Emergency situations do notrequire the 48-hour wait but LA One Call must be notified. Note: “Near” is defined by Westlake tobe within 100 feet.-.- (LA One Call Map – Non-Westlake Pipelines Within Plant - WESTLAKE Drawing# 32A-20181.1). 61iii. Determine if the area to be excavated is in one of the following area: South Terminal,Derivatives, North Dock area, Membrane, Sabine River Water Pond, Barge Slip, Coon Island Loop,Bayou Verdine, Plant A transformer areas, Isolated Canal, or WESTLAKE Canal. If so, contact theenvironmental department prior to excavating for special instructions. 62iv. Determine if the excavation is a Confined Space. If so, assure that all requirements are met inaccordance with the Safety Procedure #8, Confined Space Entry. 62v. All excavations will have barricades, guard rails, hand rails, warning lights, etc, as needed forprotection of personnel, vehicles or equipment working or passing adjacent to the excavation site.62

When the job is notcomplete, the person(s) working on the equipment will inform their supervisor and the job mustnot be signed as completed until the problem has been corrected. 74 Nesting is prohibited. . 74 When the authorized employee who applied the locks is not available to remove it, the lockmay be removed under the direction of the supervisor in charge of the job (job foreman, shiftsupervisor, field engineer). In doing so, the supervisor will: . 74a. Verify that the authorized employee who applied the lock is not in the plant. . 74b. Make a reasonable effort to contact and inform the authorized employee that the lock has beenremoved. . 74c. Ensure that supervision informs the employee of the removal before they return to the same job. 74d. Ifa fixed emergency safety shower or eyewash is not located near the operating area, a portableANSI approved safety shower/eye wash station must be set up near the line opening area butoutside of the barricade. 76 The process opening affected area shall be barricaded with red barricade tape and tags per4301-06-343 Plant Barricades, at a minimum of 10’ horizontal radius from the point of the openingfor process openings at grade. 77 For elevated process openings, the process opening affected area shall be barricaded with redbarricade tape at each level to include the grade level, which should be at a minimum of 15’horizontal radius. . 77 Shielding may be used when the use of barricade tape is impractical to protect nearbypersonnel, piping and equipment. . 77 Clearance verification shall be conducted by the equipment Owner/Operator and theMaintenance /Construction supervisor or General foreman to review the method and results of theclearing process. A determination will be made based on a visual or quantitative results whether ornot the process is “verified” clear or “unverified”. 77 Clearance verification will be agreed upon by the Equipment Owner/Operator andMaintenance/Construction Supervisor or General Foreman and a plan to safely complete the taskwill be formulated and documented on the General Work permit. 77NOTE* Minimum PPE for “Verified Clear” openings is normal PPE plus goggles and a face shield andthe appropriate gloves. When vapors may be present, respiratory protection shall be addressedbased on quantitative verification. Otherwise,a full face respirator with

Contractors assume the full responsibility and liability forthe safety of their agents, and employees and for the compliances of itssubcontractors. Any exception to the rules set forth herein must have priorwritten authorization from the Owner. The Owner is established as WestlakeCorporation.In addition to the rules set forth herein, Contractors working in or near an existingoperating facility must be knowledgeable of and must comply to the Owner'ssafety rules for the area and any federal state, or local laws governing.Should any procedure set forth herein conflict with applicable federal, state, orlocal laws, the latter will apply.Contractors unable or unwilling to secure personnel whose performance is inkeeping with these rules will not be acceptable. Contractor's supervisors andemployees who do not exhibit support in word and action for performing worksafely and complying with established safety procedures will be unacceptable.EMPLOYEES WHO LEAVE A JOB DUE TO SAFETY VIOLATIONS WILL NOTBE ALLOWED TO RETURN TO OWNER'S WORK SITE WITHOUT PRIORAUTHORIZATION FROM THE OWNER.Contractors and their employees are required as of February 15, 1993, to havesuccessfully completed an eight hour safety awareness and training program oran approved reciprocal program. This program is administered by the SafetyCouncil of Southwest Louisiana for this area.On the Westlake Chemical South LC Intranet webpage there is a complete list ofWestlake Chemical South Safety Procedures. If you do not gain access to the LCIntranet system, get with your Westlake representative to produce theseprocedures for you.II.Definitions"Owner", "Owner's Representative". "Contractor", "Contractor'sRepresentative", "Subcontractor", and "Work" shall be as defined in theGeneral Terms and Conditions and the Agreement."Construction Management Contractor" shall mean the contractor who hasbeen duly authorized to act as “Owner's Representative" in thoseresponsibilities so delegated by the Owner.
