Title:NOTICE OF EXAMINATIONExamination for Certificate of Fitness for Coordinator ofFire Safety & Alarm Systems in Homeless Shelters (F-80)Date of Exam: Written tests are administered Monday through Friday (except legalholidays) at 2:45 PM: by appointment only. Starting processing timefor a test is 2:45 PM. No test will be administered to applicants whoarrive after the 3:00 PM. To schedule an individual or groupappointment, please use this link S FOR WRITTEN EXAMApplicants must be at least 18 years of age and must have a reasonable understandingof the English language. Applicants must apply the exam in person and bring thefollowing materials and required fee:1. Applicant must provide two forms of identifications; at least one identificationmust be government issued photo identification, such as a State-issued Driver’sLicense or Non Driver’s License or a passport.2. Applicants must present a completed application for certificate of fitness ds/pdf/business/cof-application-form.pdf3. Applicants who are not currently employed may take the test without submittingthe F-80 Verification Letter or F-80 Application. If the applicants pass the test,FDNY will issue a temporary letter with a picture for job seeking purposes. The Cof F card will not be issued until the applicant is employed and submits both theF-80 Verification Letter and F-80 Application.4. Special requirements for F-80 test:1) A valid unexpired School Graduation DiplomaPrior to taking the FDNY computer based examination, all applicants shallsatisfactorily complete an approved course for Coordinator of Fire Safety &Alarm Systems in Homeless Shelters from a FDNY accredited school.The complete list of FDNY accredited schools can be found in the following Applicants must attend all training classes to be eligible to take the trainingcourse’s final examination. School Graduation Diploma will be issued after thecandidates obtain a passing score of 70% on the Graduation Test. Candidatesshall be allowed two (2) opportunities to pass the Graduation Test. Candidateswho fail the final examination on the second attempt shall be required to reattend the course in its entirety. The Graduation Diploma is valid for onlyone (1) year in order to take the FDNY computer based exam (F-80).2) Verification letterApplicants must present a verification letter from his/her employer , , shelterdirector or the Department of Homeless Services (DHS). The letter must benotarized. The sample verification letter is provided on the following page.4

3) F-80 application formApplicants must present the F-80 application form compelted by his/heremployer, shelter director or the Department of Homeless Services (DHS). Theform must be notarized. The F-80 application form is provided on the followingpage.4) Audit requirementsThe FDNY reserves the right to verify the contents of both the F-80 VerificationLetter and the F-80 Application immediately after submission by performing anunannounced audit of the contents of the letter. This audit will occur at theshelter location(s) in which the F-80 applicant is employed.5) Two homeless shelters onlyF-80Certificate of Fitness holders can only be certified in two homeless shelterlocations at one time without applying for a variance. You must apply for avariance with the FDNY requesting permission to work at three or morelocations. Both the F-80 Letter of Verification and the F-80 Application for eachindividual location are required.6) Exemption from S-95/F-53 Certificate of Fitness Individuals who are certified for the F-80 Certificate of Fitness will beexempt from having to obtain a separate S-95 or F-53 Certificate ofFitness for Supervision of Fire Alarm Systems and Other Related Systems.This exemption is for Coordinator of Fire Safety & Alarm Systems inHomeless Shelters who work in shelters only. The applicants who pass the F-53 Certificate of Fitness test are allowed toobtain the S-95 Certification of Fitness for another premisese without takingthe S-95 exam. However, employer recommendation letter (refer to the NOEof S-95) and payment ( 25) are still required.7) Current F-24 Certificate of Fitness holdersAfter July 31, 2014, the F-24 Certificate of Fitness will no longer be recognized.F-24 C of F holders must surrender their Certificate of Fitness for a premiserelated F-80 Certificate of Fitness for Coordinator of Fire Safety & Alarm Systemsin Homeless Shelters prior to July 31, 2014.5

It is a time-limit exam. Based on the amountof the questions, you will have 75 minutes to complete the test. To change a work location,Submit a letter from your current employer (on company letterhead) confirmingthat you are an employee and stating your new work location with a copy ofyour C of F and a 5.00 fee. Applicants must also submit an F-80 VerificationLetter and an F-80 Application completed by their current employer, shelterdirector or the Department of Homeless Services (DHS). Both must be onofficial letterhead and must be notarized. Please reference to the previouspages for sample documents.To request a replacement certificate:Submit a driver’s license or passport, social security number, mailing addressand a 5.00 fee.RENEWAL REQUIREMENTSThis Certificate of Fitness must be renewed every THREE YEARS. The renewalfee is 15. FDNY also reserves the right to require the applicants to take a reexamination within a particular timeframe. The FDNY may also require thatCoordinator of Fire Safety & Alarm Systems in Homeless Shelters completerefresher training at any time deemed appropriate by the Fire Department.You will receive a courtesy notice of renewal 90 days before the expiration date.However, it is your responsibility to renew your Certificate. It is very importantto renew your C of F before it expires. Renewals submitted 90 days (up to oneyear) after the expiration date will incur a 25 penalty in addition to therenewal fee. Certificates expired over one year past expiration date will not berenewed. New exams will be required.The certificate can be renewed On-line, by Mail or in Person. Renewal onlineIf you are an individual, make sure you have your 12 digit Certificate of FitnessAccess ID. This can be found on your Renewal Notice. If you do not have yourRenewal Notice, your Access ID is your 8 digit Certificate of Fitness numberand the last four digits of your social security number.
