Program OverviewOur department prides itself on being supportive of its fellows and places strong emphasis on the qualityof teaching on a person-to-person basis. Please see our y didactic morning conferences are held for the residents. The fellows are welcomed to attend theseconferences as their schedule permits. Department faculty, residents and fellows provide most didacticlectures, with frequent lectures from outside HMS faculty, past residents, as well as guest lecturers fromaround the world. Visiting professors from other major teaching centers in the United States and abroadspend varying periods of time in the Department. Some have taken sabbatical leave at the Beth IsraelDeaconess Medical Center.Medical CenterRadiology also has a close working relationship with a number of clinical services outside ourdepartment, which allows Radiology to participate in a number of management conferences, includingMedical Management, Melanoma Management and Urology Management, as well as Chest, Pancreaticobiliary, and Liver conferences; Medical Grand Rounds, Surgical Grand Rounds, and others.BostonThere are many great medical conferences sponsored not only by Harvard-affiliated programs, but alsoby a number of other medical centers. OVERVIEWbegun a monthly multidisciplinary ‘classics’ journalclub with pathology and surgery where wereview key articles in the breast literature. Fellowsparticipate in summer lecture series for fellowsand are encouraged to attend resident breastlectures throughout the year. Finally, our fellowswill also participate in the Harvard Breast ImagingFellows Meeting, where Harvard breast imagingfellows meet to discuss a topic and review cases.The breast imaging fellowship at Beth IsraelDeaconess Medical Center is a one-year fellowshipwith extensive experience in clinical care, researchand teaching. In addition, there is full exposureto QC/QA processes and issues related to themanagement of a breast imaging center.Upon completion of this fellowship, graduatesshould be competent to pursue an academiccareer in breast imaging or a career in privatepractice as subspecialty consultants in breastimaging.Fellows will have some responsibility forteaching medical students and residents howto interpret breast studies and how to managediagnostic work-ups. Breast fellows will sharethese responsibilities with the senior residents onservice. Academic time will be given for researchprojects. Each fellow will work with a facultymentor to design a research project(s), obtain IRBapproval, collect and analyze data and preparemanuscripts for presentation and/or publication.Elective time will be allotted based on the needsof each fellow. Six practice sites allow exposure toMQSA and ACR inspections throughout the yearand our QA program provides opportunities forfocused learning around difficult/complex casesand system improvements for patient care.The breast fellowship program at BIDMC is basedat our main Boston outpatient campus where over10,000 screening mammograms, 8,500 diagnosticmammograms, 4,500 ultrasounds and 1,000 MRIsare performed each year. Cases are referred fromfive of our off-site campuses that perform over25,000 additional examinations each year. Theprogram integrates diagnostic mammographywith ultrasound and MRI for the work-up ofbreast abnormalities. The breast fellows interpretdiagnostic mammograms with accompanyingbreast ultrasounds and breast MRI as needed. Wehave an active interventional service with over 40breast procedures performed per week.Fellows have the responsibility of performingchallenging wire localizations, US and stereotacticguided core biopsies as well as MRI biopsieseach week. Breast MRI is fully integrated into thediagnostic workflow with 20-25 cases per week.Screening services include mammography, MRIand whole breast ultrasound. Fellows spend theequivalent of 4 weeks interpreting screeningmammograms. In addition, fellows rotate throughbreast pathology, radiation therapy, nuclearmedicine and the BreastCare Center to get acomplete overview of what is involved in caringfor and managing patients with breast issues.Fellows will participate in multiple conferencesincluding a weekly Breast Radiology PathologyCorrelation Conference, a weekly MultidisciplinaryBreast Cancer Conference and Tumor Board, amonthly/quarterly Journal Club, monthly QA/Interesting Case Conference and monthly BreastImaging Research Meeting. We have recentlyFacilities5 digital mammogram units including 2 withtomosynthesis4 state-of-the-art US units3.0 T and 1.5 T MRI unit with biopsy capabilitiesStereotactic core biopsy table 3D upright3 9-gauge vacuum-assisted core biopsy devices4

PROGRAM GOALSConferences and TeachingThe fellow will participate in multiple conferenceslisted below and will have some responsibilityfor teaching medical students and residents howto interpret breast studies and how to managediagnostic work-ups. In addition, the fellow willprepare 3-4 other conferences incuding morningresident conference, resident board review and/orradiology grand rounds.1. To provide an organized, comprehensiveand highly supervised educationalexperience in breast imaging so that thefellow can function as an independentbreast imaging consultant and practitionerin academic or private practice setting2. To train fellows to screen healthypopulations for breast cancer includingscreening mammograms, whole breastultrasound and high-risk screening MRI3. To train fellows in the selection,performance and interpretation of:a. diagnostic breast imaging studies inall modalitiesb. interventional procedures along withan understanding of radiologic-pathologic review for concordance ofpathology findings4. To provide sufficient experience to advisereferring clinicians on the most expeditiousand efficient imaging techniques to answerclinical and imaging breast problems.5. To encourage and provide opportunitiesand skills for research in the field of breastimaging6. Conference time that is beyond 1week will come from this time if you are presenting anabstract/exhibit from the section, or have the need ofadditional academic time for manuscript preparation.**Fellows will get 1 full day of academic time during thefirst quarter to develop a research plan and submit springconference abstracts. After that, each fellow will get 1/2day academic time unless an approved research proposaljustifies additional time up to 1 day per week.5

SPECIFICS OF THE BREAST FELLOWSHIP PROGRAM1) VOLUMEScreening Mammography750-1000Diagnostic Mammography400-600Ultrasound300-400Mammo LocStereo Bx5) PROCEDURESIt is important that the fellow keeps track ofprocedures that he/she performs. The fellow isexpected to preview all procedures that she/he will be doing 1-2 days ahead of time. Thisincludes confirming modality of procedure,making sure to clearly identify the targetlesion, and ensuring all imaging neededfor the procedure is available. The fellow isresponsible for following up on all pathologyresults and any complications.30-4050-100US Procedures100-200Breast MR150-200MR Interventionby the end of the year. In addition, this is agreat opportunity to improve your own USscanning techniques.20-40These volume goals for our fellows are basedon prior fellow rotations through the breastimaging section. We hope that you will striveto complete as many cases as possible toenhance your learning.6) BREAST MRThe fellow will protocol and monitor all BreastMR Studies performed weekly. They willinterpret and dictate all completed Breast MRStudies under the supervision of an AttendingRadiologist in a timely fashion. They willparticipate in all scheduled breast MRBiopsies, obtain pathologic results for thesecases, and issue an addendum regardingconcordance/discordance with appropriatefollow up recommendations for theseprocedures.2) WORKING WITH THE TECHNOLOGISTSFellows will each spend 1 week workingwith the ultrasound technologists learninghow to scan. Fellows will also spend one fullday working with the mammography techslearning how mammograms are acquired.Half of the day will be for screening and theother half of the day will be for diagnosticmammograms. In addition, fellows will spendtime observing QC procedures.7) COMMUNITY SITES3) SCREENINGFellows will spend approximately 1-2 weeks inone or more of the outpatient centers (BIDNeedham or Lexington). Transportation is tobe arranged by each individual fellow.Fellows will be on the screening rotation atrandom intervals throughout the year. Allfellows should strive to read 30-40 screeningmammograms per screening session. Theycan also read screening mammograms whileon other rotations, in between cases, if thereis time. When you start, you will read outcases with a covering attending. As youprogress, you will read out independentlyand review only the cases for which you havequestions and callbacks.8) BREAST RADIOLOGY PATHOLOGYCORRELATION CONFERENCEThis conference occurs on Wednesday from3-4 pm (except for the 1st week of the monthdue to staff meeting). The Breast Fellows andour Nurse Practitioner, Nancy Littlehale, NP,will alternate presenting at this conference,with occasional conferences given by theresidents4) DIAGNOSTICSFellows will be responsible for interpretingthe images, making managementrecommendations, and discussing findingsand recommendations with the patient andreferring clinicians. When starting, fellows willtake 4-6 cases per session. However, fellowsshould strive to take 10-15 cases per session9) INTERESTING CASE/QA CONFERENCEThis monthly conference occurs the 4thWednesday of the month and includesattendance by Attendings, Residents, CoFellows.6

10) RADIOLOGY JOURNAL CLUBEach fellow will be expected to lead 1-3Journal Club Meetings per year. The JournalClub Meeting includes attendance byAttendings, Residents, Breast Imaging Fellowsand other colleagues.11) TUMOR BOARD AND MULTIDISCIPLINARYCONFERENCESTumor board and multi are two conferencesthat take place every week. Patients’ imaging,pathology, and treatment are discussed withall breast care teams present. The fellows areresponsible for presenting at some of theseconferences.12) MULTIDISCIPLINARY CLASSICS JOURNALCLUBThis will take place monthly with thepathology and surgery fellows. Each groupwill alternate presenting articles.13) HARVARD BREAST IMAGING FELLOWSMEETINGThis will take place 3x per year at will rotateat each of the different fellowship programs.The fellows will receive a lecture and presentcases.14) SECTION/DEPARTMENT/RESIDENTCONFERENCEEach fellow will be expected to give a talk ona topic of their choosing.15) RESEARCH PROJECTSThe Breast Imaging Fellow is required tobe involved in at least one Breast ImagingResearch Project during the Breast ImagingFellowship. This includes a print or electronicpublication (e.g., research or review article,chapter) or a local, regional, or nationalpresentation/poster or electronic exhibit.The Breast Imaging Fellow can work with anyattending in the Breast Imaging Section.16) RESIDENT LECTURESDedicated breast imaging lectures are givento the residents in week long blocks (Mondaythrough Thursday) from 730-9am, 4 timesin the academic year, for a total of 16 daysof resident lecture. You are encouraged toattend these lectures on basic breast topics.17) FELLOWS SUMMER LECTURE SERIESAs part of the didactic curriculum, fellows willhave a dedicated weekly summer lecture serieson basic topics related to breast imaging. Thesewill be geared towards the beginner fellowshiplevel and will set the foundation for yourlearning. Attendings will provide additionalresources for their lecture topic for you to buildyour foundation of knowledge.18) SECTION AUDIT/QA/QCFellows will work together to present thesection’s Breast audit to the group using thesection. There is also opportunity to work withthe technologists to learn about quality controlfor a breast imaging department.19) PROGRESS/EVALUATIONSThe Breast Imaging Fellows will meet monthly forcheck-ins and quarterly for 30-60 minutes as partof formal evaluations with Drs. Jordana Phillipsand Rashmi Mehta.The Breast Imaging Fellow should run her/hisvolumes. These numbers will be reviewed at thequarterly meeting and your feedback on selfperformance will be solicited and attendingfeedback will be provided. Quarterly writtenevaluations will be given and fellows areexpected to provide quarterly evaluations/feedback as requested by the Education office.20) CALLFellows will participate in the Body Imagingcallpool which includes reading diagnostic caseson weeknights, taking procedure call during theweek and a few weekend calls. The callrequirements are equitably divided amongmultiple fellows and should not interfere withbreast imaging training. For those headed to jobsthat require general skills, this is an excellentopportunity to maintain that knowledge. Forthose who are going to solely practice breastimaging, there is no requirement to participate inon-call, non-breast-related procedures.7
