Section 1 – IntroductionThis directory provides a list of Health Net Medicare Advantage plan providers. To getdetailed information about your health care coverage, please see your Evidence of Coverage(EOC).You will have to choose one of our network providers who are listed in this directory to be your PrimaryCare Provider (PCP). The term “PCP” will be used throughout this directory. Generally, you must get yourhealth care services from your PCP. As a member of Health Net Medicare Advantage, you will have a PCPwho coordinates all of your care. Your PCP is a health care professional who is trained to give you basic care.He or she is responsible for providing or coordinating covered services while you are a member of HealthNet Medicare Advantage.When you enroll in Health Net Medicare Advantage, you will select a Health Net Participating PhysicianGroup from our network. You’ll also choose a Primary Care Physician from this Health Net ParticipatingPhysician Group. This Health Net Medicare Advantage participating provider directory is a list of all HealthNet Participating Physician Groups (and their affiliated Primary Care Physicians and hospital affiliations).As a Health Net Medicare Advantage member, all of your routine health care is provided and arranged byyour PCP. Your specialty care, X-rays, laboratory tests, therapy, medications, hospital admissions, and followup care will also be provided, authorized or coordinated by your PCP. If you go to a doctor, hospital or otherprovider without the approval of your PCP – except in an emergency or when you need urgent care, out-ofarea renal dialysis, or certain gynecological care or other self-referred services – you will be responsible forpaying any charges for these services. Neither Medicare nor Health Net will pay for non-emergency servicesor non-urgently needed care without the prior authorization of your PCP.The network providers listed in this directory have agreed to provide you with your health care services. You maygo to any of our network providers listed in this directory; however, some services may require a referral. Otherproviders are available in our network. If you have been going to one network provider, you are not required tocontinue to go to that same provider. In some cases, you may get covered services from out-of-networkproviders. Even though your PCP is trained to handle most of your common health needs, there may be timeswhen he or she feels you need more specialized treatment. In that case, you may receive a referral (writtenpermission) to see a specialist. Specialists are doctors who provide health car services for a specific disease or partof the body. Examples include oncologists (care for cancer patients), cardiologists (care for the heart) andorthopedists (care for bones).Please refer to your Evidence of Coverage for further information.3
