They apply in addition to installationand construction charges made because unusual costs areincurred in the provision of service . Service charges may bewaived for a period of time when optional services are beingpromoted . Charges for services not specifically defined inthe tariff shall be based on a time and materials used basis .A . Categories of Service Charge sThe work functions required to establish, add to,move or change telephone service for a business or residenceclass of service customer are classified by type of servicecharge as follows :1 . Service Ordering Charge - work performed inconnection with receiving, recording, and processing acustomer request for service to be performed or provided atthe same time, on the same account and on the same premises .One Service Ordering Charge is applicable per access line orchannel .2 . Central Office Access Charge - work associatedwith the testing and connecting functions within a centraloffice required to establish or rearrange service .3 . Line Connection/Trip Charqe - work and travelassociated with installing or changing the drop wire oroutdoor circuit between the serving central office and thecustomer's premises on the Company's side of the demarcationpoint .4 . Station Handling Charge - work associated withconnecting, moving or changing terminal equipment on thecustomer's premises (applicable only to Semi-Public TelephoneService provided the instrument is Company-owned . )sY' k ., . Rules1.All registered terminal equipment and systems andinside wire may be directly connected to thetelecommunications network as specified in, or authorized by,the Registration Program in Part 68 of the FederalCommunications Commission's (FCC's) Regulations .2 . Customer Premises Inside Wir ea . The customer has the option of providing theinside wire and standard jacks (see Part C of this sectionfor a definition of inside wire) on his premises himself, orcontracting with a vendor qualified to provide the service .b . Customer premises inside wire, standardjacks, and equipment provided by the customer or his agentmust be in accordance with the standards of the NationalElectric Code, the National Electric Safety Codes, Part 68 ofthe FCC Regulations, the equipment manufacturers and otherapplicable codes . The customer will save the TelephoneCompany harmless from any and all liability, claims, ordamage suits arising out of the customer's provision ormaintenance of inside wire .3 . Charges specified in this section do not apply toservices furnished under concurrence provisions filed inSection 6 of this tariff . These services may include, butare not limited to, WATS access lines and access lineextensions, and all interexchange private line services andchannels and access services provided . Nonrecurring chargesfor these services are stated in other companies' tariffs, oras exceptions or additions to concurrent provisions inSection 6 of this tariff .4 . Normal maintenance and repair of theTelephone Company's equipment and service .b . Change or correction in name or billingaddress when there is not a change in responsibility .C . Definition s1 . Customer Premises Inside Wire - all wire within acustomer's premises, including connectors, jacks andmiscellaneous materials associated with the wire'sinstallation . Premises Inside Wire is located on thecustomer's side of the Telephone Company's demarcation point .By definition, Customer Premises Inside Wire excludes riser,buried and aerial cable .2 . Demarcation Point - the point of separationbetween customer provided and regulated Telephone Companyfacilities . The Network Interface is the normal demarcationpoint and is provided by the Telephone Company . For a simpleresidence or business, the Network Interface will be normallylocated on the exterior of the building . Requests to establish an account for initialconnection of service and subsequent requests for service,number and/or feature change restoration of service at thecustomer's request and change in class of service . Connection of additional local exchange accesslines, private lines, or detached access lines to anestablished service .3 .
