Only the Policyholdermay determine who is a Primary Cardholder,whether an Account is in Good Standing and2whether the insurance pursuant to this Certificatehas come into or is in force.This policy contains a provision removing orrestricting the right of the Insured Person todesignate persons to whom or for whose benefitinsurance money is to be payable. No person iseligible for coverage under more than one certificateof insurance providing insurance coverage similarto that provided hereunder. In the event that Werecord any person as an Insured Person under morethan one such certificate of insurance or policy, suchperson shall be deemed to be insured only underthe certificate of insurance or policy, which affordsthat person the greatest amount of insurancecoverage. In no event will a corporation, partnershipor business entity be eligible for this insurancecoverage. DefinitionsBenefitsBenefit MaximumsEmergency Medical andDental Coverage 2,000,000 perAccident/Accidental – a sudden, unexpected,unintended, unforeseeable external event,occurring during an insured Trip, arising wholly fromaccidental means which, independently of any othercause, causes Injury.IncludedAccidental Bodily Injury – a bodily Injury causedby an Accident of external origin occurring duringthe Coverage Period and being the direct andindependent cause of the loss.Insured PersonIncludedEmergency MedicalTransportation24-Hour Emergency TravelAssistanceInclusive Travel Medical Insurance providesinsurance for the Primary Cardholder and theirSpouse who are under age 75 on the date thePrimary Cardholder’s eligible Mastercard card isactivated or on Your annual renewal date, and foreligible Dependent Children. Coverage is active foran unlimited number of Trips taken immediatelyfollowing the date MBNA receives and approves thePrimary Cardholder’s request for the Mastercard card. Coverage under this Certificate is providedfor the first 15 days of each Trip. Your DepartureDate and Your Return Date are both counted andincluded as separate days when determining theTrip length. For Trips of a longer duration, Youmay be eligible to purchase an extension to Yourcoverage. This additional coverage may requirethe completion of a medical questionnaire. Take this Certificatewith You on Your Trip and the telephone4Account – the Primary Cardholder’s Mastercard account, which is in Good Standing with thePolicyholder.Certificate – the entire certificate of insurancedocument containing the terms and conditions ofthis insurance and issued to You by Us.Common Carrier – any land, air or waterconveyance for regular passenger service, which isfully licensed to carry passengers for compensationor hire and which undertakes to carry all personsindifferently as to who may apply for passage, solong as there is room and there is no legal excusefor refusal.Contamination – the contamination or poisoningof people by nuclear and/ or chemical and/or biological substances, which causes Illnessand/or death.Coverage Period – the time insurance is ineffect, as described in the various sections of thisCertificate.Covered Service – a service or supply, specifiedherein, for which We provide benefits underthis Certificate.5

Departure Date – the date on which Youdepart on Your Trip, (using the local time at YourCanadian address).Departure Point – the city within Canadafrom which You depart on Your Trip on YourDeparture Date.Dependent Children – unmarried persons who aredependent on the Primary Cardholder for supportand are: under 21 years of age; or under 26 years of age if a full-time student; or mentally or physically incapable of self supportand over 20 years of age.Emergency – an unforeseen event that occursduring the Coverage Period and makes it necessaryto be hospitalized or to receive immediate Treatmentfrom a licensed Physician.Emergency Dental Care/Treatment – the servicesor supplies provided by a licensed dentist, Hospitalor other licensed provider that are immediately andMedically Necessary.Emergency Medical Care/Treatment – theservices or supplies provided by a licensed Physician,Hospital or other licensed provider (physiotherapist,chiropractor, chiropodist or podiatrist) that areMedically Necessary to treat any Illness or othercovered condition that is acute (onset is suddenand unexpected) and which cannot be reasonablydelayed until You return to Your home countrywithout endangering Your health.Family Cardholder – a Primary Cardholder’s Spouseand/or Dependent Children who have been issued asupplemental Mastercard card.Family Member – includes the Insured Person’sSpouse; parents; children, including childrenwho are, or are in the process of becomingadopted; siblings; grandparents or grandchildren;step-parents; step-children; or step-siblings; inlaws (parent, son, daughter, brother, sister orgrandparent); aunts; uncles; nieces; nephews; legalguardians; or wards; whether or not they travelwith You.6Good Standing – being in full compliance with allof the provisions of the terms of operation, or otheragreement in force between the Primary Cardholderand the Policyholder, as amended from time to time.Government Health Insurance Plan – the healthinsurance coverage that Canadian provincial andterritorial governments provide for their residents.Hospital – an establishment that is licensed as ahospital and is operated for the care and Treatmentof Inpatients, has a registered nurse always on duty,and has a laboratory and an operating room on thepremises or in facilities controlled by the hospital.Hospital does not mean any establishment usedmainly as a clinic, extended or palliative care facility,rehabilitation facility, addiction treatment centre,convalescent, rest or nursing home, home for theaged, or health spa.Illness – a Sickness, infirmity or disease occurringduring the Coverage Period that requires EmergencyMedical Care and which did not occur prior to theDeparture Date.Injury – bodily injury occurring during the CoveragePeriod, resulting directly and independently of allother causes, from an Accident.Inpatient – a person treated as a registered bedpatient in a Hospital or other eligible facility and forwhom a room and board charge is made.Insured Person – the Primary Cardholder, his/herSpouse and eligible Dependent Children.Mastercard – a Choice Rewards World EliteMastercard , or a Select Mastercard credit card asissued by the Policyholder.Medical Condition – an Accidental Bodily Injury orSickness (or a condition related to that AccidentalBodily Injury or Sickness), including disease, acutepsychosis and complications of pregnancy occurringwithin the first 28 weeks of pregnancy.Medically Necessary – the services or suppliesprovided by a Hospital, Physician, dentist, orother licensed provider that are required toidentify or treat Your Illness or Injury and that Wedetermine are:7

consistent with the symptoms or diagnosis andTreatment of Your condition, Illness, ailmentor Injury; appropriate with regard to standards of goodmedical practice; not solely for the convenience of You, aPhysician or other provider; the most appropriate supply or level of servicethat can be safely provided to You.When applied to the care of an Inpatient, it furthermeans that Your medical symptoms or conditionrequires that the services cannot be safely providedto You as an Outpatient.Mountain Climbing – the ascent or descentof a mountain requiring the use of specializedequipment, including crampons, pick-axes,anchors, bolts, carabineers and lead or top-ropeanchoring equipment.Operations Centre – the contact centre maintainedby Global Excel Management Inc., Our administratorfor claims and assistance services.Outpatient – someone who receives a CoveredService while not an Inpatient.Physician – a person (other than an Insured Person)who is not related to the Insured Person by bloodor marriage who is licensed to prescribe drugs andadminister medical Treatment (within the scope ofsuch license) at the location where the Treatment isprovided. A physician does not include a naturopath,a herbalist or a homeopath.Policyholder – as defined on page 2 ofthis Certificate.Prepaid – paid prior to Your Departure Date.Prescription Drug – a drug or medicine thatcan only be issued upon the prescription of aPhysician or licensed dentist and is dispensed by alicensed pharmacist.Pre-Existing Condition – means aMedical Condition:8 for which symptoms appeared in thePre-Existing Condition Period; which was investigated, diagnosed or Treatedduring the Pre-Existing Condition Period; or for which further investigation wasrecommended or prescribed, or for whicha change in Treatment was recommended(including a change in medication or its dosage)during the Pre-Existing Condition Period.Pre-Existing Condition Period – is the period oftime (outlined below) that ends immediately beforethe Coverage Period. The Pre-Existing ConditionPeriod is: 180 days for Insured Persons who are 64 yearsof age or younger; 365 days for Insured Persons who are 65 yearsof age to 74 years of age; 365 days for Insured Persons who are 75 yearsof age or older.Primary Cardholder – the Cardholder who hassigned an application for a Mastercard card, asPrimary Cardholder, and for whom the Mastercard Account is established.Professional – a person who is engaged in aspecified activity as their main paid occupation.Reasonable and Customary Charge – a chargein an amount consistently made by other vendors/providers for a given service in the same geographicarea, which reflects the complexity of the servicetaking into account availability of experiencedpersonnel, availability of services or parts.Return Date – the date on which You arescheduled to return to Your Departure Point (usingthe local time at Your Canadian address).Sickness – any sudden Illness or disease requiringthe immediate medical care or Treatment ofa Physician.Spouse – the person who is legally married to or ina legal civil union with the Primary Cardholder; oris living with the Primary Cardholder in a conjugal9

relationship and is publicly represented as thePrimary Cardholder’s spouse, or domestic partner, inthe community in which You reside. You may onlyhave one spouse for the purposes of this insurance.Stable – any Medical Condition or related condition(including any heart condition or lung condition) forwhich all the following statements for that MedicalCondition or related condition (including any heartor any lung condition) are true: there has been no new Treatment or newprescribed medication; there has been no change in Treatment orchange in prescribed medication (including theamount of medication to be taken, how oftenit is taken, the type of medication or change inTreatment frequency or type); there has been no new symptom, more frequentsymptom or more severe symptom; there have been no test results showingdeterioration; there has been no hospitalization or referralto a specialist (made or recommended) andthe results of further investigations havebeen completed.Terrorism or Act of Terrorism – an act including,but not limited to, the use of force or violenceand/or the threat thereof, including hijacking orkidnapping, of an individual or group in order tointimidate or terrorize any government, group,association or the general public, for religious,political or ideological reasons or ends, and does notinclude any act of war (whether declared or not), actof foreign enemies or rebellion.Ticket – evidence of full fare paid for travel on aCommon Carrier.Travel Advisory – a formal written noticeissued by the Canadian government to advisetravellers not to enter a foreign country or a givenregion in that country. It does not include travelinformation reports.10Travelling Companion – any person whotravels with the Primary Cardholder for the entireTrip and whose fare for transportation and/oraccommodation was Prepaid at the same time asthe Primary Cardholder’s.Treated or Treatment – means any medical,therapeutic or diagnostic procedure prescribed,performed or recommended by a Physician,including but not limited to prescribed orunprescribed medication, investigative testing andsurgery. The term “treatment” does not include theunaltered use of prescribed medication for a MedicalCondition which is Stable.Trip – a period of round trip travel of definite lengthto a destination outside of the Insured Person’sprovince or territory of residence that is not for thepurpose of obtaining health care or Treatment ofany kind, which includes: travel by a Common Carrier; or a stay in a hotel or similar accommodation; or a package tour which has been sold as a unitand includes at least two of the following:– Common Carrier transportation;– car rental;– accommodation;– meals;– tickets or passes for sporting events or otherentertainment, exhibition or comparableevent; or– lessons or the services of a guide.We, Us and Our – as defined on page 2 of thisCertificate.You and Your – refers to the Insured Person.3. Eligibility for CoverageTo be eligible for this insurance coverage You must: be a resident of Canada; be covered under Your Government HealthInsurance Plan for the full duration of Your11

Coverage Period; be under age 75 at time of card activation oron Your annual renewal date; and not be employed outside of Canada on a full orpart-time basis. if You have purchased additional days ofcoverage from Us, Your coverage will end at12:01 a.m. on the day immediately followingthe final date of coverage as indicated on Yourdeclaration page; orPLEASE NOTE: You must meet all of the aboverequirements to be eligible for coverage. the annual renewal date of the year Youattain age 75 (for Dependent Children see thedefinition for age limits); or4. Certificate Effective Date andTermination Date at the time You return to Your province orterritory of residence.Except as otherwise stated herein, this Certificateshall come into effect on the date MBNA receivesand approves the Primary Cardholder’s request forthe Mastercard card.Except as otherwise stated herein, this Certificateshall terminate on the earliest of: the date the Insured Person is no longer eligibleto participate; the date the eligible Account is defined asineligible by the Policyholder; the date the Account is closed; the date the master group insurance policy isterminated; or the date of Your annual renewal when Youattain age 75.5. Travel Medical Benefits5.1 EMERGENCY MEDICAL AND DENTALCOVERAGEOptional Extension of CoverageCoverage may be extended provided no event hasoccurred that would give rise to a claim under thisinsurance and provided You request an extension byphone prior to the 15th day of Your Trip.To arrange for an extension of coverage, pleasecall the Operations Centre at 1-866-520-8827while in North America, or if elsewhere, call collect1-519-742-9356. The additional coverage mayrequire the completion of a medical questionnaire.Premium payment must be charged to YourMastercard card.When making a claim hereunder, evidence of YourDeparture Date from, scheduled and actual ReturnDates to Your province or territory of residence willbe required.Automatic ExtensionCoverage begins at the time of Your departure fromYour province or territory of residence.Coverage will be extended provided Your returnis delayed due to the hospitalization, duringthe Coverage Period, as an Inpatient or MedicalCondition of You, Your Spouse or Your DependentChildren or Your Travelling Companion, when theyare travelling with You.Your insurance under the Emergency Medical andDental Benefit ends on the earliest of:If coverage is automatically extended, coverage willend on the earliest of either:Coverage Period at 12:01 a.m. on the 16th day (including YourDeparture Date) after You depart on a Trip; or12 Your arrival at Your province or territory ofresidence or return destination based on Yourtravel itinerary; or13

5 days after Your scheduled Return Date;however, if You are hospitalized as an Inpatient,if Medically Necessary, We will extend insurancefor 72 hours from the time You are dischargedbut under no circumstances will coverage beextended for more than 3 months from Yourscheduled Return Date.Covered BenefitsEmergency Medical and Dental Coverage reimbursesYou for eligible expenses if You require EmergencyMedical and/or Dental Care during Your Trip. Thiscoverage will also cover expenses for Emergencymedical transportation back to Your province orterritory of normal residence. In the event of Injuryor Illness while on a Trip, during the CoveragePeriod, We reimburse You for Reasonable andCustomary Charges for the following MedicallyNecessary expenses required by You.Emergency Hospital, Ambulance andMedical ExpensesWe provide coverage up to 2,000,000 perInsured Person (unless as specified otherwise belowfor a specific benefit), for the following:Hospital Inpatient ChargeHospital Inpatient room and board charges, up tosemi-private ward or the equivalent. If MedicallyNecessary, expenses for Treatment in an intensivecare unit.Emergency Medical Treatment (includingx-rays and lab)This insurance covers Emergency Medical Careor Treatment of any Medical Condition that isacute (onset is sudden and unexpected) andconsidered life threatening or, if left unattended,could deteriorate resulting in serious andirreparable harm.Emergency Dental Treatment (includingx-rays and lab)14This insurance covers the following dentalexpenses when required as Emergency Treatmentand ordered by or received from a licensed dentist: if You need dental Treatment to repair or replaceYour natural or permanently attached artificialteeth because of an Accidental blow to Yourface, You are covered for the Emergency dentalexpenses You incur during Your Trip. You arealso covered up to a maximum of 1,000 tocontinue necessary Treatment after Your returnto Canada. However, this Treatment must becompleted within 3 months after the Accident. if You need Emergency Treatment to relievedental pain, You are covered for the Emergencydental expenses You incur during Your Trip, upto a maximum of 250, and the complete costof Prescription Drugs.Professional FeesThis insurance covers expenses for EmergencyTreatment by a licensed physiotherapist,chiropractor, chiropodist or podiatrist to amaximum of 350 per profession.Licensed Private Duty NurseThis insurance covers the cost of licensed privateduty nursing services to a maximum of 5,000while You are an Inpatient, when pre-approved bythe Operations Centre.Prescription DrugsThis insurance covers the cost of PrescriptionDrugs, limited to a supply of 30 days, if prescribedbecause of an Emergency condition.Medical AppliancesThis insurance covers the cost of medicalappliances including wheelchair, braces, crutches,walker, or Hospital-type beds, if ordered by alicensed Physician. We will pay the lesser of therental or the purchase price.15

AmbulanceThis insurance covers the cost of local groundtransport to a medical service provider inan Emergency.Medical Assistance ServicesMedical AssistanceIf You have a medical problem or Emergency, Youmust contact the Operations Centre, who willrefer You to a local Physician, dentist, Hospital,medical facility, or other appropriate resource.Medical Consultation and MonitoringIf You are hospitalized, Global Excel ManagementInc. medical staff will keep in contact with Youand Your treating Physician to get information onthe care You are receiving and determine the needfor further assistance. The Operations Centre willalso contact Your personal Physician and family athome, if necessary.Emergency Medical TransportationWe will arrange and pay for medicaltransportation services as specified below, whichare required by You as a result of an Injury orIllness that occurs during the Coverage Period thatrequires transportation to an appropriate medicalfacility or return to Your province or territory ofresidence. All Emergency medical transportationservices must be authorized in advance andorganized by the Operations Centre. Such servicesthat the Operations Centre does not pre-authorizeshall not be covered.Transportation to an AppropriateMedical FacilityIf Our consulting Physician and the local attendingPhysician determine that adequate Treatmentis not available locally and that Treatment isMedically Necessary, You will be transported tothe nearest appropriate medical facility.16Return to Your Province or Territoryof ResidenceOnce You have received Emergency Medical Careand Our consulting physician determines You areable to and recommends that You return home,We will arrange for You to return to Your provinceor territory of residence.We will arrange and pay for the following servicesand expenses to evacuate You to Your province orterritory of residence: the cost of an economy class one-way ticket ona commercial flight via the most cost-effectiveroute, less any refunds from any unused returntrip Tickets. If Medically Necessary or required bythe airline, We will also pay the expenses for aqualified medical attendant to accompany You. the cost of a stretcher fare on a commercialflight via the most cost-effective route to Yourprovince or territory of residence, if a stretcher isMedically Necessary. the cost of air ambulance transportation tothe most appropriate facility in Your provinceor territory of residence, if the use of an airambulance is required and Medically Necessary.Accommodation & MealsWe will pay up to 150 per day per Account,up to a total of 10 days to cover hotel expenses,meals and taxi fares, if You or Your TravellingCompanion because of receiving a coveredEmergency Treatment: are delayed beyond the initial scheduled ReturnDate; or have to relocate to receive theEmergency Treatment.Bedside VisitsIf You are travelling alone and will be hospitalizedas an Inpatient for more than 3 consecutive17

days, We will pay for the cost of a round-tripeconomy fare on a commercial flight via themost cost-effective route, to bring a FamilyMember or a close personal friend of Your choicewho is required at Your bedside while You arehospitalized. We will also pay up to 150 per dayper Account, up to a total of 10 days, for thatperson’s reasonable accommodation, taxi faresand meals.This benefit is subject to pre-authorization andmust be arranged by the Operations Centre.Repatriation of RemainsIn the event of Your death during Your Trip froma Medical Condition covered under this insurance,the insurance covers a maximum benefit of up to 5,000 for: the cost for reasonable and necessary servicesneeded for the transport of Your remains fromthe place of death to Your city of residence; or the burial or the cremation of Your remainswhere Your death occurred. The cost of a burialcoffin or urn is not a covered expense. the cost of a round-trip economy fare on acommercial flight via the most cost-effectiveroute for a person to identify Your remains, iflegally required. Meals and accommodations forthat person are covered up to a maximum of 150 per day per Account, up to a maximumof 3 days.Return of Dependent Children and Escort forDependent Children to their Departure PointIf Dependent Children insured under one of Ouremergency medical insurance products travelwith You or join You during Your Trip and You arehospitalized for more than 24 hours or You mustreturn to Canada because of Your EmergencyMedical Condition covered under this insurance,this insurance covers:18 the extra cost of a one-way economy air fare ona commercial flight via the most cost-effectiveroute for the return of those DependentChildren to their Departure Point; and the cost of a round-trip economy air fare via themost cost-effective route on a commercial flightfor an escort, if the airline requires that theDependent Children be escorted.Vehicle ReturnIf, as a result of a covered medical Emergency,You are unable to return Your vehicle or Yourrented vehicle to its point of origin, this insurancecovers the reasonable costs up to a maximum of 2,000 to return the vehicle to Your residence orto the rental agency, when pre-authorized by theOperations Centre.Emergency Medical and Dental coverageis also subject to the General Conditions,Limitations and Exclusions, as well as thePre-Existing Condition Exclusions.5.1.1 Conditions and Limitations Specific toEmergency Medical and Dental Coverage1. You must contact the Operations Centre beforeseeking care. If You do not notify the OperationsCentre or if You choose to receive Treatment froma service provider other than that recommendedby the Operations Centre, You may be responsiblefor 30% of Your medical expenses under thisinsurance. If Your Medical Condition prevents Youfrom calling before seeking Emergency Treatment,You must call as soon as medically possible. As analternative, someone else (Family Member, friend,Hospital or Physician’s office staff, etc.) may callon Your behalf.2. The medical staff of the Operations Centre mustapprove all cardiac procedures, including cardiaccatheterization, angioplasty and cardiovascularsurgery in advance.19

3. If Your employer (or former employer if You areretired) provides an extended health insuranceplan and:during Your Trip where the Operations Centredetermined and recommended You should returnhome and You chose not to do so; if Your lifetime maximum coverage is less than 50,000, We will not coordinate payment;8.
