If the hallmark ofRSNA’s first century was the ability todeliver ever more precise and meaningfulimages, the Society’s second century willbe defined by the development of innovative ways to manage those images and theassociated information in ways that mayultimately improve patient care.As radiology bridges these two eras,there are challenges – and not all of themare technical, according to Murray Reicher,M.D., chief medical officer at MergeHealthcare, an IBM company.“Radiologists are drowning in images,”Dr. Reicher said. “We have entered an erawhere 1,000 image CT, MRI, and PETexams are common. In academic environments, it is not at all uncommon to see3,000-, 4,000- and 5,000-image studies. Inaddition, radiologists now need to assimilatemore clinical data than ever and take intoaccount more scientific research in orderto optimally perform their jobs. Radiologists need to track recommendations aswell as communicate with other physiciansand patients. It’s a daunting task that hasbecome overwhelming.”“Let’s first look atjob satisfaction,” Dr.Reicher said. “Part ofenjoying work is getting into a mental stateof flow. Job satisfaction requires a sense ofmastery and creativecontrol over one’senvironment, as well Murray Reicher, M.D.as knowing that youare doing a diligent job and helping people.You can’t get into a mental state of flowand you can’t enjoy your work when presented with an insurmountable task.”There are ways to help resolve thisdilemma, and Merge Healthcare, an IBMcompany, will address these topics atRSNA 2015.“Cognitive computing holds greatpromise to help,” Dr. Reicher said, and heoutlined three applications of computertechnology Merge Healthcare believes mayhelp radiologists efficiently navigate theirworkloads.“First,” he said, “cognitive computingcan be used to collect, compile, analyze,and present clinical data.”In the future, he pointed out, prior toreading an exam, instead of inferring thepre-test probability of various diagnosesbased on a keyhole view of the patient, heenvisions that the radiologist will receivean accurate, concise presentation of existing diagnoses and be able to identifyoptions for treatment that consider theimaging studies in the context of otheravailable patient data, such as doctors’notes from a patient record or patient contributed data from wearable devices.“Second, image analytics can bedesigned to analyze the images to helpclinicians identify anomalies,” Dr.Reicher said.As an example, he described the dailyworkload of a hypothetical radiologist whomust read 50 CTs a day (“Which is not verymany,” he added.) If each of those CTs has1,000 images, and there’s an old CT forcomparison purposes, those 50 CTs translate to 100,000 images in a day’s work.“Rather than a human triaging an insurmountable number of images, perhaps technology can enable the radiologist to insteadfocus on uniquely human and rewardingtasks, such as evaluating relevant images,applying our experience to making diagnoses, directing further work-ups, consulting with referring doctors, supervisingtechnologists, participating in selection ofproper imaging procedures, and educatingpatients,” adds Dr. Reicher.Finally, Dr. Reicher suggested, “there’sa third, under-emphasized area of researchand development, “perceptual design.” Perceptual design is informed by the study ofhuman perception and cognition, and aimsto create a radiologist’s reading environmentthat maximally leverages our human capabilities while helping us with our weaknesses.All three applications of computer technologies may help – more efficient andcontinued on page 6bSo revealingMaking an accurate breast cancer diagnosis can be a complex process,and time is always of the essence.
