What is CareLink Plus?We can help you:CareLink Plus is Brighton & Hove City Council’s communitytelecare alarm service. We can arrange urgent assistanceshould you need it, offering peace of mind to you, your familyand carers. We provide a range of services and equipmentknown as telecare to help support you at home, and alsosome telecare devices which can help support you whilst youare out and about.In addition, through our Living Well team we can also assist ourcustomers with practical help to improve health and wellbeing.Our Brighton based monitoring centre operates 24 hoursa day every day of the year. 1CareLink Plusprovides you withan alarm unit thatplugs into your telephoneline which has a loud speakerand a microphone, and anemergency button which youcan wear on a cord around yourneck or on a wrist strap.If you don’t have a landline telephoneconnection we can supply a unit thatuses the mobile network, subject tosufficient signal strength in your home.When activated, an alarm call isautomatically made to the CareLink Plusmonitoring centre without the need touse the telephone.2A range of telecare sensors canbe provided to help manage risksin your home and detect possibledangers, such as smoke or carbonmonoxide leak. Telecare sensors areavailable to detect if you fall duringthe day or night. These sensorsautomatically generate an alarmcall if they activate, without theneed for you to press your button.CareLink PlusHow doesit work?4Should you need help, wewill arrange the appropriateresponse. This could be tocontact a partner, family member orfriend, your carers, or a nominatedemergency contact. If necessarywe will arrange for a doctoror paramedics, the Police, orthe Fire Service to attend.Here for you24 hoursevery day3Your alarm call is received inthe CareLink Plus monitoringcentre where your details will bedisplayed on screen, so we will knowwho you are and important informationsuch as your medical conditions andhow to access your property. We will talkto you to find out what helpyou need. If we cannotmake contact with youwe will assume thatyou need help andtake action, rather thanassume everything is ok.

CareLink Plus offers a Standard andEnhanced telecare alarm package, anda GSM mobile network service.There is an initial four week free period available to supportcustomers who are being discharged from hospital, supportedby an informal unpaid carer or supported bythe Living Well team.The Standard Package includes: An assessment to establish the telecare equipmentthat would meet your needs and any requirementsthe Living Well service can assist with. An alarm unit and emergency button. Installation of CareLink Plus telecare alarmequipment. 24 hours, 7 days a week monitoring of alarm callsand response co-ordination. Service and maintenance of all telecare equipmentsupplied.Subject to the assessment this package can also include: Smoke, heat, carbon monoxide, and flood detectors linked tothe monitoring centre. Falls detecting pendant alarm button. Additional fixed alarm button in a location where falls arelikely, such as the shower, toilet, on the skirting beside the bed. A fixed button by the front door to alert in case of bogus callers. A fixed pull cord alarm trigger in a location such as beside thebed or in the bathroom. Automated reminder alerts or well-being check calls, thatraise an alarm call if you fail to respond. Additional equipment prescribed by Living Well. Support to access services to improve your well- being suchas befriending, meal delivery and social activities.The Enhanced Package includes everythingavailable in the standard package plus thefollowing extras subject to assessment: Bed and chair sensors that will activate in the eventof falls or absence. Pressure and movement detectors. Epileptic seizure sensors that will raise an alarm callwhen a seizure is detected. Detectors that will activate if doors are opened ornot closed. Systems to alert onsite carers when an alarm callhas been raised. GPS locator devices for use when you areout and about.The GSM Mobile Network Service is available where acustomer does not have a landline telephone connection,subject to sufficient signal strength in the property. A SIMcard is provided and there are no additional call charges foralarm calls made. This service includes everything from theStandard and Enhanced packages subject to assessment.

CareLink Plus must be ableto get help into your homewhenever you need it at anytime 24 hours a day sevendays a week. To do this weuse keyholders who could befriends, family, or neighbourswho we can call upon tocheck on your well-being,or to open the door for theemergency services.To have the CareLink Plus service you will need; A minimum of two people who live in the Brighton& Hove area or very close by, who will hold thekeys to your home, who we can contact to attendyour address. I f you have only one keyholder, or none at all, youwill need to agree to have a keysafe installed if youdo not already have one. A landline telephone connection where theCareLink Plus alarm unit is to be installed. I f you do not have a landline connection we canprovide a GSM alarm unit that uses the mobilenetwork, subject to adequate signal strength inyour home. A n available electric socket to power the CareLinkPlus alarm unit which must remain switched on atall times.If any of these are a problem please contact us todiscuss your options.How much does it cost?There is a service fee which is charged monthly. All telecareequipment supplied remains the property of CareLink Plus.Help with costs may be available following assessment.The Standard CareLink Plus Package costs 19.70 per month.The Enhanced CareLink Plus Package costs 23.60 per month.If a GSM alarm unit is required the package costs 25.50 per month.A keysafe can be provided and installed byCareLink Plus on a needs basis. The keysafe remainsthe property of CareLink Plus and will be removed whenthe service is no longer required. CareLink Plus supplyPolice approved keysafes which are fixed to the exteriorof your property and accessed via a unique code.There is an initial four week free period availableto support users who are being discharged fromhospital and those supported by an informal unpaidcarer. The Living Well team at CareLink PlusWe are a team of Adult Social Care Managers whospecialise in Telecare and assistive technology.We can help you to live more independently athome, prevent you going into hospital unnecessarily,support you when returning home from hospital,and link you back into your community.We will work with you to find solutions that feel rightfor you. We aim to do this by visiting you at homewithin two weeks of a referral, where we will speak withyou and your family or carers about what the challengesare for you at this time to living safely and well. We willassist you to navigate Health and Social Care Services,advising on accessing any financial support you mightbe eligible for, as well as the all the different voluntarysector support services.We can help to support you by: Reviewing your existing CarelinkPlus service and seeing if additionalTelecare or assistive technology willbe of benefit to you. Assessing for and providing dailyliving equipment such as chairraisers, rails or bathing aids. Assisting your unpaid carer to accesssupport services in their own right. G iving you advice if you need major adaptations toyour home and referring you to the right team to providespecialist assessment and provision of service. I f adaptations cannot help you to access the community oressential facilities in your home, such as the bathroom or kitchen,then we can help you to start the process of re-housing. A ssessing if you need formal care to help with your dailypersonal and domestic care routines, and organising a financialassessment to help you understand what contribution you mayhave to pay towards care. W orking in partnership with other organisations such as ThePolice, The Fire Service, Housing, Social Work and your GP, tomake sure you have the right level of support at the right time. Signposting or referring you to other services such asCommunity Meals, Befriending, Home Help, Advocacy,Money Advice, or finding activities in your local area. lease note we are not part of Emergency or ResponsivePServices but if we find you in a crisis, we will do whatwe can to help you get the support you need.
