In fiscal year 2017, we provided careto nearly 48,000 individuals and delivered 2,500 babies, through over 140,000 visits across our clinic“ Thanks to you,last year wasfull of excitingsteps forward.”sites in Lynwood, Downtown LA, and the San Fernando Valley. We took a pivotal step in our work toaddress childhood obesity through our FVRx program. We launched a new effort to equip expectingfathers with the knowledge, skills, and support needed to be caring partners and parents. An Apple a Day IsKeeping the Doctor AwayThanks to our partnership with Target and Wholesome Wave, Eisner Healthparticipated in the Fruits and Vegetables Prescription Program (“FVRx”) for itssecond consecutive year, prescribing daily fruits and vegetables to our patients.The program is simple. Participating Eisner Health doctors identify patients to enrollin the program — focusing on pediatric patients at risk of diet-related disease andfood insecurity. Once enrolled, patients meet with a doctor or case manager to sethealthy eating goals and receive vouchers for free fruits and vegetables. During thecourse of the six-month program, patients redeem the vouchers for fruits andvegetables for their family at Target stores or participating farmers markets.The outcomes of the FVRx program are real. At the end of the program, 93% ofparticipants were meeting fruit and vegetable recommendations,and the cups of fruit and vegetables consumed went up by128% from the start of the program.93%of participants metfruit and vegetablerecommendations bythe end of the program4Eisner pediatrician Dr. Yolanda Rebollo says, “When Ican offer a family a prescription for produce, it makessuch a difference. It’s going to go a long way to cuttingcosts for the family and for our healthcare system.”

25,000 BabiesDelivered in25 Years!This year we celebrated 25 years of delivering babies! Inthe past quarter-century, over 25,000 babies have beenbrought into the world under the care of Eisner Healthobstetricians and midwives.Our OBs and midwives serve our communities’ mostvulnerable moms, and are available round-the-clock,every day of the year at both California Hospital MedicalCenter (CHMC) in downtown Los Angeles and at MartinLuther King, Jr. Community Hospital in South Los Angeles.We also train OB and Family Medicine Residents fromUSC, UCLA, Harbor UCLA, CHMC, and Cedars-SinaiMedical Center.Our goal is to support our patients throughout everystep of their pregnancy. We ensure that our mothersare able to stay with their baby in one room from birthto discharge. We are proud to provide prenatal anddelivery care to high-risk OB patients, and receive manypatient referrals from partner community health centersthroughout Los Angeles County.5

STAFF SPOTLIGHTAna Rosas,Eisner Health Case ManagerAna Rosas connects patientswith community resourcessuch as health, economic,legal, or educational services.“Most patients just don’tknow what is available toAna Rosasthem. When they come to seeme, they aren’t going to leave not knowing.”Ana works with some of our most extremecases, including patients experiencing domesticviolence. “If I can help someone, I do,” she says.“I advocate for my patients. By asking questions,I’m able to really see how I can help them.”She recalls a young, undocumented man withmental health issues, named Marco. “The firstthing we had to do was get him covered.”After weeks of helping Marco with paperwork,scheduling appointments, and making severalphone calls on his behalf, Marco received healthcoverage. “Once he was covered, then I was ableto help him navigate the system, and get themental healthcare he needed.”6

Restoring Smiles,Hopes, and FuturesDr. Juliana Panchura has been the pillarof Eisner Health’s cleft lip and cleft palateprogram for almost 30 years. She hastreated thousands of patients, seeingmany of them grow into young adults.Individuals grappling with craniofacialDr. Juliana Panchuraabnormalities often have problems witheating and speaking clearly. Ear infections, hearing impairment,teeth problems, and social torment are just a few of the issuesthat these patients face.For patients with inadequate insurance coverage, existingtreatment locations are often over capacity, which results inchildren going untreated. For those who receive care atEisner Health, the results are life-altering.7

Crash Course For DadsWith funding from our friends at the GOOD Foundation and The Eisner Foundation, we launched anew program for expecting dads in October 2016. Daddy Boot Camp is a unique father-to-fatherworkshop that equips men of different economic levels and cultures to become confidentlyengaged with their infants, support their partners, and be great dads.Three-hour workshops are facilitated monthly in both English and Spanish. Men are able to interactwith a veteran dad and his baby, participate in an open discussion with their peers, and developnecessary skills. Expecting fathers watch veteran dads console, bathe, feed, and play with theirlittle ones. These new and returning dads-to-be learn how to nurture a child as they navigate theirtransition into fatherhood.8

Growing toMeet the NeedTo meet the growing need in the San Fernando Valley,during 2016 we brought our promise of care to PanoramaCity, Van Nuys, and Sherman Oaks. Collectively, ourcaring staff at these three clinics provide care to over7,000 patients annually.At our Panorama City and Van Nuys locations, we offerOB/GYN and pediatric services. In Sherman Oaks, we offerpediatric services and dental care. To ensure our patientsreceive the comprehensive care they deserve, supportiveservices are available, including case management,behavioral healthcare, and prenatal health education classes.At Eisner Health,we remaincommitted toadvocating forour patientsand promotingtheir right tohealthcare andhealth coverage.Advocacy inUncertain TimesAt Eisner Health, we remain committed to advocating for ourpatients and promoting their right to healthcare and healthcoverage. Last year we worked to engage our communityand our elected officials at the local, state, and federal levels.We support the advocacy efforts of our partners at theNational Association of Community Health Centers, theCalifornia Primary Care Association, and the CommunityClinic Association of Los Angeles County. In the comingyear, we will continue to elevate the voices of our patients,employees, community partners and leaders to reachelected officials, working to improve access to quality carefor our community.9

Helping ChildrenGet Their Best StartWe are so grateful for community partners who share our beliefthat every child should be positioned for success. Thanks to a grantfrom First 5 LA, we have been able to intervene early for childrenwith developmental delays, giving them the best possible chance ata happy and healthy life.Support from First 5 has helped many Eisner families, like Eva andher two year old son Ricardo. When Eva noticed that Ricardo wasunable to verbalize his communication, she approached oneof Eisner’s case managers, who administered a developmentalscreening. Ricardo’s results were abnormal, prompting a referralto speech therapy.Speech therapy transformed Eva and Ricardo’s communication.Their sessions incorporated sign language, pretend play, andhomework in the form of educational toys and age-appropriateactivities. Eva learned the tools and skills she needed to bettercommunicate with Ricardo.10

We are so grateful for community partnerswho share our belief that every child shouldbe positioned for success.11

Eisner Health Launchesa Volunteer ProgramIn 2017, we launched a volunteer program to offer more support to ourpatients and hardworking staff.Eisner volunteers help with a wide array of projects that assist our patients,such as helping with monthly baby showers for our pregnant moms, workingon craft projects and reading with our younger patients in the pediatricswaiting room, and aiding our annual Holiday Toy Drive.Our volunteer Ariel has helped our pediatric patients grow their imaginationsthrough a variety of activities and art projects. The arts are integral to a child’sdevelopment, and we are thrilled to provide opportunities for creativity toour patients.12
