WELCOME!The Idaho Division of Veterans Services and the Idaho Veterans Affairs Commission welcomeyou to the Idaho State Veterans Home-Boise. We hope you find your stay here comfortable andenjoyable.Specific rules and policies have been established by the Idaho Division of Veterans Services andthe Idaho State Veterans Home-Boise to operate the Idaho State Veterans Home-Boise in a mannerthat will provide a desirable living environment. Each resident is expected to comply with theserules and policies and conduct him or herself in a manner that is not offensive to others.This handbook is designed to help you understand the Idaho State Veterans Home-Boise. Thehandbook contains the policies and rules applicable to residents and describes the servicesavailable to residents. These descriptions are provided for the convenience of residents. In theevent of any inconsistency between the handbook and the rules or policies, the applicable rule orpolicy prevails over the descriptions in this handbook.You are invited to discuss concerns you may have about the Idaho State Veterans Home- Boiseand its operations with the Home Administrator or Home staff. We hope your stay with us is apleasant experience.Description of The Idaho State Veterans Home – BoiseThe "Old Soldiers Home" formally opened in Boise in May 1895. It was eventually replaced bythe New Home which was dedicated in November 1966.Situated on a sprawling, tree-lined campus at the base of the beautiful Boise foothills, the BoiseVeterans Home provides a quiet, peaceful setting just a few convenient miles from the heart ofdowntown Boise.The facility offers a 122-bed skilled nursing care unit including a 17-bed special care unit forveterans with Alzheimer's disease or related conditions and a 36-bed residential care/ domiciliaryunit. Each room is individualized to provide residents with maximum privacy, and all havewindows and individually controlled heating and air conditioning. The facility features skilled andrestorative therapy department, a spacious dining room, pleasant common areas, a large coveredpatio and courtyards. Services available to residents include extensive activity programs, onsiteveteran and social services, library services, and transportation to medical appointments.Mission StatementWe are dedicated to serving Idaho’s veterans and their families by providing superior advocacy,excellent assistance with benefits and education, high quality long-term care, and respectfulinterment services in a dignified final resting place.NURSING CARE HANDBOOK - 104/2019

Vision StatementThe Idaho State Veterans Home is “Caring for America’s Heroes.” The goal of the Idaho StateVeterans Home is to assist residents in attaining or maintaining their highest level of independencewithin the least restrictive environment. The Idaho State Veterans Home is dedicated totransitioning residents to independent living in the community whenever possible.Admission to Nursing CareNursing Care Eligibility RequirementsThe eligibility requirements for nursing care at the Idaho State Veterans Home are set forth inIdaho Code title 66, chapter 9 and the rules found at IDAPA 21.01.01. Attached hereto thisHandbook as IDAPA 21.01.01 - Rules Governing Admission, Residency, and MaintenanceCharges in Idaho State Veterans Homes and Division of Veterans Services AdministrativeProcedure, and by this reference incorporated herein.Applicants may be peacetime or wartime veterans and spouses of eligible veterans, with wartimeveterans retaining priority admission status. Applicants must have been discharged underhonourable conditions and be a bona fide resident of the state of Idaho. In addition, applicantsmust be unemployable as a result of age, illness, or disability. Effective July 1, 2000, applicantsmust apply for and become eligible for Medicaid benefits or must pay the established maximummonthly nursing care charge.The Idaho State Veterans Homes is a Medicare and Medicaid certified facility. The following is asummary of these eligibility requirements. To be eligible for residency in nursing care, applicantsmust meet the following criteria:1. Applicants may be peacetime or wartime veterans, and their spouses, with wartimeveterans receiving priority admission status. The total service time must be at least 90 days,with an honourable discharge. A copy of the Veteran’s discharge papers (DD-214) mustbe provided with the application. If the discharge papers are unavailable, a copy can berequested through the Department of Veterans Affairs. We will assist with the request.2. Applicants must be a resident of the State of Idaho at the time of application.3. Applicants must apply for (or already be in receipt of) a VA pension, and Aid andAttendance. We will assist with the pension application4. Applicants must either be eligible and apply for Medicaid benefits, or pay the currentmaximum monthly charge. Please include with the application a copy of the Veteran’sMedicare card, private insurance cards, Social Security card, and Medicaid card. Wewelcome applicants who may not be Medicaid approved yet, and we will assist them withthe application process.5. Spouses or widows of veterans (eligible for nursing care only) will need to provide proofof marriage when they apply.6. Veterans may be admitted without regard to their financial status.NURSING CARE HANDBOOK - 204/2019

Nursing Care Resident Room AssignmentsThe Idaho State Veterans Home assigns residents to rooms based upon considerations such as theextent of the resident’s disability, age of the resident, the resident’s likes and dislikes, the level ofcare received by the resident, and the availability of rooms. There are 122 nursing care beds onthree units. The majority of the rooms are semi-private and share toilet facilities with the adjoiningroom. Private rooms are located nearer the nursing station and are reserved for isolation andobservation purposes, or for the more critically ill. Each unit has one or two four-bed rooms witha bathroom. Residents are encouraged to decorate their room with personal items to make it ashomelike as possible. Storage space for clothing and other personal items is provided in eachroom. Permission to add or remove room furnishings must be granted by the Home Administratoror his/her designee. Families are encouraged to take excess personal items and larger items suchas suitcases with them as storage is limited. Upon discharge from the facility, family is asked toremove personal items within thirty (30) days from discharge. Because there is no storage spaceavailable, no items will be put into long-term storage.A room assignment need not be permanent, and every effort will be made to make thearrangements satisfactory. Residents may request room reassignments by contacting the SocialServices Department. The Idaho State Veterans Home will make room reassignments on a caseby-case basis.Available Long-Term Care Resources and Specialty Medical Care for Veterans in State VeteransHomesAttached hereto this Handbook as Summary of VA Long-Term Care Programs, FY2010, and bythis reference incorporated herein.Services Available To Nursing Care ResidentsMedical and Nursing ServicesAs a resident of the Idaho State Veterans Home skilled nursing units, residents’ general medicalcare will be provided, within the facility, by a duly credentialed physician on the Idaho StateVeterans Home’s contracted medical staff unless the resident notifies the facility he/she hasarranged for a different physician to supervise his/her care. Physicians are always on call foremergencies.Resident’s will be assigned one of the facility’s staff physicians at the time of admission. Ifresidents need to contact their physician, at any time, we recommend they inform the on-dutynurse. The on-duty nurses will assist in contacting the physician. For urgent medical needs, theunit nurse will automatically contact the resident’s physician. For other matters, the unit nurse willcontact the physician on behalf of the resident through our non-urgent communicationprocedures. If you wish to contact your physician by other means, please discuss the matter withthe physician at your next visit. 1- West Nursing Unit 780-16642- West Nursing Unit 780-16741- East Nursing Unit 780-1684Professionally licensed staff trained in geriatrics will provide twenty-four (24) hour nursing care.Each nursing unit always has a licensed nurse on duty, as well as a full complement of certifiednurse aides. Medications will be provided through our contract pharmacy. Physical, speech, andoccupational therapy services are available when ordered by the physician.Social ServicesSocial workers are available to assist residents and applicants with admission and dischargeplanning. They provide individual and family counseling and advocate for residents’ non-medicalproblems. During the admission process, the social worker will provide information to nursingstaff about the resident’s social history and family situation. This information will assist staff inrecognizing each resident as a unique individual. Social workers make daily rounds of the facility,providing support, assistance and counseling as needed.The Social Services Department is dedicated to assisting residents and their family members indealing with the multitude of details related to their stay. Each resident has a social worker toassist him or her to obtain and maintain the highest practical level of function. Being accessible topromote the highest level of communication is the key to quality care. Please do not hesitate tocontact the Idaho State Veterans Home social services staff.Religious ServicesThe Chaplain is available upon request to all residents and to residents’ families and visitors.Office hours are posted outside of the chapel. The Chaplain is also available whenever anemergency exists. In an emergency, the Idaho State Veterans Home nursing and social servicesstaff will assist residents by contacting the Chaplain. The Chaplain provides pastoral counseling,Bible study, and assistance with funeral arrangements. The Chaplain holds weekly Protestantservices while local congregations provide Catholic and LDS services. Residents are encouragedto attend any and all religious activities that are of interest. Please feel free to contact the Chaplainabout any concerns including arranging for specific church services.Veteran Service OfficersThe Office of Veterans Advocacy (OVA), a part of the Division of Veterans Services, providesassistance with problems or questions relating to veterans’ benefits. Their office is located at 444W. Fort Street in Boise, and they can be reached by telephone at 780-1380. Business hours are7:30 a.m. to 4:30 p.m., Monday through Friday.NURSING CARE HANDBOOK - 404/2019

For convenience, a Veteran Service Officer has regular office hours at the Idaho State VeteransHome. To make an appointment, please call 780-1398 and ask for the Service Officer. You mayalso contact a member from the social services or nursing staff to assist in scheduling this meeting.Food ServicesOur dining room offers an array of tasty and healthy foods. Menus are varied and change witheach season. While we encourage residents to eat in the dining room, meals can be brought to theunit if ordered by nursing staff. It is preferred that tableware remains in the dining area; however,residents may take fresh fruit with them to be eaten later. We do ask that residents store anyperishable food kept in their room in a sealed container.Dietary: Menus are posted throughout the facility and most special diets can beaccommodated. During the admission process, a Registered Dietitian or the DietaryServices Manager will visit with the resident. Along with resident’s personal preferencesand his/her physician’s orders, an individual diet that is both appetizing and healthful willbe developed.Pantry: Each nursing care unit has a pantry near the nursing station. This enables ournursing staff to serve snacks, beverages and sandwiches as needed for special dietary plans.Snacks: Snacks are available on a 24-hour basis. Please contact the nursing station if anutritious snack is desired.Family Meals: One guest may eat with residents once a month free of charge. There willbe a 2.00 per meal charge for any additional meals. Meal tickets may be purchased fromthe vending machine located near the elevators at the west side of the building. In all cases,residents or visitors should notify the dietary or nursing staff as early as possible of extrameals that are needed. This is especially important during the holiday season. Problemswith the meal ticket machine should be directed to the Business Office.Canteen: For the convenience of residents and staff, the Idaho Veterans Assistance Leagueoperates the Fort Boise Canteen. Volunteers from veterans’ organization or from thecommunity are welcome to assist by contacting the Idaho State Veterans Home VolunteerServices Department. The canteen is located across from the Business Office near the frontentrance. Confections, toiletries, and a variety of snack foods are available at reasonableprices. There are also vending machines in this area. We require that all beverages in thefacility, except those consumed in the canteen or the dining room, have a cover to preventspillage.Activities and RecreationRecreational and social activities are vital to residents’ social, physical and emotional well-being.By offering a variety of activities, the Idaho State Veterans Home encourages residents to continueassociating with groups, develop new interests or continue old ones, and maintain physical fitness.NURSING CARE HANDBOOK - 504/2019

To facilitate resident participation in activities, residents will be interviewed soon after admissionto determine their recreation and activity preferences. Group and individual activities are offeredon a daily basis and a schedule of activities and events is posted on each unit. Exercise and walkinggroups, birthday and holiday parties, fishing trips, musical events, field trips, barbecues, bingo,movies and out-of-facility meals are just a few of the activities available.Recreation Areas: Recreation areas are available for resident use. The recreation areasoffer cribbage boards, card tables, and an assortment of paperback books and popularmagazines for residents. Some recreation areas are furnished with comfortable sofas andchairs and are well lighted for reading.Reading Library: Daily newspapers, books and magazines are available in the library forresident enjoyment. Residents are encouraged to take only the book they are reading andreturn it when they are finished and before selecting another book.Video Library: A video library with over 1,200 movies is available for residents to use. Alist of titles and instructions is posted in the reading library. Nursing unit living roomshave a television and DVD and/or VHS players for watching movies.Arts and Crafts: The Activities Department has leather kits, ceramics supplies and otherarts and crafts items available for resident use.Televisions: Televisions are provided throughout the Idaho State Veterans Home forresident use.Computers: Computers are available for resident use and are located in the activity room.Residents must follow the Idaho State Veterans Home requirements for computer use. TheIdaho State Veterans Home may limit the times and length of use and the use of the Internet,where available.WIFI Internet Access: The Idaho State Veterans Home offers free wireless Internet accessthroughout the entire building for the convenience of residents, their families and visitors.For assistance, please contact the Social Services or Activities Department staff.TelephonesA telephone will be provided to each resident in their room. The Idaho State Veterans Home alsoprovides a cordless telephone on each nursing unit, at the nursing station. A resident may contactthe nursing staff or his/her social worker if assistance is needed in placing a telephone call. Inaddition to the resident phone, family members may contact the nursing station by using the mainswitchboard number, (208) 780-1600, at any time. Please limit telephone calls to ten minutes toprovide others with the use of the telephone.NURSING CARE HANDBOOK - 604/2019

Cable Television ServicesBasic cable television services will be provided to each resident in their room. If a resident wishesto purchase expanded cable television through Cable One, he/she will be responsible for the costof the upgrade as well as the month to month charge over and above the basic cable price. If aresident desires to change rooms, he/she will be responsible for any costs incurred in transferringthe expanded cable services. If nursing staff believes it is in the resident’s best interest to changerooms, the Idaho State Veterans Home may be responsible for those costs. Cable One must performall installations or other work on cable television.If residents wish to have expanded cable brought into their room, Social Services are available toassist residents in arranging services.MailAn outgoing US Post Office mailbox is located in the front lobby of the Idaho State VeteransHome. Residents may purchase stamps at the canteen. Incoming mail will be delivered toresidents’ rooms on a regular basis. Stationary supplies are available through the ActivitiesDepartment upon request.
