TYLER COUNTY HOSPITAL SCHOOL OF VOCATIONAL NURSINGSTUDENT APPLICATION FOR ADMISSIONCLASS OF 2021(Complete in INK in own handwriting)APPLICATION:Accepted for consideration beginning April 1, 2020, and must be in the school offlce by 4:00 pmon July 24, 2020. Any applications received after that date (including by mai[) will not beconsidered. There are two options for submitting the application.Option #1Return the completed application, with the nonrefundable fee of 68 (oayable bycashier's check or money order only) for TEAS testing at the school, and a copy of yoursocial security care and driver's license.Option #2Return the completed application, plus results from a TEAS test taken within the pastyear, a 10 administration fee (payable by cashier's check or money order onlv), and acopy of your social security card and driver's license.Applications may be submitted in person to the school office or mailed to:Tyler County HospitalAttn: LVN School1100 W.
