I was promoted toAssociate Member in 2010 and to Full Member in 2015. I am a physical therapist and clinical epidemiologist. Myresearch is focused on recognizing, describing, and remediating functional limitations in childhood cancer survivors. This includes determining the extent to which the disease and/or its treatment have left childhood cancer survivors withorgan system deficits that immediately or eventually interfere with their abilities to participate in normal movement, and thus restrict participation in school, work, leisure time, physical activity, and sport. My work uses epidemiological, clinical, and laboratory approaches to evaluate and describe physical performance, fitness, and physiologic reserve. This work has also helped me determine risk factors for and identify potential biological mechanisms responsible forthe impairments that result in, functional loss. Data from epidemiologic and clinical investigations have allowed me tounderstand the specific physical limitations and fitness deficits experienced by children during cancer therapy and bychildhood cancer survivors, to identify those most at risk for developing limitations, and to develop interventionsdesigned to prevent physical disability or restore physical health. My more recent work with computational biologyand laboratory investigators has allowed me to begin to document the biological processes potentially responsible forthe loss of physiologic reserve (frailty) in childhood cancer survivors. My Human Performance Laboratory offers methe opportunity to collaborate with other investigators who are also interested in physiological impairments andproblems with movement and physical function. I am a member of the Childhood Cancer Survivor Study (CCSS)Steering, Executive and Publications Committees, the Children’s Oncology Group Survivorship and Outcomes SteeringCommittee and the Oncology Section of the American Physical Therapy Association Research Committee. I serve onthe Editorial Boards of the Journal of Pediatric Blood and Cancer, Physical Therapy, Pediatric Physical Therapy andOncology Rehabilitation. I also served for four years as a member of the Clinical Management of Patients inCommunity-Based Settings Study Section for the National Institutes of Health (NIH), and have served as an ad hocmember of several Special Emphasis Panels for the NIH and other granting agencies. At St. Jude, I am the co-chair ofthe Global Working Group for the Cancer Prevention and Control Program, serve as the faculty representative to theFederal Demonstration Partnership, and am a member of the Clinical Trials Scientific Review Committee (CTSRC)and the CTSRC Behavioral and Psychosocial Sub-committee. I also teach the Introduction to Epidemiology course forthe Master of Clinical Investigation program at St. Jude. In the department of Epidemiology and Cancer Control, I

manage all data requests for the St. Jude Lifetime Cohort data and supervise the workflow of the analytic team. I alsomanage internal requests for CCSS and other data. In July of 2021, I assumed the role of MPI with Melissa Hudsonfor the St. Jude Life (SJLIFE) Cohort Study.4.Professional CareerPhysical TherapistHigh Plains Easter Seals Rehabilitation CenterSterling, COAug 1983 – Aug 1984Physical TherapistSt. Joseph’s Medical CenterBrainerd, MDAug 1984 – Aug 1986Physical TherapistNorthwest Rehab, Inc.Redwing, MNAug 1986 – Aug 1987Physical TherapistMinnesota Valley RehabilitationPrior Lake, MNAug 1987 – Jan 1992Physical TherapistCapernaum Pediatric Physical TherapyMinneapolis, MNJan 1992 – Jan 1993Physical TherapistMinneapolis Children’s Medical CenterMinneapolis, MNJan 1993 – Aug 1994Rehabilitation DirectorRehab Dynamics, Inc.Minneapolis, MNAug 1994 – June 1998InstructorPhysical Therapy ProgramUniversity of MinnesotaMinneapolis, MNJul 1997 – July 1998Assistant Professor and Academic Coordinator of Clinical EducationPhysical Therapy ProgramUniversity of MinnesotaMinneapolis, MNJul 1998 – June 2002Physical TherapistFairview University Medical CenterMinneapolis, MNSept 2000 – June 2006Physical TherapistAllina/Intrepid USAMinneapolis, MNMay 2002 – Aug 2002Research AssistantClinical Research/Epidemiology DivisionDepartment of PediatricsUniversity of Minnesota Medical SchoolMinneapolis, MNAug 2002 – Dec 2004Teaching AssistantDivision of EpidemiologyJan 2004 – Feb 20042
