GeneralPage 8 2021 ADRC Resource DirectoryLong Term Care ProgramsFamily careFamily care is a public program that offers individualsa wide variety of long-term care services, makingit possible for seniors and adults with physical anddevelopmental disabilities to live at home and stayindependent as long as possible.Family Care members receive support from a caremanagement team made up of a care manager andnurse. They perform a comprehensive assessment ofyour needs, abilities, preferences and values to developa personalized plan to meet your outcomes and goals.Members continue to choose their own physicians whileFamily Care provides the added long-term care supportyou need. Your team may make suggestions and assistIRIS (INCLUDE, RESPECT, I SELF-DIRECT)IRIS allows for self directed services and puts theindividual in charge of his/her own long-term care.An individual creates a plan outlining long-term caresupports and services with an assigned monthlybudget. Individuals are able to hire their own serviceworkers directly or may utilize an agency or communityprovider.
