The Provider is the party responsible to You for the benefitsunder this Service Contract, except as noted in the State Requirement section located at the end of this Service Contract. You, Yourand Contract Holder refers to You, the purchaser of this ServiceContract and the owner of the Vehicle described in the RegistrationPage of this Service Contract.PLMADMINISTRATOR: refers to Integrity Vehicle Group, Inc. The Administrator is responsible for administering this Service Contract. Allinquiries should be directed to the Administrator. Toll-free assistanceis available at 1-877-252-8036.EBREAKDOWN, MECHANICAL BREAKDOWN, MECHANICALFAILURE: Refers to a failure due to defects in materials and/orworkmanship of a Covered Part to perform the function for which itwas designed by its manufacturer. A Breakdown does not includefailure due to sludging or gelling conditions, or normal wear and tear.Further, a Breakdown does not include any failures to Your Vehicle ifthe manufacturer has announced its financial responsibility throughany means including public recalls and factory service bulletins orTSBs.CONTRACT ACTIVATION DATE: The date the Validation Periodbegins.CONTRACT RENEWAL DATE: The date the Contract RenewalPayment is due and the Service Contract renews directly followingthe previous thirty (30) day period.

CONTRACT RENEWAL PAYMENT: The amount due in order torenew the Service Contract every thirty (30) days.CONTRACT RENEWAL TERM: The thirty (30) day period measured from the Contract Renewal Date provided the Contract Renewal Payment has been received. The Validation Period does notapply to any Contract Renewal Term.COVERED PART or COVERED PARTS: Refers to the parts orcomponents listed under the section entitled “Plan Coverage.”SADEDUCTIBLE: The minimum portion of the covered repair whichYou will have to pay if You have a claim. The amount of Your Deductible is shown on Your Registration Page. This amount is applied perclaim, and to each claim.PLMEXPIRATION TYPE: This Service Contract is subject to a Plan Period and Expiration Type. The Plan Period is the number of monthsand number of miles for which You are afforded coverage under thisService Contract. The Expiration Type determines when the numberof miles is reached. Please refer to the section entitled “ExpirationType” on Your Registration Page for more information.FULL FACTORY WARRANTY, FACTORY WARRANTY: Refers tothe full Manufacturer’s Warranty provided to You at no additional cost,and covers repairs to Your Vehicle to correct any defect in material orworkmanship. This Service Contract is not a Factory Warranty.ELABOR: Total labor time for a covered repair will be determined bya current nationally published labor manual. The labor rate for authorized repairs will be based on the posted labor rate of the licensedrepair facility that You selected. If the repair facility’s labor rate isnot posted the Administrator reserves the right to approve a laborrate based on the average labor rate for similar local repair facilities.Administrator also reserves the right to adjust the approved laborrate if the repair facility’s labor rate is deemed to be excessive by theAdministrator when compared to local average labor rates for similarfacilities.

OEM: Original Equipment Manufacturer.PLAN PERIOD: This Service Contract is subject to a Plan Periodand Expiration Type. The Plan Period is the number of months andnumber of miles for which You are afforded coverage under this Service Contract. The Expiration Type determines when the numberof miles is reached. Please refer to the section entitled “ExpirationType” on Your Registration Page for more information.SELLER: Refers to the party who sold You this Service Contract.Please see the box labeled “Seller Name” on the Registration Pagefor Your Seller’s contact information.SASERVICE CONTRACT: This Service Contract is issued to You andcovers Your vehicle described on the Registration Page of this Service Contract.PLMSERVICE CONTRACT NUMBER: Please see the box labeled “Service Contract Number” on the Registration Page. Please refer to thisnumber in any written or verbal communication, such as requestinginformation or filing a claim.VEHICLE, YOUR VEHICLE: Refers to the vehicle described on theRegistration Page of Your Service Contract and owned and registered by You.TERMS AND CONDITIONSEThis Service Contract provides coverages recorded for the time andmileage stated on the Registration Page, whichever occurs first.Please refer to the sections of the Registration Page entitled “Expiration Type” and “Plan Period” to determine Your period of coverage.CONTRACT HOLDER’S RESPONSIBILITIES:1. CLAIM REIMBURSEMENTObtain approval PRIOR to having work performed thatmay be covered by this Service Contract. If You believe

the failure may be covered by this Service Contract, callthe Administrator at 1-877-252-8036, or instruct the repairfacility performing the work to call to register the claimBEFORE THE WORK IS PERFORMED.See the section entitled “If Your Vehicle Incurs A Breakdown” for additional information.SA2. VEHICLE MAINTENANCE AND MAINTENANCE REQUIREMENTSProperly Maintain Your Vehicle and KEEP THE RECEIPTS.This Service Contract is only valid if Your Vehicle hasbeen maintained in accordance with the manufacturer’sspecifications. Keep copies of all receipts (oil changes,lubrication, etc.) including the mileage interval it was performed at, . Proof of maintenance may be required whenYou file a claim.EPLMMaintenance Requirements:a. You must have Your Vehicle checked and servicedin accordance with the manufacturer’s recommendations, as outlined in the Owner’s Manual for YourVehicle.NOTE: Your Vehicle’s Owner’s Manual lists different servicing recommendations based on individual driving habits and climate conditions. You arerequired to follow the maintenance schedule thatapplies to Your specific conditions. Failure to follow the manufacturer’s recommendations that apply to Your specific conditions may result in a denial of Coverage under this Service Contract.b. It is required that You retain “Proof” of maintenance for the service and/or repair work performedon Your Vehicle, including maintenance performedprior to the purchase of this Service Contract.“Proof” means repair orders from a licensed repairfacility. Pertinent information must be included thatidentifies the Vehicle and the repairs performed, in

particular the Vehicle Identification Number (VIN),date, mileage, parts and labor.ADMINISTRATOR’S RESPONSIBILITIES:SA1. BREAKDOWN OF COVERED PARTSWe will pay or reimburse You for the reasonable costs torepair or replace any Breakdown of a part listed in the PlanCoverage Section, as determined by the Administrator usingstandard and common industry practices. COVERED PARTSMAY BE REPLACED, DEPENDING ON AVAILABILITY ANDAT ADMINISTRATOR’S DISCRETION, WITH LIKE, KINDAND QUALITY (LKQ), USED, REBUILT, REMANUFACTURED OR NEW PARTS.GENERAL PROVISIONS:PLM1. YOUR HELP AND COOPERATIONIf We ask, You agree to help Us enforce Your rights againstany manufacturer or repair facility who may be responsible toYou for the cost of repairs covered by this Service Contract.You must provide written authorization to Us to communicatewith any party other than You. We will not pay for a Breakdown if You impair these rights.E2. SUBROGATION AND OUR RIGHT TO RECOVER PAYMENTIf We pay for coverage under this Service Contract, We mayrequire You to assign Us Your rights of recovery against others. We will not pay for a Breakdown if You impair these rightsto recovery. Your rights to recover from others may not bewaived. You shall do whatever is necessary to enable Us toenforce these rights. We shall recover only the excess afterYou are fully compensated for Your loss.In the event a repair is subject to any additional third-party Service Contracts or warranties, those Service Contracts or warranties shall supersede any and all obligations under this Service Contract.

3. DEDUCTIBLEIn the event of a Breakdown covered by this Service Contract, You may be required to pay a Deductible. No Deductible payment is required with respect to 24 Hour RoadsideAssistance, Rental, and Trip Interruption if they are providedby this Service Contract. The Deductible amount will be applied on a per repair visit basis. Should a covered Breakdowntake more than one visit to repair, only one Deductible willapply for that Breakdown.SA4. COVERAGEThe Coverage afforded You for Your Vehicle is determined bythe Plan Name and Expiration Type shown on the Registration Page, which is more fully described in the section entitled“Plan Coverage.”PLM5. LIMITS OF LIABILITYOur liability for any one authorized repair shall in no eventexceed the lesser of fifteen thousand dollars ( 15,000.00) orthe actual cash value (ACV) of Your Vehicle at the time of saidrepair visit as listed in the National Automobile Dealers Association (N.A.D.A.) Official Used Car Guide for Your region(excluding tax, title, and license fees).EOur liability for all authorized repairs combined, shallin no event exceed the lesser of fifteen thousand dollars( 15,000.00) or the actual cash value (ACV) of Your Vehicleat the time of said repair visit as listed in the National Automobile Dealers Association (N.A.D.A.) Official Used Car Guidefor Your region (excluding tax, title, and license fees).In the event that the amount of any one authorized repairor the combined total amount of all authorized repairsmeets or exceeds Our liability, Your Service Contract will bedeemed expired and no further coverage will be afforded toYou, regardless of the remaining time or mileage of Your Service Contract’s Plan Period. No refund shall be due to Youupon expiration of the Service Contract.

6. SERVICE CONTRACT VALIDATION PERIODThis Service Contract may be subject to a validation period oftime and mileage from the Service Contract Purchase Date(SCPD), as shown on the Registration Page. The length ofthe validation period, if applicable, is listed on the RegistrationPage of this Contract. There is no coverage during the validation period. Coverage will commence upon the expirationof the validation period. If the Validation Period applies, theadditional time and mileage contained in the validation periodwill be added to the plan’s duration.PLMSA7. EXPIRATION TYPE: ADD-ONThe plan expires by time or mileage, whichever occurs first.a. Time: The plan expiration is measured from the Service Contract Purchase Date.b. Mileage: The plan expiration is measured from theodometer mileage of the Vehicle on the Service Contract Purchase Date.8. MANUFACTURER’S WARRANTY DISCLOSUREIf the term of this Service Contract overlaps with the term ofYour Manufacturer’s Warranty, look first to Your Manufacturer’s warranty for coverage. This Service Contract excludescoverage for any loss covered by Your Manufacturer’s Warranty, but may nevertheless provide benefits in addition tothose provided by Your Manufacturer’s Warranty.EPlan CoveragePLAN-SPECIFIC COVERAGES (only those components specifically listed are covered, and coverage forcomponents in multiple component groups only applies for thecomponent groups in which the part is specifically listed). Coverage only applies to the specific plan purchased.

BRONZE PLAN COVERAGEThe following component groups are covered: 1 – 51. Engine: The following are covered if damage is caused bythe mechanical failure of an internally lubricated part: Cylinder Block, Cylinder Head(s), and Water Pump. All internalcomponents of engine block and cylinder heads, including butnot limited to Camshaft, Crankshaft, Pistons, Bearings. Timing chain cover and oil pan are covered only if damaged bythe failure of an internally lubricated engine componentSAPLM2. Automatic or Standard Transmission and Transfer Case:The internal components of the Automatic Transmission orManual Transmission. Drive Chain, Drive Chain Gears, Carrier Bearings, Internal Transaxle Seal. The Manual Transmission Case and Automatic Transmission Case and TorqueConverter are covered, if damaged by the failure of internally lubricated parts. Transfer Case, Drive Chain, Drive ChainGears, Planetary Gears, Shift Rail Forks, Bearings, Bushings,Oil Pump, Output Shaft, Main Shaft, and all internal lubricatedparts.E3. Differential Assembly: (Front and Rear) Differential Housing,Axle Shaft, Ring and Pinion, Bearings, Bushings, and Washers. Differential Cover and all other internal parts containedwithin the differential assembly.4. Drive Axle: (Front and Rear) Drive Axle, Constant VelocityJoints, Center Support Bearings, Drive Shaft and UniversalJoints. (except when damaged as a result of a torn or missingCV boot)5. Seals and Gaskets: Seals and Gaskets are covered only forthose parts listed in component groups 1-5, provided that theyare required in connection with the repair of a covered part.Note: Seepage of seals and gaskets is considered wear andtear and is not covered under this Service Contract.

SILVER PLAN COVERAGEThe following component groups are covered. 1 – 116. Additional Transmission: Vacuum modulator, Accumulator,and the Electronic Shift Control Unit. In addition, all internalcomponents of the transmission that require lubrication foroperation are covered. The transmission case and pan arecovered only if damaged by the failure of an internally lubricated transmission component.PLMSA7. Additional Drive Axle (Front & Rear): The following components are covered: Locking hubs, drive shafts, center support bearings, universal joints, and the CV joints (except whendamaged as a result of a torn or missing CV boot). In addition, all internally lubricated components contained within thedrive axle housing are covered. The drive axle housing anddifferential cover are covered only if damaged by the failure ofan internally lubricated drive axle component.8. Additional Transfer Case: All internal components of thetransfer case that require lubrication for operation are covered.E9. Turbo / Supercharger: All internally lubricated parts of theturbocharger or supercharger are covered, provided the mandatory surcharge has been paid. Coverage applies to factoryinstalled units only.10. Air Conditioning System: The following components arecovered: Compressor, condenser, evaporator, expansionvalve, blower motor, accumulator/receiver-dryer and the orifice tube.11. Electrical: The following components are covered: Alternator, voltage regulator, starter motor, starter solenoid, ignitionswitch, front and rear wiper motors
