Foothills Community Action Partnership has received a grant that helps provide housing forchronically homeless persons. Per HUD the definition for chronically homeless is as follows: A single person with a disabling condition who is living on the streets (place not meant forhuman habitation) or in a shelter. HUD defines “disabling condition” as “diagnosable substance use disorder, seriousmental illness, developmental disability, or chronic physical illness or disability,including the co-occurrence of two or more of these conditions. A disablingcondition limits an individual’s ability to work or perform one or more activities ofdaily living.” This person must have been continuously homeless for a year or more or has had at leastfour episodes of homelessness in the past three years.Open Concern Clothing Center229 E.
